Respected Sirs,

I had submitted an application under RTIA 2005 to PIO/Ms. Rashmi Krishnan, Deptt. Of Health & FW, Govt. of
NCT of Delhi on May 8, 2009 vide ID:1264(H) asking for information regarding action taken on the
letters/reminders as mentioned below to various officers of her Deptt. (COPIES ENCLOSED FOR READY
REFERENCE).

“Letters & reminders dt. Aug. 28, 2008 and April 10, 2009 to Principal Secretary, Health; October 10,
2008 to Dr. Parveen Kumar, SPO; January 8 and 20, 2009 to Director, Deptt of Health, Kadkarduma;
and dt. May 6, 2009 to MS, DDUH – faxed, emailed and personally delivered to all concerned officers --
requesting for urgent action on the subjects: Issues Related to DGEHS, Grievances of OPD Patients
at Delhi Govt. Dispensaries, Comparatiave Statement of the Facilities provided by CGHS V/s
Delhi Govt. Health Scheme and general problems at DDUH”

Excepting partial information from Dr. Dharam Prakash, CDMO(West), no information as asked for
from PIO was received within the permissible time limit of 30 days. Hence, an Appeal to Mr. JP Singh,
Principal Secretary and FAA in Deptt. of Health & FW, was sent on JUNE 16, 2009 followed by
reminders dt. July 13 & 30 and Aug. 4, 2009 (copies enclosed for ready reference). PIO, Dte. Of Health
Services vide letter No.F.No.RTA/Addl. DHS.HQ/33665 dt. 9.7.09 asked me to appear before lthe
Appellate Authority, Director, Health Services, on 24.7.09 at 11.30 AM. Mr. JP Singh, FAA, was
informed vide my letter dt. July 13 that owing to serious condition of my father, who was hospitalised, I
won’t be attending the hearing at Karkardooma. Dr. BS Banerjee, PIO/Addl. Secretary (Health) vide his
letter No.F1/ID/RTI/TRC/H&FW/2009/4259-60 dt. August 12, 2009 to FAA/Director, Dte. Of Health,
Karkardooma, Delhi-32 gave past references of their letters dt. 18.5.09, 25.6.09, and my appeal/letter dt.
15.7.09 and 4.8.09 had requested to provide the information asked for in my Appeal under intimation to
him.

Dr. M. Lal, Director, Health Services, without taking congnisance of my prior intimation through
Principal Secretary, Mr. JP Singh, wherein I had informed that owing to serious condition of my father,
who remained in ICU of Mata Chanan Devi Hospital and underwent dialysis, I will not be able to attend
the hearing on July 24 at Karkardooma. My father is 86 yrs. Old (bed-ridden) with amputated lower
right limb below knee and cannot be left unattended even for an hour owing to his multiple chronic
ailments.

The information as asked for in my RTIA application and Appeal has not been provided and due to my
remaining absent on 24.7.09 hearing, my Appeal has been disposed off vide letter of Dr. M. Lal,
Directror, Health Services, vide his letter F.No.Addll. DHS, DHS(HQ)/38388/38390 dt. August 3, 2009.

You are requested to kindly treat this letter of mine as MY APPEAL TO CIC and have the information
as asked for my my original RTIA application and Appeal provided from Mr. JP Singh, Principal
Secretary & FAA, Deptt. of Health & FW, Govt. of NCT of Delhi, Delhi Sachivalaya, IP Estate, NEW

PS: On going through the list of RTIA applicants/Appellants under Deptt. of Health & FW at Delhi
Govt.’s site, I neither found my name with RTIA Application ID nor Appeal. This has been despite
orders vide No.F13/1/05/AR/2897-3011 dt. 6.5.08 signed by Mr. Naresh Kumar, Asstt. Director(AR),
Tele.:23392421 by Deptt. of Administrative Reforms (Govt. of Delhi), andMinistry of Personnel, GOI,
vide Notification dt. 27.3.08 signed by Mr. KG Verma, Director, Tele.23092158, have not been
uploaded. This can be verified from the website of Govt. of Delhi. There is also no mention of RTIA
2005 in the website of Delhi Govt.’s Deptt. of Health & FW and Addl. Secretary-II as PIO and
Principal Secy.(Health) have been designed as FAA in comprehensive list of PIOs/FAAs in the website
of Delhi Govt. This fact may also be got verified.

