Tips for Avoiding Health Care Fraud or Health Insurance Fraud

Never sign blank insurance claim forms.


Never give blanket authorization to a medical provider to bill for services rendered.
Ask your medical providers what they will charge and what you will be expected to pay outof-pocket.
Carefully review your insurers explanation of the benefits statement. Call your insurer and
provider if you have questions.
Do not do business with door-to-door or telephone salespeople who tell you that services of
medical equipment are free.
Give your insurance/Medicare identification only to those who have provided you with
medical services.
Keep accurate records of all health care appointments.
Know if your physician ordered equipment for you.

Counterfeit Prescription Drugs


Tips for Avoiding Counterfeit Prescription Drugs:

Be mindful of appearance. Closely examine the packaging and lot numbers of prescription
drugs and be alert to any changes from one prescription to the next.
Consult your pharmacist or physician if your prescription drug looks suspicious.
Alert your pharmacist and physician immediately if your medication causes adverse side
effects or if your condition does not improve.
Use caution when purchasing drugs on the Internet. Do not purchase medications from
unlicensed online distributors or those who sell medications without a prescription. Reputable
online pharmacies will have a seal of approval called the Verified Internet Pharmacy Practice
Site (VIPPS), provided by the Association of Boards of Pharmacy in the United States.
Be aware that product promotions or cost reductions and other special deals may be
associated with counterfeit product promotion.

BREAKING AND ENTERING-WILLOW POND
6400 block of Willow Pond Drive, between November 19, 12:01AM, and November 20, 2014 11:42AM,
unknown persons entered the victims vacated residence due to a house fire. Suspects broke in
through the only working door by destroying the dead bolt lock. They stole five herringbone cut 24
karat necklaces, 34 grams of gold, and one ring with a red stone, valued at $4,000.00. No suspects.

GRAND LARCENY FROM A BUILDING
Spotsylvania Towne Centre, 3102 Plank Road, between November 21, 10:00PM and November 22,
2014, 10:15PM, two white females were seen taking merchandise from the Shirt Vendor stand at the
mall, after the mall was closed. Taken were six shirts and hooded sweat shirts, valued at $450.00.

GRAND LARCENY FROM AN AUTOMOBILE-OAK GROVE TERRACE
5500 block Carousel Street, between November 21, 5:00PM and November 22, 2014, 10:04AM,
Someone entered the unlocked vehicle and stole a Saxophone and the black carrying case. The case
has blue paint on one side and a Minion sticker and the saxophone and case are valued at $300.00.
No suspect.

PETIT LARCENY FROM AN AUTOMOBILE-BRITTANY COMMONS
10000 block of Moorgate Avenue, on November 23, 2014, between 11:00AM and 7:00PM, someone
entered the residence and stole $100.00 from a purse on the kitchen counter, and $50.00 from
underneath a nightstand display in another room. Nothing else was noticed as missing. The victims
accidently left the doors unlocked. They do not have a suspect.

PETIT LARCENY SHOPLIFT
Rite Aid Pharmacy, 5115 Plank Road, on November 19, 2014 between 2:40PM and 2:45PM, a female
took a few boxes of hygiene pads, hosiery, and a bottle of Justin Bieber scent perfume, put them
inside of a large purse then went into the bathroom. The female exited the store and was last seen
walking on Five Mile Fork Road. She is white, wearing a poufy black coat, jeans, and carrying a brown
UGG purse. Value of the stolen property is $98.48.

FRAUD-WEAVERLY VILLAGE
800 block of Churchill Drive, on November 21, 2014, between 4:43PM and 6:00PM, the victim received
a statement from PUBLISHERS BILLING EXCHANGE, P.O. BOX 2489, WHITE CITY, OREGON, 97503,
wanting payment, claiming that the newspaper is due for renewal. The victim wrote a check for
$399.95, as indicated on the statement. When the victim noted that it was a scam, a stop payment
was made.

FRAUD CREDIT CARD-FOX POINT
(274637)

6100 block of West Melody Court, on November 19, 2014, between 12:01AM and 6:53PM, someone
used the victims Geico Credit Union card and charged $224.14 to the account in Virginia. The victim
has the card in possession. No suspect.

VANDALISM-OAK GROVE
5500 block of Tallow Street, between November 21, 7:00AM and November 22, 2014, 8:06AM,
someone entered the blue 2009 Subaru and rummaged through it. Nothing was noticed missing;
however a cell phone charger was damaged. It is valued at $10.00. No suspect.

COUNTERFEIT CURRENCY
Ace Hardware, 9795 Courthouse Road, on November 22, 2014 between 12:46PM and 1:56PM, a white
male paid for merchandise with a counterfeit $10.00 bill. The suspect mentioned that his vehicle was
broken down in the parking lot. The deputy was able to locate the suspect. The suspect stated he
received it when he sold scrap metal. The scrap metal was closed when the deputy arrived.
Investigation continues.

COUNTERFEIT CURRENCY
Michaels Stores, 3000 Plank Road, On November 22, 1014, between 5:00PM and 5:20PM, two middle
aged black females passed 31 counterfeit $50.00 bills when they purchased $1,500.00 in Visa gift
cards, and paid for other merchandise with another bogus $50.00 bill. A total of 22 bills had the same
serial number, and 9 others are the same numbers and a different serial than the other 22.

COUNTERFEIT CURRENCY
Walgreens Pharmacy, 5650 Plank Road, on November 22, 2014, between 3:50PM and 4:05PM, a male
was approached by three black females asking him to take the $1,200.00 of counterfeit currency and
to purchase $1,000.00 in gift cards, and other merchandise. The male complied and notified his
employer of what occurred. The pharmacy was able to cancel the gift cards. The three suspects were
in a black full size SUV, and a light blue smaller SUV, and appeared to be fairly new.

Spotsylvania County Crime Solvers

From January 2008 to present the Spotsylvania County Crime Solvers has paid
out $18,045 in CASH REWARDS for information through 456 calls to the Crime
Solvers telephone tip line, Crime Solvers Web Tip page, and the Text-A-Tip
phone number. Information obtained has resulted in 88 arrests involving
burglaries, drug distribution, arson, vandalism, breaking and entering and other
crimes against the citizens of Spotsylvania County. To date, there has been
$81,429 worth of stolen property, seized property, and assorted types of drugs
recovered. Since June 2010 to present, 20 Wanted Fugitives have been arrested
and are now facing their charges.
