I am open from 6:30 am until 5:30 pm. Hours before or after need to be arranged with me prior to the
week of.

Part 4
Tuition
Tuition is due on Monday or first contracted day of care for the week. If tuition is not paid by end of
first contracted day, $5 per day after will be charged, unless you communicate with me. Tuition can be cash,
checks or debit card. Checks are to be made payable to Crystal Helling. Debit cards will be added an additional
$5 for fees. Insufficient checks/bank funds will be billed $35 plus bank fees incurred. No post-dated checks.

Part 5
Holidays
The following 10 holidays will be closed and paid (8 hours). No vacation days can be used for paid
holidays: New Years, Memorial Day, Independence Day & Day after, Labor Day, Thanksgiving Day & Day
after, Christmas Eve & Day, and New Years Eve

Part 6
Vacation/Days Off
Each family has two weeks (10 days) of unpaid vacation/sick days per year, which can be used
combined or separate. Any vacation days beyond that will need to be paid.
I will take up to 5 personal days off per year, which can be used for a sick day or a personal day. This is
to be paid. Any days I take beyond these 5 will be unpaid. No vacation days can be used for the paid day and
no vacation days can be used the week of my paid vacation day. I will give plenty notice for the days I request
off.

Part 7
Illness/Injury
Each family has 10 days off which can be used for sick days or vacation days. Any over the 10 days will
be paid.
Any child with a communicable disease like pink eye or hand, foot, and mouth, will not be allowed to
come to daycare unless doctors approval and antibiotics for at least a day. Runny nose and coughing does not
mean a child cannot come to daycare. Please use your best judgment. If your child becomes ill at daycare
(vomiting or lethargic) will result in a call home to be picked up within the hour.
Any minor injuries (cuts, scrapes, bites) will be given first aid. Any major/serious injuries that require
medical attention will be sent to ThedaClark in Neenah. Any fees that are incurred with calling emergency
vehicles or emergency room fees will be responsible by the parent. Parents will be notified immediately when a
serious injury occurs.

Part 8
Meals
Mealtimes will be breakfast: 8:00am, lunch: 11:00am and snack: 3:00pm. Breakfast will be cereal or
pop tarts. Lunch will be a main food (sandwich, mac n cheese, etc.), a fruit and a veggie (I have the option for a
veggie but more children refuse to eat them). Snack will be a healthy treat. All meals will be given with juice or
milk.

Part 9
I understand that
I must follow this contract that has been provided. Parent or Provider may terminate this contract at any
time by providing a written and paid 2 week notice. If parent breaches contract, he/she will be liable for all
collection and court costs involved and be terminated immediately. This is a legally binding contract and agree
to abide by the guidelines outlined herein.

This contract is only good through December 31, 2014.
