CHESTQUESTION 1
A 22-year-old woman was found unconscious in a pool
of vomit with an empty 1-qt bottle of bourbon nearby.
She was taken by ambulance to the ED where she was
endotracheally intubated. Vomit was found in the
posterior pharynx during intubation. In the ICU, Sao2
was only 88% despite receiving 100% oxygen by
mechanical ventilation (assist-control mode, set
volume of 400 mL, set rate of 12/min, PEEP of 5 cm
H20, and Fio2 of 1.0).

Physical examination findings are remarkable for a
healthy-appearing female who is deeply sedated and
chemically paralyzed. Her height is 61 in (155 cm) and
her weight is 135 Ib (61 kg). Her BP is 80/50 mm Hg,
heart rate is 130/min, and respiratory rate is 12/min
(all controlled). She has diminished breath sounds and
dullness to percussion of the left hemithorax. The
endotracheal tube is positioned at 25 cm at the
