I. THE SCHOOL ORGANIZATION


A.

The CLASS TEACHER takes the role of a parent in school. He/ She has the direct
and immediate authority over the class assigned to him / her for the whole year,
helps children in their studies, and maintains discipline in class.

The CLASS ADVISER is the homeroom teacher who takes charge of student
attendance, report cards and promotions. All specific affairs pertinent to his / her
respective classes and their management are his / her responsibility.

B. Refunds on Tuition and Miscellaneous Fees


The school follows DepEd policy on refunds. Section 6 of the 1992 Manual of Regulations
for Private Schools, Eighth Edition, stipulates that a student who transfers or otherwise
withdraws in writing within two weeks after the beginning of classes and who has
already paid the pertinent tuition and other school fees in full or a fraction thereof
will be charged (10%) of the total amount due for the whole school year. The student
will be charged all the school fees in full if he / she withdraws any time after the 2nd
week of classes except when withdrawal is due to illness. In such case, the student will
be charged the pertinent fees only up to and including the last month of attendance.
Reservation fee is non refundable.

B. The Student Organizations


1. The STUDENT COUNCIL ORGANIZATION (SCO) for both Grade School and
High School is the highest student body of the school. It is composed of class
officers selected in a popular election. The SCO functions as coordinating body
to the School Administration regarding student affairs.
2. The CLASS OFFICERS are elected in class and are responsible to assist the
class adviser / moderator and / or teachers in class management.
3. The CLUB OFFICERS are elected by club members as soon as the clubs are
organized.

II. GENERAL RULES, GUIDELINES AND PROCEDURES


A. Admission
NSDGA has set certain requirements for the admission of new students. Parents,
guardians, students who are seeking admission to NSDGA should express willingness
to abide by the rules and regulations of the school which gives them the rights and
privileges that they enjoy in the school as a Christian community.
In order to gain admission in NSDGA, applicants must meet the following requirements:
1. Eligibility
a. Applicants must pass entrance exams administered by the school.
b. Applicants must have a general average not lower than 80% from his / her
previous school
2. Requirements
a. Photocopy of Birth Certificate (Most recent NSO Copy)
b. 1 1X1 ID Picture
c. Medical History
d. Form 138 / Report Card
e. Form 137 / Permanent Record (original) from school of origin
f. Certificate of Good Moral Character / Certification of

C. Grooming
a. Students are expected to come to school in complete uniform with school logo,
black leather shoes when in daily uniform or rubber shoes when in PE uniform,
and white socks.
b. Students must wear the school Identification card at all times within the
school premises. Students should always tap their IDs to the kiosk located at
the entrance gate to record time of arrival and departure. Loss/breakage of
the identification card must be reported immediately to the Office for Student
Affairs.
c. Every student should come to school with neat fingernails (no nail polish);
proper haircut (for boys); no make - up (for girls). Male students are not allowed
to wear earrings, sport any form of body piercing and other accessories
ordinarily used by females.

D. Attendance
A student who incurs absences more than 20% of the prescribed number of school
days for the school year shall be given a failing grade and will not be given credit for
the course or subject.
If a student has been absent during the previous day/s, he / she must secure an ADMIT
TO CLASS SLIP from the office. In order to secure this slip, a parent / guardian of the
student must accomplish an Excuse Slip in the Students Diary which he / she will
present to the Office for Student Affairs. If the student fails to submit an ADMIT TO
CLASS SLIP he / she will still be considered absent on the day he / she returns to
school.

E. Tardiness:
Students are expected to be punctual in coming to school. They should be in school
fifteen (15) minutes before the start of classes.
A student is considered late in the following occasions:
a. If a student is not present at the start of the flag ceremony;

b. If a student is not present in his / her class after five (5) minutes from the start of
the class.
The school follows DepEd policy that a student is considered absent for one day when
he / she incurs three (3) tardiness.
A late student has to secure an ADMIT TO CLASS SLIP from the Office for Student Affairs
in order to be admitted to class.

