Dragi colegi,

Am deosebita onoare de a v ura Bun Venit! la cea de-a patra ediie a Conferinei
Naionale Alzheimer.
Tema central a Conferinei din acest an este Demena n Romnia - pe agenda
public?. Am ales acest subiect de dezbatere datorit necesitii urgente de
mbuntire a condiiei persoanelor vrstnice din ara noastr i n special a celor mai
vulnerabili.
n pofida reformelor continue din ultimii 22 de ani, sistemul nostru sanitar i social se afl n criz profund,
aa cum reiese din analiza numeroilor indicatori precum sperana de via, mortalitatea evitabil, ponderea
bolilor cronice etc.
Perioada de criz pe care o traversm nu poate fi ns la infinit o scuz pentru neintroducerea pe agenda
public a problematicii pacientului cu tulburri neurocognitive.
De aceea, n cadrul Conferinei, la iniiativa Societii Romne Alzheimer, va fi lansat Planul Naional pentru
Demen, la a crui elaborare au contribuit membrii Alianei Naionale Alzheimer i care va fi propus spre
adoptare guvernului Romniei. Sperm ca activitatea noastr susinut de lobby de peste 20 de ani s contribuie
la decizia guvernului de a dezvolta o strategie naional n domeniul demenelor i bolilor neurodegenerative.
V invitm s fii alturi de noi!
Ca i n ceilali ani, Conferina va cuprinde un curs de neuropsihologie, workshopuri i prelegeri cu teme de
larg i actual interes, cum ar fi: politici europene n domeniul demenei, criterii actuale de diagnostic pentru
clinician i pentru cercettor, neuroimagistic, neuropsihologie, markeri biologici n demene, nevoile specifice
ale pacienilor suferinzi de tulburri neurocognitive majore, aspecte legislative privind aprarea drepturilor
suferinzilor, prevenirea abuzurilor i combaterea fenomenului de stigmatizare.
V ateptm cu drag s participai la Conferin i s meninem astfel tradiia acestui eveniment deosebit!

Dear Colleagues,
It is a great honour to welcome you to the fourth edition of the National Alzheimer Conference.
The main topic of the Conference is Dementia in Romania - on the Public Agenda?. This issue has been
chosen due to the urgent need to improve the condition of elderly people from our country and especially of
those most vulnerable, those suffering of dementia.
Despite the continuous reforms from the past 22 years, our health and social system are in a deep crisis, as
it results from the analysis of numerous indicators such as life expectancy, avoidable mortality, ponderable
proportion of chronic diseases etc.
The crisis period cannot be indefinitely an excuse for not introducing dementia on the public agenda.
This is the reason why, at the initiative of the Romanian Alzheimer Society the National Plan for Dementia will be
launched and then it will be sent to the Romanian Government in order to be adopted. The plan was elaborated
by the members of National Alzheimers Alliance with a significant contribution of the medical professional
associations.
We hope that our activity of lobby and effective campaigning in the last 20 years will have an influence on the
governments decision to develop a national strategy in the field of dementia and neurodegenerative diseases.
We invite you to join us!
As in previous years, the Conference will include a course of Neuropsychology, workshops and plenaries on
up-to-date topics of great interest, such as: European politics in dementia, recent diagnosis criteria for both
clinician and researcher, neuroimagistics, neuropsychology, biological markers in dementia and neurodegenerative
disorders, specific needs of the patient with major neurocognitive disorders, legislative aspects regarding the
sufferers rights, abuse prevention and fighting against stigmatization.
We look forward to welcoming you to this Conference, to keep the tradition of this outstanding event!

More people are getting old and the elderly are getting older. Increased life expectancy is a worldwide trend.
This is due to a number of reasons: better hygiene, much higher survival rates of new born children, better
health care and prevention and management of infectious disease, but also economic growth and prosperity
for larger parts of our populations. Many grow old healthy, but this trend also means more people will develop
Alzheimers disease and other dementias, for which age is by far the main non-modifiable risk factor.
Dementia has a huge impact on dependency. Dependence is defined as the need for frequent human help or
care beyond that habitually required by a health adult. Currently 1/3 of people who need care are elderly, but
by 2050 that will be almost half of them. Increases in numbers of dependent older people will be particularly
dramatic in lower and middle-income countries.
Almost half of the older people who need personal care globally have dementia, while four-fifths of older people
in nursing homes are people with dementia. This means that long-term care for older people is mainly dementia
care. Governments and policy makers should pay much more attention to dementia as the main underlying
condition for needs for care. This is more urgent if we take into account the large number of people who already
have dementia and the expected increase in the next decades.
A recent study, published just before the G8 Summit on Dementia in December 2013, shows that the numbers
are increasing even more badly than we thought a few years ago. From a current global number of 44 million
people living with Alzheimers disease or other form of dementia, we expect an increase to 76 million by the
year 2030 and 135 million by 2050, unless we find a cure or treatment that can delay the onset of the disease.
Europe already has an older population and now 25% of the cases occur in this part of the world, with an
expected increase of 90% by 2050. But more steep increases will be seen in the relatively younger continents
with the Asia Pacific region (228%), the Americas, especially Latin America (248%) and Africa (344%).
Dementia is now the most neglected health care challenge, according to Professor Peter Piot, the former
director of the United Nations HIV/Aids programme. And neglecting the problem is actually the worst thing
to do. Australian research shows that the cost of people with dementia staying in hospital is 50% higher than
those who stay with the same reason but dont have dementia, often because dementia is not recognised and
makes other diseases more difficult to treat as well. There is limited research evidence available, but according
to the World Alzheimer Report 2013 it is cheaper to diagnose people with dementia timely and try to manage
the disease well, including caregiver support, compared to doing nothing and waiting for a crisis.
The way to manage dementia well is by organising the whole continuum of care during the disease pathway.
That means diagnosed people timely and possibly early, followed up with post-diagnostic support, including
medical and psycho-social treatments. Examples of these social treatments are cognitive stimulation therapy,