This has reference to a letter No.RTI/ADDL.DHS/HQ/33665 DT. 9.7.09 from PIO(no name mentioned),
Dte. Of Health Services, Kadkardooma, marked as Most Urgent under RTI Act, ID No.1264(H),
requesting me to appear before the Appellate Authority, Director, Health Services, on 24.7.09 at 11.30
AM

As Health Deptt.’s site has no mention of RTIA 2005, Delhi Government’s Website gives complete list
of PIOs and FAAs in different Departments. Mrs. Rashmi Krishnan, Addl. Secy.II, has been named as
PIO and you as Principal Secretary, have been named as FAA under RTIA 2005. Thus, my Application
under RTIA dt. May 9, 2009 was sent to Mrs. Krishnan. She forwarded my application to PIO, Dte. Of
Health, Kadkardooma vide her letter dt. 18.5.09. After not receiving the information, an appeal was
faxed to you on June 16, 2008 which too was forwarded to FAA, Director, Dte. Of Health, by Mrs.
Krishnan vide her letter dt. 25.6.09.

PIO, Dte. Of Health Services, without going through contents of main Application and enclosed
letters, forwarded it to PIO, Ministry of H&FW, GOI, Nirman Bhawan. This was sent by Min. of
H&FW to Director, CGHS, and in their reply, Addl. Dir.(CGHS) informed that they have
nothing to provide. In fact, Ministry of H&FW or Director, CGHS, did not have any information
which was asked for in my RTIA application. Perhaps a statement entitled ‘COMPARISON
BETWEEN CGHS AND DGEHS DISPENSARIES’ sent to you and Director, Dte. Of Health,
along with my letter/reminder dt. August 28, 2008 and April 10, 2009 was by mistake taken as a
GOI document. Dr. Praveen Kumar, in one of his letters, sent to me had informed that no
information has been asked for in my RTIA application.

I must place on record appreciation of our Association as partial information asked in my RTIA
application has been received through a hand-written letter dt. 25.6.09 from Dr. Dharam Prakash,
CDMO(West District) of your Deptt. whom copy of my RTIA application might have been sent. While
conveying the decisions taken at his end, for other issues he made it clear that these are to be decided by
higher authorities in Deptt. of Health.

You are once again requested to kindly have some one deputed to go through contents of my
original RTIA application along with the enclosed letters so that specific information desired
could be provided without delay. I won’t be able to attend the meeting at Dte. Of Health on 24th
July as my father is serious and is admitted in the ICU of Mata Chanandevi Hospital, Janakpuri,
since July 5.

I am awaiting the information within the stipulated period of 30 days from the date of my Appeal to
FAA which is June 16, 2009.

As no information in response to my application dt. May 8, 2009 has been received from PIO, Mrs.
Rashmi Krishnan, Addl. Secy.II, or any other Department/Division of Deptt. of Health & FW, my
original letter may please be treated as MY APPEAL TO FAA UNDER RTIA 2005. It is reproduced
below:

Dr. Khosla, Addl. CDMO(West) had telephonically informed us last year that sanction for providing
RO water filteration units at all West Delhi Dispensaries has been obtained and the machines will be

installed soon. However, the fact is that the DJB water supply at C-2 Block, Janakpuri, Dispensary has
been disconnected and at Tihar Complex the water is being arranged from nearby houses in insulated
plastic campers only for use by staff. There is no drinking water kept for patients in any of these
dispensaries at the time when Delhi temperature has crossed 40 degree C. Senior Citizens, Disabled
patients, ladies with infants in their laps, school children visiting these dispensaries remain thirsty with
no water to drink.

You are requested to provide information under RTIA 2005 within 30 days regarding actual
action taken on all the above mentioned letters (copies were enclosed with my original
application) between the dates these letters were received till date of despatch of the information
being asked for, specifically action taken regarding provision of potable water to patients visiting
dispensaries.

Respected Sirs,

This has reference to DGEHS which has been made compulsory for all Delhi Govt. employees and
optional for pensioners. The medical cards issued to the Employees are required to be registered with
Delhi Govt. Dispensaries. The existing dispensaries were originally meant for general public but with
additional load of Govt. employees, it is becoming difficult to attend to the patients in OPD as the
number has increased manifold. A number of pensioners have also become life members of this Health
Scheme.

Health Department has been trying to bring the Delhi Govt. Dispensaries at par with CGHS
Dispensaries. Lot needs to be done in this regard. As you are kindly aware, DDUH is attending to OPD
patients more than double of its capacity and there is no respite for the patients who have no other option
but to stand in long queues first for registration and then outside the doctors’ rooms for hours. The
corridors, common waiting rooms, labs, dressing/injection rooms, emergency as also doctors rooms are
crowded beyond their capacity which pollutes the environment. Additionally, there is a regular flow of
patients from Tihar Jail, MLC patients brought by Delhi Police which add to the bursting capacities of
this very famous Hospital of West Delhi. On visiting this hospital with a dog-bitten patient on 26th
August, while moving from New Emergency Wing to Injection Room, I witnessed the mad rush at each
and every covered and open area of this hospital, specially I saw stray dogs moving freely in corridors of
this Hospital in the presence of Security Staff.