F. Sickness
If the student becomes ill and will be absent for three (3) or more days, the parent /
guardian of the student should notify the school and/or adviser. This procedure will
help facilitate giving of make - up quizzes and other graded activities to the student
when he / she returns to school. In case the student becomes ill while in school, the
school nurse will notify the parent / guardian. A student who has recovered from any
contagious disease must present himself / herself before the school nurse and secure
clearance regarding his / her fitness to report back to school before securing an ADMIT
TO CLASS SLIP.

G. Dismissal Time
The school is responsible for the students only within thirty (30) minutes after dismissal
time. Students must be fetched immediately after dismissal time. After dismissal from
classes, no student is allowed to stay inside the classroom. All doors and gates are to
be locked after thirty (30) minutes. Students caught staying inside any classroom will
receive serious punishments from the Head of the OSA.

H. Curfew Time
Curfew time is at 6:00 PM. No student shall be allowed to stay in school beyond this
time unless for valid reasons. During this period, no student will be allowed to enter the
school premises even to get something that has been forgotten.

I. Coming to school beyond class hours and school days.


As a general rule, no student is allowed to enter the school premises beyond
prescribed class hours and school days. Unless for some valid reasons, the
teacher concerned with the activity after class hours or school days shall notify the
parent / guardian regarding the venue, time of arrival to and departure from
school and other particulars of the activity through a Parent Notification Slip. Only
students with this slip duly signed by a parent or guardian will be allowed to enter the
school grounds.

J. Cancellation of Classes
NSDGA follows local government unit advisory on cancellation of classes. In case that
classes are suspended when classes are already on going, the school shall duly announce

the cancellation at least one (1) hour before dismissing the students. The students
shall remain in their respective classrooms with their advisers until they are
properly fetched.

K. Off Campus Permit


Any student who wishes to leave the school before dismissal time must present to
the Head of OSA a written request from his / her parent / guardian for approval.
In case a student becomes ill while in school, the school nurse will make proper
recommendations to the Head of OSA. In both cases, however, the student must secure
first an OFF CAMPUS PERMIT from the Head of OSA before he / she will be allowed by
the school security personnel to leave the campus.

L. Miscellaneous
1. Cellphones Students are strictly prohibited from bringing cellphones to
school.
2. Electronic / Battery operated Gadgets Students are strictly prohibited from
bringing cassette / CD/ DVD/ music players, cameras, tablets, laptops and the
like unless required by a subject teacher as part of the lesson. These gadgets
must be surrendered before and after use to the concerned subject teacher
and will be claimed after dismissal time.
3. Use of school phone Students may be allowed to use the school phone for
emergency purposes but never to call their parents / guardians to bring to
school forgotten things.
4. Buying of food outside the school premises Students are strictly prohibited
from buying food outside the school premises as per directive by the
Department of Education. Students have the options of either bringing their
own food or purchasing food from the school canteen.
5. Special Activities NSDGA has varied activities lined up every school year.
All students, regardless of denomination, must attend all school functions
and activities which complement classroom activities.
6. Educational Tours Educational Tours are scheduled once a year. These
educational tours supplement classroom activities.
7. Taking Medication while in School Parents / guardians should coordinate
with the school nurse if their child needs to take medication while in school.
8. Test Paper Viewing for Parents Periodical Test Papers are given back to
students on a particular day for the parents/guardians to view and sign.
These test papers must be returned to school the next day. Test papers of a
student may be viewed only by his / her parent / guardian.
9. Giving of Gifts to NSDGA Employees Students and their parent are discouraged
from giving gifts to any NSDGA employee. Employees of the NSDGA are
prohibited from accepting gifts from parents and students except during the
Christmas season.

10. Setting Appointments for Meetings / Conferences Parent/guardian must set


an appointment with person concerned via the Student Diary (correspondence
section) and instruct his / her child to show correspondence to person
concerned. Appointments made via phone to NSDGA shall not be honored.
11. Policy on Non School Personnel Parents, guardians, etc. are NOT allowed
to stay inside the school premises. They shall be allowed to wait for or
meet their children in designated waiting areas. Visitors will have to secure a
VISITOR PASS.