physical exercise and caregiver education and support. In a later stage respite care might be considered to
reduce the burden on the caregiver. Examples are care in day-care centres and special holidays for people with
dementia that are organised in some European countries (Belgium, France, Netherlands, Switzerland and UK).
Finally residential care and hospice/end of life care can be a good solution. Despite the negative image of care
in care homes and contrary to popular belief, we found in the World Alzheimer Report 2013 that the quality of
life in a care home is comparable with the quality of life of people with dementia at home. Living at home as
long as possible is a good aim, but there may come a point in time where someone with dementia needs such
a high level of care that admission to a nursing home is a better solution for both the person and caregiver and
one should not feel ashamed of that. Good quality care is possible and we should be proud of organising that
in each of our countries for our elderly!
A next issue is how we are going to finance long-term care for elderly. In the 1950s this was easy to answer.
According to data from the United Nations we had in 1950 all over the world 12 people in the working age
of 16-64 years old, against 1 over 65. The working class could easily pay some tax or public insurance to
finance a pension system as well as some basic long-term care. It was the time when in many high income
countries some sort of welfare state was introduced. But according to the same UN data, by the year 2050 we
will only have 4 people working against 1 over the age 65 and in some countries it might be even 1 against 2.
A system where two people in the working age have to pay social security for one elderly is not sustainable. This
demographic shift urges us to rethink these arrangements and discuss how we can find ways in our countries
how we can move into a system where people save for their own future as well and costs are divided over all
generations in a fair way.
So is it time to put dementia on the public health agenda? Yes it is and it looks there is no time to lose. More
and more countries have made a good start by creating a national dementia strategy or Alzheimer Plan. The
World Health Organization launched a report, Dementia: A Public Health Priority, in 2012 and Director-General
Dr. Margaret Chan said in the foreword: The overwhelming number of people whose lives are altered by
dementia, combined with the staggering economic burden on families and nations, makes dementia a public
health priority.I can only agree!

It gives me great pleasure to welcome participants to this years Conference of the Romanian Alzheimer Society.
Under the motto Dementia in Romania, the Conference will address the importance of dementia as a public
health priority.
I wish to congratulate my Romanian colleagues on this forward looking theme which is fully aligned with
Alzheimer Europes campaign to make dementia a European priority. We are encouraged that more and more
European countries have started with the development and implementation of national dementia strategies and
we hope that this trend will be continued in Romania as well. The importance of dementia has recently been
recognised at a G8 Summit convened in London by the UK Prime Minister and the call for greater European and
International collaboration in this field has been welcomed by Alzheimer Europe and all our national member
organisations.
I hope that this Conference will be another opportunity to exchange examples of best practices in the field of
research, care and political advocacy.

Clinical Diagnose and Neuropsychological Investigations from the DSM V Perspective


The Vth edition of DSM brings major changes both in neurodegenerative disorders terminology and significant
conceptual modifications by approaching as a continuum the neurocognitive decline from normal functioning
to extreme psychopathological intensity, that of dementia.
By classifying the neurocognitive disorders into major and minor disorders, the mild cognitive impairment
becomes a diagnostical target by determining the clinician to pay attention to the discrete symptoms, which
do not modify in an obvious way the functionings level and the individuals behaviour and that have most
often been ignored so far. The health professional is guided to lead a complex, clinical investigation through a
neuropsychological exploration of great subtlety.
Defining the six cognitive domains with the detailed description of the clinical features as well as the ways of
examination, alongside the biomarkers investigation prescription, are the key elements for establishing the
etiological aspects of the neurocognitive disorders and represent the big changes proposed by DSM V.

XXIst Century - One Hundred Years of Solitude


The paper starts from the epidemic of insomnia in the famous novel One Hundred Years of Solitude by
Gabriel Garcia Marquez.
The author of the paper makes an analogy with the contemporary world and especially with the situation in
Romania, where the demographic disaster caused by the aging population will create insurmountable problems.
These problems will be increasingly acute as there is no chance that the huge costs involved by the disease
could be sustained any how.
Statistics data show that the Romanian Health System cannot afford not even the hundredth part of the medical
costs in the United States for a patient with Alzheimer Dementia.
The only chance would be an inventive and miraculous cure to extinguish the epidemic in the shortest time,
as it happens in Marquezs novel.
Also, an increasing level of education could be an antidote to benefit from the numerous potential candidates
at the disease.
An excerpt from Marquezs farewell letter, he himself touched by this terrible disease, ends this essay paper:
I would teach the old men that death comes not with old age but with forgetting.

