1. REGISTRATION PERIOD
1.1. Pre-registration (PAMET Offce): Oct. 15, 2014 to Nov. 15,
2014. THERE WILL BE NO EXTENSION.
1.2. On-site (PICC): Nov. 30, Dec. 1 and 2, 2014 (7:00 AM -5:00
PM) and Dec. 3, 2014 (7:00 AM- 12:00 NN).
2. MEMBERSHIP DUES must be settled frst before registration.
2.1. For EXISTING MEMBERS: Membership should be updated
for the period of 2015.
2.2. For INCOMING NEW MEMBERS: Application Form for PAMET
Membership must be properly flled-up and submitted to
PAMET, for processing, together with the PRC ID.
3. PROCESS
3.1. The Convention Registration Form must be properly flled-up.
3.2. The UPDATED PAMET ID must be presented.
3.3. The payment of the applicable registration fee(s) must be
paid either personally at the PAMET Offce in Makati or
through online via Metrobank (GT-Tower Makati). Payment
via online, only for PRE-REGISTRATION, must be done only
from October 15 - November 15, 2014.

4. ENTITLEMENT: Full convention fee entitles the delegate to:

THERE WILL BE NO REPLACEMENT FOR LOST MEAL TICKETS,
SOUVENIR PROGRAM OR ANY COMPONENT OF THE KIT.
5. CERTIFICATE OF ATTENDANCE will be issued at the end of the
convention upon submission of evaluation form and CPD answer sheet
6. CANCELLATION & REFUND
Cancellation of registration must be made on or before November
21, 2014. Submit a cancellation letter and surrender the Offcial
Receipt and the convention kit at the PAMET Offce. 50% of the
payment is refundable when cancellation is made before November
21, 2014. NO REFUND WILL BE MADE AFTER NOVEMBER 21, 2014.
NO LETTER, NO REFUND. Refund, to be released through CHECK ONLY,
IN THE NAME OF THE PAYOR, will be processed only after the convention.
Processing will take at least one (1) month. NOTICE THROUGH
EMAIL when the check can be released will be sent to the payor.