Your Ministry must already be taking remedial measures to overcome difficulties a picture of which has
been tried to be projected in above paragraphs. To supplement your efforts, an exercise has also been
done to jot down a ‘Comparative Picture’ of the CGHS vs. Delhi Govt. Dispensaries and a copy of
this is being sent with this letter for your kind perusal. I also offer my services for any work
assigned to me in the matter or for getting associated with any such group formed for the purpose.

I shall be grateful if this grievance letter is acknowledged first and the follow-up action taken is
communicated to the undersigned.

SUB: SIMPLIFICATION OF MEDICAL SCHEME PROCEDURES

Dear Sir,

This is in continuation of our earlier letters addressed to Principal Secretary, Deptt. of Health, on various
issues including submission of a ‘Comparative Statement’ of CGHS and DGEHS. A number of
procedural difficulties have been experienced by the pensioners while taking benefits of DGEHS. Some
of these are given below for your perusal and necessary action.

1. A medical card beneficiary after consulting Medical Specialist of a Delhi/Central Govt. Hospital
is required to visit the Dispensary to which he/she is attached. Here the medicines/tests
prescribed by specialists are re-written on OPD slips by Doctor In-charge.
2. The dispensary Chemist tick-marks and writes ‘N.A.’ on the medicines not available in the
Dispensary. Now the beneficiary (a patient) is asked to get two photocopies each of the Hospital
prescription, Dispensary OPD slip and Medical Card. In the case of Tihar Jail Complex
Dispensary, the photocopying facility is not available nearby and one is required to go to
Janakpuri ‘C’ Block.
3. Once these photocopies are submitted, the medicines are indented from DDU Hospital. The
patient cannot start taking medicines prescribed by Specialists the same day as after submission
of the required documents, he is asked to visit the Dispensary on the third day to take delivery of
the medicines indented.
4. In the case of medical/pathalogical tests/X-ray/ECG, etc. the Dispensary requires two sets of
Hospital Prescription Card/Medical Card and another set of the same is required by the
approved Laboratory/Diagonistic Centre.
5. If more than one Medical card beneficiaries have to get medicines/carry out tests, each one of
them is required to follow the above mentioned procedure individually.

One fails to understand, when a Medical Card is registered with a Dispensary and all particulars are
recorded in their register, why have they been instructed to take an extra copy of medical card and
Specialist’s prescription for their records/retention.

You are requested to look into the patients’ grievances as mentioned above and take appropriate
decisions expeditiously. Procedures may be simplified to the extent possible and modifications may

immediately be informed to Dispensaries and CDMOs all over Delhi. We are sure, by now some efforts
must have been made to bring the DGEHS at par with CGHS on the basis of the comparative statement
submitted to you earlier.

SUB: GRIEVANCES OF MEDICAL CARD HOLDERS

Dear Madam,

This is in continuation of our earlier letter dated January 8 faxed to you, Dr. Praveen Kumar, Dr.
Prashad, MS, DDUH, Dr. S.D. Khosla, CDMO(W) and a copy of which was personally handed over to
Dr.Archana, MOI, Delhi Govt. Dispensary at Tihar Complex, on the subject mentioned above.
Another letter was sent to Dr. Praveen Kumar on October 10, 2008 highlighting the cumbersome
procedures laid down by your Deptt. for taking the benefit of Medical Card Facility.

In yet another communication emailed and faxed to all above named officers in Deptt. of Health, a
Comparative Statement giving details of the benefits offered under CGHS and DGEHS, the
Association awaits action at your end. The Delhi Govt. employees and pensioners have been facing
lot many problems at their Dispensaries/Hospitals which needs urgent action. May be the decision
taking process is slow and the present policies requiring change are being continued resulting in
delays in getting the medicines prescribed by the Medical Specialists, approved Hospitals charging
heavy OPD registration amounts from the Medical Card holders, etc.

The approved pathological laboratories are asking for ‘Essentiality Certificates’ from the Card
holders. Thereafter, these labs are required to run from the Offices of the beneficiary employees to
their District Offices and some times even to different departments of Delhi Govt. It has been
noticed that the present procedures result in exorbitant delays in disbursement of payments of bills
of these labs and approved hospitals and thus they show reluctance in attending to DGEHS.