Why have we failed to find a treatment for Alzheimers disease (AD)


The notion that the progressive deterioration in cognition and function can be attributed to an identifiable
protein deposition - amyloid - is scientifically, psychologically and socially attractive. Furthermore, the fact that
a specific genetic mutation is associated with amyloid deposition has provided a molecular rational to the link
between AD and amyloid. Moreover the fact that the amyloid deposits could be visualized in vivo and measured
in CSF has constituted additional impetus for this hypothesis. Together this led to several mega-trials intended
to reduce the amyloid load.
However, in the march from amyloid deposits to anti-amyloid drugs, a number of findings, inconsistent with an
amyloid hypothesis, have been ignored. Amyloid deposits are present in non-demented individuals, demented
individuals might not have heavy amyloid loads, the correlation between the amyloid load and severity of
dementia is weak and the progression of dementia is not parallel by increased amyloid deposits. The genetic
mutation(s) is present only in a very small minority of early onset AD patients, but not in the overwhelming
number of demented patients. Furthermore, brain atrophy and synaptic lose is the hallmark of AD and not the
amyloid plaque. Infarct, Lew bodies, and other histological lesions are as frequent in demented individuals and
amyloid plaques.
Indeed, over the last 2 years long-term trials with drugs which were shown to reduce the amyloid load have not
benefited dementia, a fact interpreted by some that the trials should have been conducted in at risk individuals
and rather than in individuals already ill.
An alternative explanation would suggest that the treatment of AD and dementia should be approach from
several angles the same way as ASCVD or other age dependent and metabolic disease are approached.
Some of these approaches will be discussed.

Helga Rohra was born and studied in Cluj, Romania. In 1972 she emigrated
to Munich, Germany where she graduated at the International Language and
Translator Institute.
She specialized in the medical field.
She was diagnosed with dementia several years ago and still is very actively
involved in national and international activities.
She is present in TV shows and is a highly appreciated speaker at conferences.
Her book published in 2011, Stepping out of the shadow, became a bestseller.
In her new function in the Board of AE (Alzheimer Europe) and as First Chair of the EWGPD (European Working
Group of People with Dementia), in her speech she will refer to the challenges facing pwd (people with dementia).
She is the Chair of the first European Working Group of people with dementia, as well as a member in the
Board of Alzheimer Europe.
Mrs. Rohra is an author and a strong campaigner all over Europe for the rights of people with dementia.

Assoc. Prof. Cristina Tiu has graduated the University of Medicine and Pharmacy
Carol Davila, Bucharest, Romania in 1987. She have passed the board exam in
1993; I became senior neurologist in 1997, senior lecturer in 2000 and associated
professor in 2006. This could be the summary of 27 years of activity, of which
24 spent in the Department of Neurology of the University Hospital Bucharest.
Beyond the figures, a lot of work, enthusiasm, disappointments, perseverance,
satisfactions, determination and above all the attachment to a fascinating specialty.
Neurology occupies a central position, having connections with all the other
medical specialties and offers the opportunity to study different fields of pathology
of the peripheral or central nervous system. The field of cerebrovascular disorders has especially interested
Assoc. Prof. Cristina Tiu, so she added also neurosonology to clinical and teaching activities, a domain in which
she organize yearly a teaching course.
In 2012 Assoc. Prof. Cristina Tiu became European Master of Sciences in Stroke Medicine. She currently
coordinates the Stroke Unit from The University Hospital Bucharest.

Complexity of Neurono-Glio-Capillary Injuries in Alzheimer Disease


Introduction: Alzheimer disease (AD) is the most common form of dementia, a progressive degenerative,
incurable and terminal encephalopathy, and therefore an essential and topical health objective. Complex
researches of AD causes, mechanisms, lesions and effects can elucidate many medical aspects and find
efficient solutions in prophylaxis and treatment.
Materials and Methods: After death, human brains of AD patients (both genders, 62-93 years) were examined
by macro- and micromorphological methods, especially by cytochemistry, light, fluorescence and electron
microscopy.
Results and Discussions: Cortical and hippocampal atrophy, ventricular dilatation and decline of brain
weight-volume are the outcome of neurons number decrease. Selective loss of cholinergic cortical nerve cells
is the effect of neuron destruction in brainstem. Both reductions determine important decreases of dendrites,
axons and synapses. These quantitative neuronal losses coexist with suffering neurons having characteristic
damages: neurofibrillary tangles, granulo-vacuolar degenerations, Lewy and Hirano bodies. Glia cells are
strong connected with neuroinflammatory component of neurodegeneration. Firstly, astroglia atrophy induces
disruptions in synaptic connectivity, disbalances in neurotransmitter homeostasis, neuronal death through
increased exotoxicity, and secondarily, astrocytes became activated initiate neurodegeneration by inflammation.
A semblable mechanism is present in microglia behaviour. Chronic oxidative stress starts neuroinflammation
and activates microglia which release pro-inflammatory cytokines, initiators of neurodegeneration. Moreover,
-amyloid damages oligodendrocytes and thus induces substantial white matter lesions. Also, neuropil shows
characteristic alterations: amyloid plaques and neuropil threads, while capillary vessels present vascular
amyloidosis. All these neuropathological changes coexist with large lipopigment neurono-glio-capillary
accumulations.
Conclusion: Our studies prefigure new pathological connections and efficient therapeutic directions.

