Section 1.3: Prohibited Wastes
Employees will receive training upon initial assignment to the facility and annually thereafter on the procedures for
detecting and rejecting prohibited wastes and for handling any prohibited waste that was inadvertently accepted.
All waste loads will be inspected at the facility prior to acceptance by the facility. If any prohibited waste is
discovered during load inspection, the waste will be rejected, the customer will be directed to transport the waste
to the designated destination for the waste, and SNHD will be notified by the next working day via a rejected load
form in a format specified by SNHD. SNHD shall also be notified, in writing and in a format specified by SNHD,
whenever the owner or the operator anticipates rejecting a load of material from a specific person for any reason,
including non-payment of fair market-priced fees.
If it is discovered that any prohibited waste has been inadvertently accepted, the facility will use best management
practices for handling the waste and will arrange for the transportation of the waste by the designated transporter
for the waste to the designated destination for the waste within one week.
An accurate record of any rejection or inadvertent receipt of prohibited wastes will be maintained at the facility.

The facility will document the transport of materials to and from the facility using manifests and receipts for
materials delivered and removed from the site. The manifests and receipts will be kept for at least 3 years and
will include the following information: (a) the quantity in tons of materials transported to or from the facility and (b)
the origin or destination of the reinforcing steel. This information will be available for inspection by SNHD during
regular business hours and will be summarized in an annual report which must be submitted to SNHD by March 1
on a form provided by SNHD.

Section 2: Dust Control


The operator will ensure that paved access roads and outdoor waste storage areas will be maintained. A water
truck will be utilized to maintain adequate dust compliance on the haul roads and disturbed areas. The operator
will ensure that speed will be restricted to 5 miles per hour inside the facility.
The operator will incorporate and maintain an effective water spray system on the material processing equipment.
The material processing equipment will be operated in accordance with the Clark County Department of Air
Quality (DAQ) dust regulations.

Section 3: Emergency Control


Section 3.1: Fire Control
In the event of an incipient stage fire at the facility, employees will use a fire extinguisher or a hose from the water
truck to extinguish the fire. If a fire at the facility grows beyond incipient stage, 911 will be called for assistance.
The fire lanes around the perimeter of the outdoor waste pile will be kept clear of flammable or combustible
material or vegetation.
Section 3.2: Pollution Control
The processing of the concrete, cement, soil and aggregate materials will produce fugitive dust emissions. Those
emissions will be controlled by maintaining moisture in the minus materials as required by the Department of
Air Quality (DAQ) operating permit.

Section 3.3: Other Procedures
The Emergency Response Coordinator will be responsible for ensuring compliance by all employees with the
emergency response procedures, ensuring training of all employees in the facility's emergency response plan,
and recording of all information required in the event of an emergency. The Plant Manager will be designated the
Emergency Response Coordinator. If, for any reason, the Plant Manager is unable to perform the duties of
Emergency Response Coordinator, the Plant Operator shall be the secondary Emergency Response Coordinator.
Both the Plant Manager and the Plant Operator shall be required to carry cellular phones.
Employees will receive training upon initial assignment to the facility and annually thereafter on the procedures for
preventing and controlling emergencies such as fires, floods, and pollution.
If an emergency occurs:
1. The Emergency Response Coordinator shall instruct the Plant Operator to immediately shut down the
material processing equipment using the emergency shut-off switch located near the processing
equipment. In the event of an emergency that prevents the use of this switch, the location of the master
circuit breaker for the facility shall be known to the Emergency Response Coordinator so that power to
the facility may be cut.
2. All employees will evacuate the facility via the nearest available exit as per the Emergency Escape Plan.
They will re-group at the designated gathering area or at an alternative gathering area if the first one is
inaccessible. The Emergency Response Coordinator will affirm that everyone was safely evacuated.
3. The Emergency Response Coordinator will call 911.
4. All traffic in the parking lot and in any receiving or shipping area will be immediately directed to vacate the
property to ensure maximum space for emergency response vehicles.
5. The Emergency Response Coordinator shall contact Mr. Galen Stockton at 702-355-3811. Mr. Stockton
shall be responsible for providing required notification and reports to SNHD and other authorities as soon
as possible.
6. The Emergency Response Coordinator shall document the event including the nature of the event, the
adequacy of employee responses, the response of emergency response authorities, and any witness
statements. The managers shall compile the reports together with an assessment of damages, if any.
If the facility cannot be used as planned, customers will be directed to transport concrete, cement, soil and
aggregate materials to the nearest non-metallic mineral processing facility.
An accurate record of any emergencies or unusual events will be maintained at the facility.

