account with a user name and password of your choice.

IMPORTANT INFORMATION REGARDING YOUR FOOD STAMP BENEFITS
You did not have to complete the interview or provide all eligibility information before we approved your initial benefits
because you met the criteria for Food Stamp expedited services. Before you can receive any more Food Stamp
benefits, you will need to complete the interview or provide the following information:

You have until July 09, 2012 to complete the interview or provide the information listed above. If you do not complete
the interview or provide the information by this date, you will not receive any additional Food Stamp benefits, and your
Food Stamp case will close without advance notice.
If your case is closed because you did not provide the requested information, it will need to be provided the next time
you apply for Food Stamp benefits.
If the information you provide makes you ineligible or reduces the amount of your Food Stamp benefits, we will take
action without advance notice. If you apply for Cash Assistance and are approved, your Food Stamp benefits
may change without advance notice.
If this is the first time you have been approved for food or cash benefits, your EBT Card will be mailed to you. If you
received benefits before and had a card but have lost or misplaced it, please call EBT Customer Service at
888-356-3281 to ask for a replacement card.

Here is some important information about public assistance programs:

You have the right to ask for a hearing before a state hearings officer. You can bring with you or be
represented at the hearing by a lawyer, relative, friend or anyone you choose. If you want a hearing, you must
ask for the hearing by writing, calling the call center or coming into the office within 90 days from the mailing
date at the top of this notice. If you ask for a hearing by the end of the last day of the month prior to the
effective date of the adverse action , your benefits may continue at the prior level until the hearing decision.
You will be responsible to repay any benefits continued if the hearing decision is not in your favor. If you need
information about how to receive free legal advice, you can call the ACCESS Florida Customer Call Center toll
free at 1-866-762-2237 for a listing of free legal agencies in your area.
In accordance with Federal laws and State policy, the Department of Children and Families is prohibited from

income Tax Credit up to $ 5,751. For more information on where to find free

You may also be eligible to receive a $ 12.75 discount on your monthly phone bill through
Floridas Lifeline Assistance Program. Please call your phone company or the Florida Public
Service Commission at 1-800- 342-3552 for enrollment information. You may provide this letter
as proof of your Food Assistance, Temporary Cash Assistance or Medicaid eligibility if you
have been approved for benefits.
