Application for Passports Requirements

The following documents are to be submitted to the High Commission via a courier of the applicant choice,
along with a pre-paid completed courier airway bill.

Adult Application (renewal)

1.

Completed application form. This can be downloaded at www.pica.gov.jm

2.

Original Passport

Original Birth Certificate (with translation, if applicable)

4.

Original Citizenship/Naturalisation/Adoption Certificate (if applicable)

5.

Original Deed Poll (if applicable)

6.

Original Marriage Certificate (if applicable)

Original Divorce Certificate (if applicable)

8.

Two Passport -sized photographs, one of which should be certified

Fees
a.

US$80.00 bank draft in the name of Ministry of National Security, being cost of
document

b.

US$10.00 bank draft in the name of Jamaican High Commission, being Consular Fee.

Minor Application (renewal)

1.

Completed application form, which can be downloaded and printed at www.pica.gov.jm. Please
note that the consenting parent should fill out and sign at Section C and also at Section E,
even though it says signature of applicant. Note also that in Section G the Certifying officer
should sign knowing the consenting parent, not the minor. If the minor is 12 years old and
older, he/she should sign in the signature box.

US$50.00 bank draft in the name of Ministry of National Security, being cost of
document

b.

US$10.00 bank draft in the name of Jamaican High Commission, being Consular Fee.

A replacement passport application will require that an original Police Report be attached as well as
additional certified copy of an identification card. The fee for the Ministry of National Security is
US$160.00 for an adult and US$100.00 for a child.

Citizenship Applications
The Mission mostly handles applications for Citizenship by descent and by Marriage
o Application for Citizenship by Descent
(1) Completed Citizenship Application Form, which can be downloaded and printed
at www.pica.gov.jm.
(2) Applicants Original Birth Certificate (showing parents names)

o Application for Citizenship by Marriage

Visa Applications for nationals living outside of Trinidad)

The applicants should, in the first instance, scan and submit the documents by email. In the case of
referred visas, the application will be sent to the Visa Unit in Jamaica for processing. This can take up to 2
weeks.

One

(1) passport sized photograph which should be affixed to the upper right hand corner of the
application form.

The fee for all visas, independent of nationality or multiple/single entry requests, is US$20.00. It is to be
made in the form of a bank draft made payable to Jamaican High Commission and should be sent with all
the documents via courier.
Emergency Certificate (EC) Applications
The Counsellor, until amended, handles all EC Applications.
ECs are issued at a cost of US$70.00. The Bank Draft is made payable to: Jamaican High Commission.