Section 5: Equipment
Section 5.1: Equipment List
The Equipment List in Tab 10 is the most current list of equipment used for material processing activities at the
facility. SNHD will be notified of any change in equipment within 30 days of the change.
Section 5.2: Equipment Maintenance
Equipment will be inspected daily to ensure they are in good condition. Preventive maintenance of processing
equipment will be performed daily.
In the event of equipment break down, the equipment will be repaired promptly, and substitute equipment will be
obtained as needed.

Section 6: Facility Access


Access gates for the site will be closed and locked during non-operating hours.

Section 7: Facility Capacities


The quantity of wastes accepted by the facility will be limited to ensure that the facility will not exceed the storage
capacities of the facility for unprocessed wastes and the processing capacities of the facility. Processed wastes
will be removed from the facility with sufficient frequency to ensure that the facility will not exceed the storage
capacities of the facility for processed wastes.
The total processing capacity will be 9,625 tons/day.
The facility is expected to remain in operation indefinitely.

Section 8: Litter Control


Wastes will be contained within designated containers. Employees of the site will be responsible for their own
litter and will dispose of it in an appropriate manner.
The facility will be inspected at least daily to collect any scattered lightweight debris and to clean the facility.

Section 9: Odor Control


Wastes managed by the facility will not have any odor that causes a public nuisance.

Section 10: Public Areas


This facility will not be open to the public.

Section 11: Traffic Control


The operator will direct the flow of traffic through the facility and will ensure that speed will be restricted to 5 miles
per hour inside the facility. Additionally signs indicating the entrance and exit of the facility will assist in controlling
traffic.

Section 12: Vector Control
Wastes managed by the facility will not include any putrescible waste.
The facility will follow best management practices to prevent rodent infestations and to clean up after rodents.
This includes (a) eliminating any nesting materials, (b) preventing rodents from entering storage rooms and
employee facilities by sealing all openings greater than a quarter of an inch, and (c) using wet cleaning methods
involving a 10% bleach solution (1 and a half cups of household bleach per gallon of water, or 1 part bleach to
nine parts water) or equivalent solution to remove rodent droppings and urine.
The operator will keep the openings in the containment walls around the outdoor storage areas clear to help
prevent the creation of standing water. Standing water will not be allowed to exist for more than 72 hours during
the mosquito breeding season (March to October).

The reportable quantity will be determined by weight
measurement:
A. After solid waste with the reportable quantity is
loaded into a container, a floor scale at the location
shown on the Plot Plan in Tab 8 or a certified truck
scale at the facility will be used to measure the
gross weight in tons of the container. The weight of
the solid waste with the reportable quantity will be
calculated by subtracting the tare weight of the
container from the gross weight.
B. The reportable quantity for a calendar quarter or
year will be the weight of all solid waste with the
reportable quantity removed from the facility during
the calendar quarter or year, respectively.

Waste measurement procedures


x The reportable quantity will be determined by weight

The operator will maintain an accurate record of the previous calendar quarters reportable quantities at the facility
for at least 3 years.
A recycling measurement survey form (similar to forms A and B available on the Internet at
http://nevadarecycles.gov/main/forms.htm) provided by SNHD will be completed and returned to SNHD by
February 15 of each year to report the previous calendar years reportable quantities.
SNHD will be provided with a report by the fifth working day of January, April, July, and October to report the
previous calendar quarters reportable quantities.