Dr. Roxana Crare is a medically qualified associate professor in cerebrovascular


ageing in the University of Southampton.
Having graduated in general medicine in 1994 in Bucharest, Roxana completed
her PhD in experimental neuropathology in 2006, in the University of Southampton,
UK.
Her main international recognition has come from the research she leads,
demonstrating the unique pathways by which fluid and soluble amyloid are
eliminated from the brain along intramural perivascular spaces, within the walls of cerebral capillaries and
arteries.
Dr. Roxana Crare leads the undergraduate Masters in Medical Sciences Programme in Southampton. Proud
of her Romanian heritage, she holds the role of Honorary Consul of Romania.
Dr. Roxana Crare is Trustee of Age UK Southampton, Patron of Libra Foundation, Diversity Champion and
Editor for Fluids and Barriers in the CNS and Frontiers in Ageing Neuroscience. She sits on the International

Failure of Perivascular Drainage of Amyloid-Beta and the Pathogenesis of Alzheimers Disease


Solutes, including A, are eliminated from the brain, with interstitial fluid, along the basement membranes in
the walls of capillaries and arteries, towards the surface of the brain. Cerebral amyloid angiopathy (CAA) is a
reflection of the failure of perivascular elimination of A from the elderly brain and in Alzheimers disease (AD).
Our lab uses a range of approaches - from observations in human brains, to experimental models in mice and
in vitro methods like atomic force microscopy and testing the perivascular system with a range of in-house
synthesized nanoparticles. Ageing, ApoE 4, hypertension and hypercholesterolaemia result in morphological
and biochemical changes in cerebral arteries and, as the major risk factors for AD, contribute to the failure of
perivascular elimination of A from the brain.
These observations suggest that management of the metabolic spectrum of diseases, combined with the
development of therapeutic strategies to facilitate perivascular elimination of A in ageing individuals and those
with the ApoE 4 allele may result in a reduction in amyloid load in the brain and reduce the incidence of AD.

Brain Aging and Neuroplasticity


Cognition decline associated with brain aging is a very complex and heterogeneous process possibly open
to modifications. Large variety of molecular, cellular and functional processes are the background of brain
aging, making extremely difficult to establish a borderline between normal and pathological age related brain
changing. Alteration in synaptic connectivity seems to be an important driver of cognitive impairment.
Neuroplasticity is the term used to describe the brains ability to change existing structure, function and
connections, in response to environmental stimulus, such as learning or experiencing something new or to an
injury. This reorganization can be described at many levels ranging from the cellular to functional or structural
connectivity to behavior, and these varied levels can presumably be mapped onto each other.
Neuroplasticity means also the brains ability to restructure itself after training or practice. Also, neuroplasticity
mediates the transition from early learning to automatic responding.
This presentation will give a brief overview of the main mechanisms supporting the neuroplasticity in brain
aging.

Ethics of Elderly People Care and Treatment - Legal and Forensic


The population of Romania is going through a remarkable aging process which, in 2010, lead to the existence
of approximately 3.150.000 people over 65 (double than in 1970) and 630.000 people that can be included in
the category of very old people (over 80) that means three times their number in 1970.
It is obvious that the old peoples physical health increased, simultaneously with that of the other categories of
age, therefore increasing both longevity and the life expectancy. Though it must be mentioned the undeniably
fact that the physical health is not always associated with mental health, but, on the contrary, a longer life
is supposed to mean (at least theoretically) a high number of destroyed neurons, variable element from one
person to another.
The increase of life expectancy of the population imposes to study aging not like an arithmetic relations, in
which years summed up.

The presentation reveals the complex aging process, with profound implications,that begin with the legislation
and ethics and are extended to economy, medicine psychology. Therefore, the research is multidisciplinary
due to the tremendous problems that occur (physiological, pathological, intellectual, of adjustment) in the dual
relation between age and society. It is also to be mentioned the fact that, at older ages, the feeling of exclusion
deepens, especially when it is correlated with an economical situation, mostly precarious of the elderly.
The paper also approaches the juridical aspect of the elderly, especially of the disabled ones that is included
in the Penal Law as well as in Civil Law. In the Penal Law, a question is raised regarding the mental capacity of
the person, presumed with mental disorders that, that committed acts of criminality. The Civil Law establishes
the psychic capacity for the disposition documents (wills, sales and purchases documents etc.).
Finally, from the point of view of the protection of the disabled old person, two approaches for treatment result:
autonomy or paternity.

Non-Specific Functional Decline in Older People


Functional decline, defined as a decrement in physical and/or cognitive functioning that leads to reduced
ability to perform the activities of daily living (ADLs) that are necessary to live independently is a common
outcome especially for frail older people and often in hospital, regardless of whether or not any adverse events
or geriatric syndromes occur during their stay in hospital.
Functional decline leads to occurrence of functional impairment that interferes with daily activities and the latter
is necessary for the diagnosis of dementia. Functional impairment is a result of several deficits affecting both
cognitive and physical functions.

