OBJECTIVE RECORD SHEET
For OCTOBER 2014 and ACCOMPLISHMENT REPORT
FOR THE MONTH OF SEPTEMBER 2014

I. OBJECTIVES (SMART)
A. INSTRUCTIONAL LEADERSHIP (30%)
1. Shall check daily plans of teachers
2. Shall observe at least 10 classes in K to 12. Health classes and Science
classes.
3. Shall conduct post observation of teachers being observed.
4. Shall conduct informal visits to all Grade 5 and 6 classes.
5. Shall observe the provision of differentiated instruction to address
needs abilities and interest of all types of learners.
6. Shall monitor the reading level of pupils.
7. Shall monitor the mathematical ability of Grade 3 pupils.
8. Shall participate in academic and academic competition in the
district e.g Science Quiz Bee , YES-O Camp, HEKASI and Math,
DAMATH and Sports Competition
9. Shall encourage pupils to continuously register Boy and Girl Scout of
the Philippines.
10. Shall monitor the utilization of TGs and LMs in Grade 1,2 and 3.
11. Shall utilize summative assessment.

B. LEARNING ENVIRONMENT
1. Shall create an environment that promotes fairness among pupils,
teachers in appreciating ones achievement in their own diverse
capacities and resources.
2. Shall monitor the provision of interventions to address learning
problems of the students.
3. Shall monitor anti-bullying policy.
4. Shall monitor Gulayan sa Paaralan for Feeding Program
Emplementation.
5. Shall plant atleast 10 malunggay trees per month.

C. HUMAN RESOURCE MANAGEMENT AND DEVELOPMENT
1. Shall observe 100% attendance in all school/ district related activities.
2. Shall encourage at least 1 teacher to enroll in graduates studies.
3. Shall encourage staff conference once a month.
4. Shall encourage teachers to conduct action research.

D. PARENTS INVOLVEMENT AND COMMUNITY PARTNERSHIP
1. Shall involve at least 75% of the PTA to support GULAYAN SA
PAARALAN.
2. Shall encourage stakeholders to pupils would be PARDOs.
3. Shall coordinate with Master Teachers in identifying problems in school

E. SCHOOL LEADERSHIP MANAGEMENT AND OPERATIONS
1. Shall manage the MOOE judiciously to improve academic and
physical facilities.
2. Shall submit accurate financial liquidation on time.
3. Shall prepare/ submit all accurate required reports.
4. Shall craft workable and comprehensive SIP/AIP
5. Shall check/monitor teachers/pupils absences
6. Shall identify Pupil at risk of Dropping Out (PARDO) of the school
7. Shall assign teachers as manager in special assignment.

IV. Previous Month Accomplishment
1. All teachers were observed
2. 45% pupils who are non-readers were given a special reading class.
3. 40% slow numerates were given a remediation by the math teacher.
4. 100% potential athletes participated the school,district and area sports

5. All teachers were observed in their teaching strategies

V. Felt Needs/Problem Met
1 unit classroom needs minor repair
Recommendation:
Submit program of works to LGU for funding.
