PREVENTIVE MEASURE
1. All hand tools shall be kept in good
condition. They must be kept clean
and stored properly. Handles shall be
inspected frequently for damages and
shall remain tightly secured. Defective
tools shall be removed from service and
repaired or disposed.
2. Train the personnel on how to handle
and operate the equipments or tools
properly.
3. Provide adequate shoring and
bracing system to protect the wall from
collapsing.Wall shall be sloped when
excavating in an open area.
4.Provide hard barricades around the
excavation site and bar any unwanted
personnel, provide ladder for access
in and out of the excavation. Ladder
shall be extended min.1m. above ground
level.Provide handrail at least on one
side of the ladder.
5. Daily house keeping shall be done.
6. Conduct regular check of equipments
to maintain good operating condition
and avoid breakdown during operation.
7. Barricades the excavation site.
Assign one trained person to lookout
and give signal to operator/driver.

3. Guys, ties, and braces shall be installed at the closest horiontal member
to the 4:1 height and be repeated vertically at locations of horizontal members every 26 ft. or less thereafter.
4. The top guy, tie or brace of completed
scaffolds shall be placed no further than