Memory impairment causes inability to remember appointments and prevents the individual from participating
in social activities or games, for example, bridge. Speech impairment interferes with social contact and may
result in an inability to understand spoken or written language. Apraxia leads to inability to use tools and to
continue engagement in previous hobbies. Spatial disorientation leads to inability to take independent walks.
Executive dysfunction leads to deficits in problem solving and judgment and prevents the individual from
planning and executing an activity.
Functional impairment may cause three secondary consequences of dementia: dependence in ADLs, inability to
initiate meaningful activities and anxiety if a person with dementia recognizes his/her limitations or if a caregiver
has unrealistic expectations about the abilities of the care recipient. These secondary consequences may
cause several peripheral symptoms: rejection of care, agitation, repetitive vocalization, apathy and insomnia.
Early recognition of frail older people at risk for functional decline is paramount.

Current Imaging Possibility of Alzheimers Disease Assessment


Alzheimers disease (AD) is a specific neurodegenerative disease and is the most common cause of dementia
in old people. Based on clinical evaluations, 13% of persons over 65 years and 45% of those over 85 have AD.
Aim of this paper is to present value of imaging techniques currently available, their priorities in relation to the
ability to early identify feature of the pathological substrate of Alzheimers disease, before mental function
deterioration.
We will present both structural imaging techniques which provide morphological information, secondary brain
damage in this type of disease - computer tomography (CT) and magnetic resonance imaging (MRI), and
functional imaging techniques more recent available - single-photon emission computed tomography (SPECT)
and positron emission tomography (PET), it will illustrate the normal and pathological aspects of each imaging
technique used, CT, MRI, SPECT, and PET / CT.

Over the past 18 years, Annette Dumas has been involved in European policy
work.
Currently, she is acting as EU Public Affairs for Alzheimer Europe, the umbrella
organisation of Alzheimer associations in Europe, having previously worked for a
European patient NGO (European Patients Forum) and one of the worlds leading
pharmaceutical companies (Merck Sharp & Dohme, Inc.).
Annette supports Alzheimer Europe in making dementia a European public health
priority.
She is responsible for the organisations strategy on European health, social and research policies, legislation
and measures that affect people with dementia and their carers.
She facilitated the setting up of the European Alzheimers Alliance in the European Parliament.
Annettes area of expertise are European public health, NGO and patient advocacy, disease awareness
campaigns and inclusion of specific diseases in the EU public health legislation.
Annette has participated in European conferences and roundtables, either as a speaker or a member of the
organising committee.
She has also published in professional and European publications.

Making Dementia a National Priority. The European Added Value


Since 2006, Alzheimer Europe and its members have relentlessly campaigned to make dementia a European
priority.
Alzheimer Europes major policy campaign embodied in the Paris Declaration of 2006 called upon the European
and national policy makers to recognise Alzheimers disease as a major public health challenge and develop
European, international and national action programmes. Since then, a series of promising developments have
gradually taken place across Europe and dementia is now on the agenda of the European Commission and
some EU Member States.
The exchange of best practices, cooperation in research and some EU instruments have further facilitated the
awareness raising about the challenges of dementia and the possibility to find innovative solutions.

care in the United Kingdom.
Dr. Liana Rada Borza successfully graduated an Intercalated Bachelor of Sciences in Neuroscience with Basic
Medical Sciences at the Royal Free and University College London Medical School, being awarded the Health
Foundation Intercalated BSc Award for completing a research project regarding mechanisms of reactive oxygen
species regulation in young and old rat nucleus basalis of Meynert neurons.
Furthermore, she is one of the authors of the Guide for Developing Community-based Mental Health Services in
Romania that was created in collaboration with the National Centre for Mental Health and Fight against Drugs
Bucharest and the World Health Organization Collaborating Center for Research and Training in Mental Health
Trieste, Italy.

The Concept of Retrogenesis: New Ways of Understanding Alzheimers Disease


Retrogenesis has been defined as the process by which the degenerative mechanisms from Alzheimers
disease (AD) inversely recapitulate the processes of the normal neurodevelopment.
This presentation gives an overview of the functional, cognitive, neurologic, neuropathologic and neurometabolic
evidence that support the concept of retrogenesis. It also emphasizes my main findings with respect to
retrogenesis, thus describing new retrogenic models, as follows: an electroencephalographic model, a brain
structural/gray matter model and a retrogenic model for detecting abuse of institutionalized people with
AD. Moreover, the presentation provides expert opinion on the possible ethical and forensic implications of
retrogenesis.

She is author / co-author of over 127 scientific papers published at home and
abroad (books, book chapters monographs, courses for students) and also project

Prof. Delia Marina Podea is member of Romanian Association of Psychiatry and Psychotherapy, of Institute for
Psychotherapy and Hypnosis Hanau and of European Association of Psychiatry.
Prof. Delia Marina Podeas interests and research are in the field of geronto-psychiatry schizophrenia (atypical
antypsychotics and quality of life), bipolar disorder (phisio-pathology and therapy) and anxiety disorder.