The medicines prescribed by Govt. Hospitals are not indented and issued stating the reason that
these are diet supplements. It is surprising that different hospitals of Delhi Govt. are treating
Medical Card holders differently, e.g., Shellcal and Ecored are being issued by Guru Gobind Singh
Hospital at Raghubir Nagar whereas these are being denied at DDUH. The pharmacist at DDUH
insists that unless a medicine is specifically mentioned for ‘therapeutic use’, it will not be issued.

You are therefore requested to have the above stated grievances looked into urgently in
consultation with all the concerned officers associated with Delhi Govt. Employees’ Health Scheme
and issue necessary orders to all the Hospitals, CDMOs and the Dispensaries so that the present

procedures are simplified to the extent possible and paper work, as far as possible, is avoided at
the Dispensary level with the use of computers and soft copies of the documents.

All the medicines as prescribed by Medical Specialists of Govt. Hospitals, e.g., SJH, and indented by
the Dispensary for issuance by DDUH are not given stating that these are 'DIET SUPPLEMENTS
'whereas these very medicines are not only issued by the CGHS Dispensaries but are also got
locally purchased through their approved suppliers. To name a few, these include NEUROBIN
FORTE, GELORED CAP, SHELCAL CT 1. These were prescribed as essential along with blood
pressure medicine tablets, TELMA H. On enquiring from pharmacists issuing these medicines at
DDUH, we are told that after these medicines are prescribed as 'essential', the MO at the
Dispensary has to write them as essential and then forward for the approval of CDMO(W), Dr.
Khosla. After this the case will be referred to MS of DDUH and only when he agrees to issue these
medicines, the same will be issued to the patient. You can imagine, how much time this whole
process will take. Twice these have not been issued stating the above stated reason.

Please have this case looked into and have the present procedure simplified so that these
medicines now denied stating as 'diet supplements' are issued along with other medicines to the
patients holding medical cards.

Two letters have been sent to Dr. Praveen Kumar, In-charge, Medical Cards, in Deptt. of Health
giving a comparative picture of the facilities to CGHS beneficiaries vis-a-vis Delhi Govt. Health Card
beneficiaries, as also the practice of submission two sets of photocopies of prescription slip/OPD
card from Medical Specialists of Hospitals, Dispensary OPD slips mentioning the medicines as ‘Not
Available’, copies of the prescriptions where path. Tests/X-rays have been advised as also the
Medical Health Card. There being no photocopy facility within the Tihar Jail Complex where the
Dispensary is located, patients are required to go to Janakpuri. This needs to be looked into and
the practice being followed at CGHS Dispensaries be followed which also includes use of computers
and where no photocopies are required to be submitted.

There is no facility of getting any prescribed medicines on urgent basis at the time when
Dispensaries are not open. CGHS has emergency facility 24 hrs. and any medicines not available
with these doctors are got issued through their approved chemists instantly by issuing an Authority
letter.

On visiting your Hospital’s OPD Regn. counter, one finds a notice displaying the timings as 9-12 (Mon.-Fri.) and 9-
11 on Saturdays. In the website, this needs to be incorporated as the OPD timings given there are 9-1.00 and 9-
12 for Mon.-Fri. and Sat. respectively. The boys manning Regn. counter do not open the window at 9.00 am
sharp; there is a long queue before this and they shut window sharp at 12.00.

There is no separate queue for senior citizens or ladies carrying infants in their laps. The Medical OPD rooms,
e.g., outside 10-G, just 20-25 people can be accommodated lin the fibre glass shed while others numbering over
50-60 are found standing in scorching heat in the open. As you know the temperature these days is above 40
degrees and some alternative arrangement needs to be made urgently so that the patients are not required to
stand in long queues in the open.

An old man is found selling cigarettes, tobacco products kept on a small table just outside the wall of the gate
close to telephone exchange. This has been brought to the notice of former MS, Dr. Prashad as also the DMS,
Mr. Kalra. But no action seems to have been taken by any one from the Hospital. The Security Staff may be
instructed not to allow tobacco products sale just outside the hospital as this is a crime.

Dental OPD in the Hospital is being run from a very conjested area where what to talk of patients, the doctors on
duty also do not find place to stand or attend to the patients. The Dengu Ward is vacant and I understand this
OPD is proposed to be shifted there. Please expedite this effort so that the patients and doctors can be
confortable.

Lastly, after the Medical Cards made compulsory for the serving staff and optional for retired staff, there is great
rush of Delhi Govt. employees at this Hospital. There needs to be separate counter for registration and Card No.
must also be printed on the OPD card so that these beneficiaries get some priority over others. This has been the
practice in Central Govt. hospitals where even there are special doctors for different ailments only attending to
CGHS card holders. This may please be looked into and action be taken accordingly.
