TITLE: HSE Management System

1.1 General Requirements

Horizon Offshore, Inc. (Horizon’s) Health, Safety and Environmental Management System (HSE
Management System) has been established and will be maintained so as to comply with all of the
requirements of OHSAS 18001:1999 - Specification for the implementation of Occupational Health
and Management System (OHSAS 18001) publication and ISO 14001:1996 – Environmental
Management Systems – Specification with Guidance for Use.

The HSE Management System has been integrated into Horizon’s overall management system. It
influences everything we do as a company.

Horizon Offshore, Inc. executive management will provide strong, visible leadership and
commitment, and ensure that this commitment is translated into the necessary resources, to develop,
operate and maintain the HSE Management System and to attain the policy and strategic objectives.
Management will ensure that full account is taken of HSE policy requirements and should provide
support for local actions taken to protect health, safety and the environment.

Horizon will sustain a company culture that supports the HSE Management System, based on:

• Belief in Horizon’s desire to improve HSE performance.

• Motivation to improve personal HSE performance.
• Acceptance of individual responsibility and accountability for HSE performance.
• Participation and involvement at all levels in HSE Management System development.
• Commitment to an effective HSE Management System.

Employees of both Horizon and its subcontractors will be involved in the maintenance of such a
supportive culture.

1.2 Documentation Requirements

1.2.1 General

The Horizon Offshore, Inc. HSE management system documentation shall include:

a) Documented statements of HSE policy and HSE objectives (see Sections 2.0 and 3.3).
b) This HSE Management Manual.
c) Documented procedures required by OHSAS 18001 and ISO 14001.
d) Documents needed by Horizon Offshore, Inc. to ensure the effective integration of HSE
management into planning, operation and control of the processes necessary to provide
services and products that meet customer requirements and expectations.
f) Control of HSE records required by OHSAS 18001 and ISO 14001 (See 1.2.4.).

1.2.2 HSE Management Manual

The Horizon Offshore, Inc. HSE Management System is based upon a three-tiered document
approach consisting of the HSE Management Manual, the HSE Management System
Procedures, other Operating Procedures, Work Instructions and HSE Forms (see figure 1).
The HSE Management Manual, HSE Management System Procedures, Operating
Procedures, Work Instructions and HSE Forms are controlled documents.

The HSE Management Manual addresses the implementation of the Occupational Health and
Safety Management System defined by the OHSAS 18001 Standard and the ISO 14001
Standard as they apply to Horizon Offshore, Inc. operations.

1.2.2.1 HSE Management System Procedures

HSE Management System Procedures are written to describe how the requirements
of the HSE Management System will be implemented.

1.2.2.2 Operating Procedures

Operating Procedures are written to describe how key operational requirements of the
Management System, including integrated HSE requirements, will be implemented.

1.2.2.3 Work Instructions

Work Instructions are written to provide directions on how to accomplish a task or

sequence of operations and will be written based upon the complexity, criticality,
skill level requirements and sequence of operations to be performed.

1.2.3 Control of Documents

Horizon Offshore, Inc. has established procedures to control the documents required by the
HSE management system (HSE-P-101 ‘Control of HSE Management System
Documentation’). The procedures shall define the controls necessary to:

a) Approve the documents for adequacy prior to issue.

b) Review and update, as necessary, should revision be required and approve the documents
prior to re-issue.
c) Identify changes to the documents and the current revision status of the document.
d) Ensure that the relevant versions of applicable documents are available at the point of
usage.
e) Ensure that documents remain legible and readily identifiable.
f) Ensure that documents of external origin are identified and their distribution controlled as
required.
g) Prevent the unintended use of obsolete documents.
h) Identify obsolete documents by suitable means if they are retained for any purpose.

1.2.4 Control of Records

Horizon Offshore, Inc. has established a procedure to control records necessary for effective
operation of the HSE Management System (HSE-P-101 ‘Control of HSE Management
System Documentation’). These records shall be established and maintained to provide
evidence of conformity to requirements. The procedure defines the controls necessary for:

a) The identification of required records.

b) The storage and protection requirements for these records.
c) Retention times for these records.
d) Disposition of these records.

The procedure specifies the controls required to assure that records remain legible,
readily identifiable and retrievable.