Positive Effects of Alternative Therapies on Cognition and Immunosenescence Process


Introduction:The actual literature have suggested that mild cognitive impairment (MCI) associated with old
age can be improved by anti-aging products like: omega-3 acids, herbal natural products (vinpocetinum,
Rhodiola-rosea, gingko-biloba), nootropics (Piracetam), antioxidants (Vitamin E, Vitamin C, Vitamin A,
alpha-lipoic acid, Co-enzyme Q10).
However, the effectiveness and safety of this treatment has not been accepted by the national and international
agencies (ex. EMEA, FDA), nor has it been demonstrated by randomized controlled trials. Therefore the
acceptance of alternative therapies in MCI is still low.
Our research is based on the study of 230 patients (over 65 years), diagnosed with MCI. The mean age was
74.98 + 8.2 years (range 6589 years). Patients were assigned to four groups treated with nootropics, herbal
natural products, antioxidants and combinations. The groups were evaluated at screening, after 6 months and
after one year of treatment using MMSE scale, Clock test, Raven test and flowcytometric immunophenotyping.
The main groups, MMSE score at the initial screening was 24.656 + 2.30 points. After 6 months and after
one year, the patients treated with combinations of herbal natural products and antioxidants had a higher
improvement compared to patients with monotherapy.
Besides the primary cognitive effects, the alternative therapy has positive effects on immunosenescence
process, significantly increasing the CD8+ nave T cell sub-population. This study proves the necessity of early
treatment for MCI to delay the outcome of dementia, so the alternative approach has proven accurate and
promising for the future.

with Dementia Specialists for Handling DE/08/LLP-LdV/TOI/147076, and DUKOM,
Improvement of vertical and horizontal permeability of competencies in the field
of geriatric care DE/10/LLP-LdV/TOI/147359 .
She has published articles and books in the fields of social gerontology and social work for the elderly: Human
developnent and aging (2001); Social assistance for the elderly. Methodological issues (2003); Assisting
elderly, care and counseling. European training models (2012); Elder Care. Studies and models of good
practice (2012 ).
As editor, she cared for publishing in Romanian language two volumes: Jennie Powell Care to communicate.
Assisting the elderly with dementia (2010); Klaus Mathes, Elder Care. Model Training (2010).
In the Society of Psychodrama J. L. Moreno, Romania, she works as a psychotherapist and trainer of classical
psychodrama.

Qualification of Multipliers for the Education of Specialists for Handling Persons with Dementia
This workshop represents a synthesis of the results of Leonardo da Vinci pilot project, ADEM / 147176/2008
- Community Action Program on vocational training: Qualification of Multipliers for the Education of Specialists
for Handling Persons with Dementia.
The projects goal was to meet the need for training in an interdisciplinary field, that of assisting and caring for
elderly with dementia.
The main objective of the project was to develop a standard for European qualification for work of skilled
personnel for patients with dementia. Expertise of professionals in the five countries participating in the
project (Germany, Bulgaria, Poland, Romania, Spain) was seized to develop a matrix model of the training,
skills-oriented.
The standard model can be developed and applied at European, national peculiarities with respect.
For illustration, some results are presented (Img.1, 2).

Adrian Stnescu is President of the Romanian Gerontologica Association a


support organization for geriatric population and for people suffering of dementia.
He had a strong contribution to the planning and organizing of the National
Educational Programme Nu uita! in the dementia field in 2010-2011.
In 2008 he founded the first private Romanian Hospital for Alzheimer patients, in
Northen Bucharest.

Patterns, Design and Management of Institutional Care for the Patient with Dementia
Alzheimers dementia and all forms of dementia represent one of the major causes of dependency of the elderly
persons and require specific approach of short (STC) and long-term care (LTC) services. For most people with
dementia, specific services begin and for many they end in the community.
Institutional care is an important part of the total package of dementia care. By supporting carers and providing
temporary respite and high quality dementia care, day hospital and also LTC services can improve the quality
of life and prolong the period spent in the community.
The provision of, and reimbursement for, long-term care differs across countries. The decision to place a
relative in a long-stay facility is a difficult one and brings with it feelings of guilt and grief, as well as more
positive aspects such as relief from sleeplessness and respite from behavioral disturbance. Patient features
that predict entry to long term care include severity of functional impairment and behavioral disturbance.
Any design of a long term care facility is a compromise between normal living arrangements and the arrangements
necessary to provide care, between the desirable and the affordable. On the other hand, a well-functioning
long-stay facility include regular medical review, individually tailored activities and appropriate management of
behavioral disturbance.
The European Union Charter explicitly recognises the rights of those commonly placed in institutional care - the
rights of elderly to live a life of dignity and independence (article 25) and the rights of persons with disabilities
to participate in the life of the community. However, since 2014 the Structural Funds can not be used to create
or renovate the long-stay facilities. As a result, a coherent program of reabilitation of these facilities in Romania
to reach European standards will become more difficult to put into practice.

