Abstract:
Endoscoic sinus sur!ery "#ic# is t#e commonly erformed nasal sur!ery #as its o"n roblems if
not erformed roerly$ %uccess of maxillay sinus antrostomy deends on includin! t#e natural
ostium to t#e antrostomy$ &or t#is to #aen t#e natural ostium s#ould be identified durin! t#e
sur!ical rocedure$ 'ommon cause of failure in endoscoic sinus sur!ery is t#e failure to address
t#e uncinate rocess$ (n all cases it s#ould be remo)ed comletely before roceedin! furt#er$

Anatomically uncinate rocess is attac#ed to t#e bony co)erin! of nasolacrimal duct$ Bony
co)erin! o)er nasolacrimal duct is )ery dense$ T#is c#an!e in t#e bony t#ic*ness bet"een t#e
uncinate rocess and t#e nasolacrimal duct ensures t#at a comlete uncinectomy can be erformed
"it# minimal trauma to naso lacrimal duct if t#is anatomical fact is *et in )ie"$ Accordin! to
Bol!er a certain amount of minimal trauma occurs commonly durin! uncinectomy$

T#e maxillary line is considered to be an ideal sur!ical landmar* if sic*le *nife is used to excise t#e
uncinate rocess from its anterior attac#ment$ Maxillary line is a mucosal rominence arisin! from
t#e anterior attac#ment of middle turbinate alon! t#e lateral nasal "all )ertically u to t#e uer
border of inferior turbinate

robe t#e osterior fontanelle area$ T#is "ould cause air bubble to arise from t#e natural ostium
