1.0 Visit to PHC Devkaran Na Muvada ;

To display sign board at the entry of the village gate with facility name and
contact numbers in visible fonts from distance. Also, signages on the road
approaching to the PHC are to be displayed.

To carry out Cleanliness Drive of PHC campus. Taking in to consideration of

huge campus of the PHC, Beautification of the PHC can be undertaken by
plantation and gardening. District QAMO shall visit and ensure the
compliance for the upkeep of the PHC-Dev Muvada.

2.0 Visit to PHC Kadjodara;

Ensure compulsory Head Quarter Stay of Doctors and AN Ms.

Normal Deliveries should be conducted at all the PHCs.Make necessary

arrangements for same and report compliance.

Ensure timely data entries of service deliveries in e-Mamta software on

priority on completion of the services. I.e. Timely data entry as soon as
services are being provided. Further, Month wise e-mamta work plan could
not be generated by DEO during the visit. Ensure generation of work plan in
advance and issuance of it in the field on mamta Divas and follow up of the
work plans.

Ensure implementation of cleanliness drive in the PHC and optimal

utilization of available resources of the hospital i.e. Equipment supplied for
the NCO programme were dumped in to the store room along with other
important equipment.

Refrain from making changes in basic infrastructure. i.e Seating space for
FHW created in the general ward. It should be revisited.

Ensure availability of format of Maternal Death Autopsy at all PHCs.

Arrange the display of IEC materials programme wise. I.e. common IEC
display pertaining to RCH, IEC display of TB/Leprosy and IEC of Malaria!
Dengue and Chikunguniya separately at appropriate places.

3.0 Visit to CHC-Dehgam

Undertake repairs at the entrance of OPD and general ward locally. Also,
ensure general upkeep of the ward, white oil paint color coating of all the
Hospital cots and bedside lockers.

Inquire in to non-supply of common injectable at the CHC and ensure the

smooth supply after checking indent in liaison with GMSCL.

Explore possibilities of making availability of services of gynecologist under

CM SETU Yojana.

Facilitate the process of making Blood storage units functional on priority

To submit a minimum standard set of requirement at CHCs.

CPO along with DD (RH) shall visit the CHC to understand the land dispute
with Taluka officials and submit a comprehensive report of the same.