Senior Consultant Psychiatrist in Oradea, Romania

Nicoleta Ttaru worked for more than 35 years at the Neurology and Psychiatry
Hospital in Oradea and for 5 years at the Forensic Psychiatry Hospital tei, Bihor.
She was also lecturer at postgraduate courses for young doctors, psychiatrists
and geriatricians on geriatric psychiatry in Oradea, Bucharest, Lisbon, Antalya and
Wroclaw.
She was the first winner of the EAG Research Award for Eastern European
members in 1995. She is the founder and president of the Romanian Geriatric
Psychiatry Association.
From 2003, she is member in the Honorary Board of Romanian Association of Psychiatry. From 2002, she is also
member in EC of Old Age Psychiatry Section of World Psychiatry Association, and between 2005 - 2011, she
was vice-president of Old Age Psychiatry Section of WPA. From 2004, Dr. Ttaru was elected in the Scientific
Panel of experts of AD Europe. In 2005, she was elected member in EC of Forensic Psychiatry Section of WPA,
and she is secretary of Forensic Psychiatry Section of WPA from 2008, till now, and from 2011 till now she
was elected officer in the Board of Psychiatry in Developing Countries Section of WPA. Between 2001-2009,
Dr. Ttaru was member in the Board of Directors of International Psychogeriatric Association.
She participated as lecturer and as organizer of symposia and workshops at more than 141 international
and national psychiatry congresses, with more than 176 presentations. Dr. Ttaru published 22 articles and
chapters in international books and journals.

BPSD - Dilemmas and Challenges for the Primary Care


Patients with different types of dementia often develop behavioural and psychological symptoms. Such
symptoms (which may include agitation, aggression, anxiety, irritability, depression and restlessness) are
distressing to caregivers and may trigger breakdown of care in both community and institutional settings.
After a short revue of the concept of BPSD and the contemporary literature on phenomenology, etiology,
epidemiology and treatment, we discuss the dilemmas and challenges for the primary care of demented people
with behavioral disturbances.
BPSD are common in outpatients and more in the nursing homes and psychogeriatric departments. Any BPSD
can occur in any dementia and in any stage of illness. These symptoms are an intrinsic component of dementia
and result in suffering, excess disability, financial loss, and a decrement in the quality of life for the patient and
the caregiver.
We compared the ICD 10, DSM IV and DSM V criteria for different types of dementia with different types of
behavioral disturbances.
Patients with dementia who have behavioural disturbances, psychotic symptoms or agitation require
admission to a psychiatric hospital for acute or chronic illness, both they who live in the nursing homes and
the ones who live in their own homes. The greatest difficulty in dealing with BPSD is in providing optimal
management of these patients. Effective treatments for BPSD improve symptoms and delay institutionalization.
Psychopharmacological treatment is accompanied with behavioral therapy and community care.
The improvement of the mood and behavior in elderly patients with the newer antidepressants and the newer
antipsychotics allow their care in the community health or social care services.
We could improve the quality of life of all frail and mentally ill elderly living in the community, including those
with dementia.

The Complex Care of the Elderly Patient with Alzheimers Disease and Comorbidities
The number of people over 65 years that will suffer from Alzheimers disease will double by 2030 but early
diagnosis and initiation of treatment ensures the decreasing of the diseases progression and maintaining the
patients and his family quality of life. Increasing number of people suffering from memory disorders represents
a medical, ethical, socio-economic problem in the healthcare system.
In Romania, complex geriatric care addresses the frequently hospitalized patients for acute exacerbations of
chronic progressive diseases such as heart failure, chronic obstructive pulmonary disease, chronic kidney
disease, diabetes mellitus and its complications and also for new acute events such as infectious disease,
cardiac disease. The purpose of geriatric management team becomes optimizing the physical, mental, spiritual
and social integration there of the patient.
Elderly patients with Alzheimers disease and multiple comorbidities require a complex care especially when
it comes to cardiac decompensation, infection with multiple causes, malnutrition, prolonged bed rest and
deterioration of the cognitive function.
Often, the patient cannot participate in choosing the therapy due to cognitive disorders and impaired decisionmaking capacity. This demonstrates the necessity of a multidisciplinary team for treating the geriatric patient,
the necessity of specialized care units for chronic patients with dementia. It also demonstrates the necessity
of a continuous update of the ethical care and protection principles when it comes to the geriatric patient with
dementia.

sesiunii. Un tehnician va fi la dispoziia Dumneavoastr pentru a v acorda asisten n caz de necesitate.

Chiar dac mai sunt participani care intr n sal, v rugm s ncepei sesiunea conform programului stabilit.
Este necesar ca autorii prezentatori s i susin lucrrile n ordinea indicat n programul tiinific. n cazul n
care exist prezentatori abseni, v rugm s ajustai ordinea lucrrilor n consecin.
V rugm s informai autorii prezentatori asupra duratei sesiunii i a celor 25 minute alocate fiecrei conferine.
Este recomandat s urmrii cronometrul setat pe ecran i s amintii prezentatorilor c trebuie s respecte
timpul alocat fiecrei prezentri. V rugm s atenionai vorbitorii cnd mai au doar 2 - 3 minute pentru
prezentare i cnd trebuie s ncheie discursul.
n cazul n care un participant depete intervalul alocat, este recomandat s ntrerupei prezentarea i s