B. Examinations
The Pre School, Grade School, Junior High School and High School of the 2010
BEC have four (4) quarterly examinations. Each of which is administered for a
period of three (3) days. An EXAMINATION PERMIT from the Cashiers Office
is secured several days before the examinations and presented to the adviser on
the first day of the examinations. Should a student forget or lose his / her exam
permit on the day of the exam, he /she must secure a new exam permit from the
Cashiers office.
Exam Protocol
1. A student proceeds to the Registrar or Finance Office in the absence of an
EXAMINATION PERMIT during the first day of exams.
2. Necessary information should be filled out on the test papers. All pages should
be checked if they are complete and clearly written.
3. All books and bags must be deposited in front of the classroom.
4. A student should go to the CR before the test begins as no one is allowed to go
anywhere once the test has started.
5. Talking to seatmates, borrowing or passing anything is prohibited once the test
has begun.
6. A student is not allowed to glance sideways. He / she may only look in front, up
or down, or at his paper during the test.
7. Before passing the test paper to the proctor, the student should check if he / she
has answered all questions.
8. After passing his / her paper, a student is not allowed to converse with other
students. No student is allowed to handle another students paper during the
course of the test.

9. It is the students responsibility to check his / her test paper for missing pages
or unanswered parts and inform the proctor.
10. Students who are caught violating any of the provisions of the exam protocol
will not be allowed to continue the test.

2. Behavioural Probation

All new students from Gr. 1 to High School and any old student in Grade School
or in High School with failing marks in GMRC or Conduct and / or who incurred
major infractions in any quarter during the school year are given behavioural
probation. Being on behavioural probation serves as a warning that any further
disciplinary infractions under level 3 or level 4 of the present school year
may have serious implications such as withdrawal from the school / or non
acceptance to NSDGA next school year.

C. Promotion and Retention


1. Grades 1 6
A student must have a final average grade with at least seventy five percent
(75%) and must have passed all subjects at the end of the school year in order
to be promoted to the next grade level. If he/she fails in one (1) or two (2)
major subjects but gets a passing final / general average grade, he / she may
be promoted to the next grade level provided that he / she takes up summer
extension classes and goes into academic probation. Failure in more than two
(2) major subjects and/or a failing final general average grade merits repetition
of the grade level. Failure in two (2) minor subject is equivalent to one (1) major
subject.
2. Junior High School
A student must have a final general average grade of not less than seventy
five percent (75%) and must have passed all subjects in order to be promoted to
the next level. If he / she fails in one (1) or two (2) subjects, he / she must take
summer classes in order to complete the prescribed subjects. He / she will be
given an academic probation. If the student fails to take summer classes, he /
she will be advised to transfer to another school. If a student gets failing grades
in three (3) or more subjects, he / she will have to repeat the grade level and
will not be accepted as a student of NSDGA or will be advised to transfer.
3. High School (2010 BEC)

High school students must have a final general average grade at least seventy
five percent (75%) and must have passed all subject to be eligible to the
next level. Student who fails in one (1) or two (2) units but gets a passing final
general average grade must take up summer removal classes and go into
academic probation to be eligible to the next year level. Students with failing
final general average grade and / or failure in three (3) or more units shall have
to repeat the year level and shall be excluded from NSDGA.

E. Make Up Quiz, Test, Graded Seatwork, Recitation


and Assignment After A Valid Absence Only
On the day a student returns to school after a day of absence or more, he /she:
1. Must present an Excuse Slip accomplished and signed by a parent / guardian.
2. Must present a medical certificate bearing the doctors name, license number
and contact number (including hospital or clinic affiliation) if the absence is
more than 4 days.
3. Is responsible for catching up with his / her missed classes.
4. Must find out if he / she has missed a test, quiz or any graded activity.
5. Must secure a MAKE UP FORM from the Head of Office for Student Affairs and
present it to his / her teacher/s for make- up schedule.

