FUNCTIONAL HEALTH PATTERN

Self-Perception and Self-Concept Pattern


Assessment is focused on the person's attitudes toward self, including identity,
body image, and sense of self-worth. The person's level of self-esteem and response to
threats to his or her self-concept may be identified.
1. History
a) How to describe self? Most of the time, feel good (not so good) about self?
b) Changes in body or things you cant do? Problem to you?
c) Changes in way you feel about self or body (since illness started)?
d) Things frequently make you angry? Annoyed? Fearful? Anxious?
e) Ever feel you lose hope?
2. Examination
a) Eye contact; attention span (distraction).
b) Voice and speech pattern; body posture.
c) Nervous (5) or relaxed (1); rate from 1 to 5.
d) Assertive (5) or passive (1); rate from 1 to 5.
Example:
Vision: Wears glasses for reading but sometimes vision is blurred. Denies
itching, excessive tearing, discharge, redness, or trauma to eyes.
Hearing: Does not wear hearing aids. Does not ask for questions to be repeated at
normal hearing level.
Smell: States she/he has no decrease in smell. Denies pain, allergies, nosebleeds,
or discharge.
Touch: States her/his feet often feel numb.

Taste: States she/he has been adding more salt to her diet because his/her food
never tastes good.
Pain: Denies any pain felt over entire body.

Roles and Relationship Pattern


Assessment is focused on the person's roles in the world and relationships with
others. Satisfaction with roles, role strain, or dysfunctional relationships may be further
evaluated.
1. History
a) Living alone? Any family? Family structure (diagram)?
b) Any family problems you have difficulty handling (nuclear or extended)?
c) Family or others depend on you for things? How managing it is?
d) When appropriate: How family or others feel about illness or hospitalization?
e) When appropriate: Problems with children? Difficulty handling them?
f) Belong to any social groups? Close friends? Feel lonely (frequency)?
g) Things generally go well at work? (School)?
h) When appropriate: Income sufficient for needs?
i) Feel part of (or isolated in) neighborhood where living?
2. Examination
a) Interaction with family member(s) or others (if present).
Example:

Married for 20 years. Lives with husband. Have 5 grown children, three of whom
live very close to her. They are very caring and visit often.
Two children that are away call very often. She is the fourth of 8 children.

Sexuality and Reproduction Pattern


Assessment is focused on the person's satisfaction or dissatisfaction with sexuality
patterns and reproductive functions. Concerns with sexuality may he identified.
1. History
a) When appropriate to age and situations: Sexual relationship satisfying? Any
changes? Problems?
b) When appropriate: Use of contraceptives? Any problems?
c) Female: When menstruation started? Last menstrual period? Any menstrual
problems? Para? Gravida?

2. Examination
a) None unless problem identified or pelvic examination is part of full physical
assessment.
Example:
Menache 14 y.o.
Use of condoms for contraceptions; sometimes natural methods.
Obstetric history: Gravida 5, Para 5, Abortions 0
Children living, five all adults, 3 reside in close proximity to client

Coping and Stress Tolerance Pattern

Assessment is focused on the person's perception of stress and on his or her coping
strategies Support systems are evaluated, and symptoms of stress are noted. The
effectiveness of a person's coping strategies in terms of stress tolerance may be further
evaluated.
1. History
a) Any big changes in your life in the last year or two? Crisis?
b) Whos most helpful in talking things over? Available to you now?
c) Tense or relaxed most of the time? When tense, what helps?
d) Use of any medicines, cigarettes, drugs, alcohol?
e) When (if) having big problems (any problems) in your life, how do you handle
them?
f) Most of the time is this (are these) way(s) successful?
2. Examination: None
Example:
States becoming overweight creates great stress.
Facial muscles and arms are tense when experiencing trouble or problem.
Occasionally drinks when under stress.
Talking it all out to someone really close to her can most of the time pacify her
in time of distress.
States, it really helps when theres always a support person you can hold on
to.

Values and Belief Pattern


Assessment is focused on the person's values and beliefs (including spiritual
beliefs), or on the goals that guide his or her choices or decisions.
1. History

a) Generally get things you want from life? Important plans for the future?
b) Religion important in life? When appropriate: Does this help when difficulties
arise?
c) When appropriate: Will being here interfere with any religious practices?
2. Examination: None
Example:
Religious orientation is Roman Catholic since youth.
Goal driven and believes that religion plays a really huge importance when it
comes to dealing with hardships in life and in dealing with success as well.
3. Other concerns
a) Any other things we havent talked about you would like to mention?
b) Any questions?
