1 Introduction
This document guides in data entry of the prescribed forms. There are 4 forms numbered as
Forma-I, Form-II, Form-III and Form-IV. These forms are to be used to enter the data required to
be submitted by all Heads of the Departments.
2 Access and Roles
Navigating to the AP Reorganisation Portal
To access login page of this application, Open any of the below URLs in any web browser:

Click on Official Login button at the above screen, then below login screen gets displayed.

3 Home Page
On the Home Page of the application user will come across 5 options as follows:
Home
Forms
Reports
General
Logout
Click on Forms options to access the 4 Forms








3.1 Form -I :HOD Details


Upon Clicking on Form-I HOD Details option, AP State Reorganisation HOD details screen gets displayed as
shown below:

2 Whether included in Schedule IX or X? Select one of 3 options
3 Whether a Constitutional/Statutory body? Select (Yes/No) Option
4 Whether Presidential Order has been applied?
(Answer 'Yes', if local cadres have been organized.
Or else, 'No')
Select (Yes/No) Option
5 Does the Department have field offices? Select (Yes/No) Option
6 Whether any Specified Gazetted Categories, notified
under the Presidential Order?
Select (Yes/No) Option; If Yes,
click on Add and enter the details
7 Whether any Categories of Posts are exempted from
the organization of local cadres?
Select (Yes/No) Option; If Yes
click on Add and enter the details
8 Whether any multi-zonal cadres transcending the Select (Yes/No) Option; If Yes,

3.2 Form-II : State Cadre Posts

Upon Clicking on Form-II State Cadre Posts option, Form II screen gets displayed as shown below:

Data Entry Steps:

1 Name of the Service
Example: AP Social Welfare Service; AP
Tribal Welfare sub-service etc.
Enter the Name of the Service
2 Name of the Post(As per Servcie Rules) From the drop down list, select appropriate name
of the post
3 Type of Post
Explanation: State-wise posts include state
cadre posts spread across the state; State-
level posts include which are only in the
Head Quarters of HOD, SLO/SLI etc.
From the drop down list, select appropriate name
of the post
4 Prescribed age of Superannuation
Explanation: This field will have 58 in
general; for some posts like office
subordinate etc. it will be 60 years
Enter appropriate age of superannuation for that
particular post
5 Grade

Select One Cadre from the 3 options: Gazetted;
Non-Gazetted and Last Grade
6 Pay Scale(PRC-2010) Select Pay Scale from the given list
7 Sanctioned Post Details Enter Filled,Vacant details at Sanctioned Post
details
8 List of Employees in the order of Seniority
8.1 Whether Seniority is published Select Yes or No - If the list is published select
Yes, else No
8 .2 Year of Publishing Enter the year of publishing
8.3 Whether in dispute Select Yes if there is any court case or any
representations by the employee (s) or else enter

8.4 Remarks on Seniority If any remarks related seniority, please enter here
9 Vacancies Enter number of vacancies; carry forward
vacancies in case of SC/ST under 3 indicated
categories. Any category that is not applicable,
enter zero
10 Employee Statistics Provide details of all employees who are on leave
working inother deaprtments , under suspensions

Data Entry Steps:

1 Name of the Post This is auto generated based on the post entry in
Form II
2 Employee ID (Allotted by DTO/PAO) Enter Employee ID provided by DTO/PAO; Other
IDs not acceptable; employees without DTO/PAO
issued IDs need to procure one
3 Name of the Employee(As per the Service
Register)
Enter the Name of the Employee
4 Name of the Office Enter the Name of the Offcie
5 Place of Office Enter the Place of the Offcie (Town/City etc.)
6 District of Office Select District of the Office from list given
7 Status of Employee From drop down list, select appropriate status of
the employee
8 Date of Birth Enter the Date of Birth in the form of DD-MM-
YYYY
9 Mobile Number
This will be used to send out SMS and
seeking any information
Enter employee's current mobile number
10 Gender Select Male or Female option
11 Martial Status Select one of the drop down 4 options given
12 Social Status Select Social status(Caste & Sub Caste); Based on
the selection sub-castes in the next box needs to
be selected. Example; If chosen SC, in the next
box appropriate sub case needs to be selected
13 Local Candidture(As pe the Service
Registered)
Select either TS or AP; If chosen None of them,
enter the home district details in 13.1
13.1 Home District When chosen None of Them in 13, select one of
the drop down district as home district. Home
district is defined as either birth place district;
birth place district of Employee's Father; district
where immovable property in the name of
employee prior to 1.6.2014 is available

14 Details of First Posting Enter following fields
14.1 Date of Entry in to service Enter Date of Entry in to the servcie in the form

14.2 Designation of the First Posting Enter the designation of the first posting
14.3 Whether Confirmed Selecte Yes or No
15 Further details of present Post held in the
Department
Enter the follwoing fields
15.1 Serial Number in the seniority list Enter numeric number of employee seniority;
system will not accept same seniority serial
number
15.2 Category of Recruitment Select appropriate category from drop down list
15.3 Type of Appointment Select appropriate category from drop down list
16 Remarks Please enter any remarks that the employee like
to enter that are not captured in the above fields
Submit Click on Submit button for saving the data

Upon Clicking on Form-IV State Cadres option Forms menu, Form IV screen gets displayed as shown
below:

Name of the Post(As per the Service Rules) Enter details of the Name of the Post
Grade Select One Option from 3 options: Specified
Gazetted, Non-Gazetted and Local Grade provided in
the list

Successor State of Telngana(TS) Enter Numeric data at Fiiled and Vacant Text boxes

No.of Local cadre Posts at the State Level Enter Numeric data at AP and TC Text boxes
Save Data Click on Save Data button for saving the data