Failure to submit the above mentioned requirements, secure a MAKE UP FORM,


or to show up during the assigned make up schedule shall mean forfeiture of
the privilege to make up for any missed test, quiz and other graded activities.
In such cases, the student shall automatically get a transmutable zero (0) as his /
her grade.

D. Probations
1. Academic Probation
All new students from Gr. 1 to High School and those old students who have
attended summer enrichment / extension courses the previous school year
are all placed under academic probation. Being on Academic Probation serves
as a warning that any more failures in academics during the school year may

IV. OFFENSES AND PENALTIES


A.

Definition of Penalties
1. Reprimand a reminder or warning
2. Jug a corrective measure for infraction of class rules and violation of certain
rules. It may consist of writing, copying or reciting materials assigned by the
teacher or Head of OSA to be done in school after classes at home and signed
by parents.
3. Post is imposed for a serious violation of school rules and classroom
discipline. It consists of undertaking a corrective measure for a definite time
designed and supervised by the Head of OSA.
4. Deprivation of privileges the student is prevented from participating any extra
curricular activity (i.e. Clubs etc.)
5. Ground improvement work work to be judiciously imposed by the Head of
Student Affairs.
6. Behavioural Probation a child may be on probation for a period of time for a
repeated and continued misconduct during which he must prove that he will
observe school rules and regulations.
As stated earlier, any student put on Behavioural Probation is given a warning
that should he / she incur any more disciplinary violation, he / she may be
excluded from the school roll.
7. Suspension is imposed for every serious violation of school rules and
classroom discipline. A student may be denied or deprived of attendance in
classes for a period not exceeding twenty (20%) percent of the prescribed class
for the school year.
8. Punitive Suspension the school prevents the student from attending class
and thus from taking any examination, quizzes, and graded recitations given
during the specified period. The student is not excused from such graded work,
thus, he shall be given a failing mark for not having taking them; nor will he be
given special examinations to make up for them.
9. Preventive Suspension a student under investigation of a case involving the
penalty expulsion may be preventively suspended from entering the school
premises if the evidence of guilt is strong and that the continued stay of the
student during the period of investigation proves to be prejudicial.
10. Exclusion the school excludes or drops the name of a student from the school
rolls for being undesirable, and transfer credentials are immediately issued.
A summary investigation shall have been conducted before the penalty is
imposed.
11. Expulsion an extreme penalty on a student consisting of exclusion from
admission to any public or private school in the Philippines. This may be
imposed for acts or offenses constituting gross misconduct.

3rd Offense Exclusion depending on the offense
Such punishment is subject to a meeting with parents

V. STUDENTS CODE OF CONDUCT


A. Relationship with Superiors
1. Respect and obey your superiors promptly and orderly
2.Greet your superiors upon meeting for the first time during the day, and say
goodbye before leaving for home.
3. Wait at a distance while your superiors are talking until you are recognized and
acknowledged.
4. Offer your help willingly when you see your superiors carrying things
5. Knock before entering a classroom or office
6. Be active and cooperative in all school activities and to be secured in the
knowledge that school activities aim to develop a sense of responsibility,
cooperation and generosity to the students.
7. Avoid idle chatter with school personnel so as not to disturb them in their work.

B. Relationship with Fellow Students


1. Respect the right and property of each and every fellow student
2. Speak kindly and gently with one another. Do not call fellow students names
that hurt their feelings.
3. Extend your help willingly for any good cause.
4. Do not laugh at the mistakes of others. Never mock nor ridicule them.
5. Show consideration and understanding to all.
6. Keep quiet in class, along the corridors, and in the library.

C. Relationship with Family and Countrymen


1. Respect, obey and seek the counsel of your parents and care for your brothers,
sisters and other family members.
2. Offer your seat to older people and the disabled in school, in public vehicles, or
in gatherings and other circumstances.
3. Inform your parents or guardians of your daily performance and activities
honestly.
4. Conduct yourself outside the school with proper decorum.
5. Be sensitive to the needs of people within your neighbourhood and your
community projects.
6. Be aware of the needs of the unfortunate, the poor, the uneducated and strive
to participate in projects that uplift condition.