Introduction
Please make sure that you are present in the designated session room 15 minutes prior to the start of the
session.
All presentations will be loaded by the Conference staff before the session. A technician will be available to
assist with any audio-visual needs.
Ensure all speakers are physically present at the session. Please note any no-show authors and inform the
Conference Hall Responsible.
Please be so kind to introduce the Speakers and keep the introduction brief, no longer than 1 minute.
Time Monitoring
Please make sure you start the session on time, as any transgression of the time limits could affect the scientific
program.
Even if the audience is still arriving, please start the session at the designated time.
Speakers should present in the order listed in the programme. If a presenter is a no-show, please readjust the
order accordingly.
Please inform the speakers about the aggregate time of the session and of the 25 minutes alloted to each
conference.
Follow the timer set on the screen and remind the speakers that they must confine their presentation to the

allotted time. Monitor the timing of each speaker closely, remind them they have only 2 - 3 minutes left for their

Each presentation has a 10 minutes timeslot allotted for discussion and questions.
Invite the audience to ask questions. Promote a fair and equal exchange of ideas.
If a paper draws no questions from the floor, you are encouraged to ask questions in an attempt to stimulate

Closing of the Session
When closing the session and before giving a final thanks to the presenters collectively, please remind the
audience when the next session commences.

V rugm s v nregistrai prezentrile, inclusiv cele video (n format: Avi, MPEG-1, MPEG-2, MPEG-4, WMV,
sau orice alt format video DVD) pe calculatoarele din Slile de Conferine, n pauza premergtoare sesiunii i la
spaiul special amenajat de Organizatori.
Toate materialele aferente prezentrilor trebuie s fie salvate ntr-un singur folder.
Mijloace de stocare acceptate: CD, DVD, USB stick i Hardware extern numai cu interfaa USB 2.0.
Dac avei ncorporate filme n prezentare, v rugm s verificai dac acestea ruleaz cu Windows Media
Player.
Prezentrile cu animaii i videoclipuri ncorporate pot cauza reale probleme n cazul n care nu au fost verificate
cu atenie naintea nceperii sesiunii.
Dac folosii propriul calculator (laptop), asigurai-v c aducei propriul cablu de alimentare i mai ales cablul
adaptor de conectare la videoproiector al sistemului Dumneavoastr.
Pentru o proiecie corespunztoare a prezentrilor, v rugm s v ajustai calculatorul astfel: rezoluia ecranului
1024x768, fr screensaver i opiunea de alimentare always on (mereu deschis).

n timpul prezentrii:

Before the presentation:
Please register the presentations, including video presentations (in Avi, MPEG-1, MPEG-2,MPEG-4, WMV, or
any other DVD video format) on the computers inside the Conference Halls and in the specially designed area,
in the break prior to the session.
All the information needed for the presentations must be saved in a single folder.
Accepted portable devices: CD, DVD, stick USB and external Hardware only with 2.0 USB interface.
If your presentation contains movies, please check their compatibility with Windows MediaPlayer.
Presentations containing animations and videos could cause real problems if they are not properly checked
before the session.
If you are using your own computer (laptop), make sure to bring your own power cord and especially the the
adapter cable connecting the system to the video projector. Please also bring any type of connectors you might
need at some particular types of laptops.
For a proper projection of the information, please adjust your computer as follows: screen resolution 1024x768,
no screen-saver and power option always on.
ALL PRESENTATIONS WILL BE DISPLAYED AT A 1024X768 RESOLUTION.
The Session Rooms technicians will be available in order to offer you assistance regarding your presentation
and its preparation. The information for your presentation copied on the available laptop in the Session Room
will be deleted by the Organizers at the end of the event.
During the presentation:
All the presentations transferred at the technical desk in the breaks between sessions will be saved on the
computer shall be processed by the Session Rooms technicians according to the instructions received from
the sessions chairpersons that will announce the conferences.
The change of slides during the presentation should be done by each speaker, either by using a portable device
for PowerPoint presentations, provided by the Organizers (you will be guided in order to easily use it), either by
using the mouse and keyboard.
If you want one of the Conference Halls technicians to operate the page change, you should let him know about
your decision as well as about the right moment to change the slide, during the presentation.
Full cooperation of the presenting authors regarding the evolution of the sessions will increase the quality of the
Conference and shall be appreciated accordingly.
In order to avoid the infecting of the computers with viruses please scan all the presentations and related
materials prior to the Conference.
We agree and respect the personal aspect of the information contained by the presentations and we shall treat
it as such. Please do not hesitate to ask the Organizers any question whatsoever.
II. Sesiune Postere / Posters Session
Formatul de prezentare poster trebuie s se ncadreze n maxim 90x120cm.
Organizatorii se vor asigura c autorii posterelor vor avea tot timpul la dispoziie materialele necesare afirii
acestora (foarfece, band dublu-adeziv).
Posterele trebuie afiate n data de 27 februarie i demontate de ctre autorii lor pn la finalul Conferinei.
Posterele care nu au fost scoase la sfritul Conferinei vor fi ridicate de ctre Organizatori.
Atenie - posterele ridicate nu sunt pstrate!
