1. INTRODUCTION
1.1

General
Emergency is unexpected abnormal potentially resulting in serious injury or
fatality to people, environmental impact, property damage, business
continuity and reputation impact and impact on neighboring industry and or
community.
This document describes of a foreseeable Emergency Response Plan (ERP)
during provision of geophysical survey services at South Sesulu located on Makassar
Strait, East Kalimantan, Indonesia.
The ERP establishes an organizational
structure, outlines actions and procedures and identifies available resources

to provide an effective and safe response. The document also outlines the
procedures to notify and communicate with lease holders, neighboring
facilities, emergency services and the community if the emergency has the
potential of involving or affecting them.
This document is designed to be a guideline for all project workforces during
Provision of geophysical survey service on how to response when a
hazardous situation and/or an accident occurred on the Vessel.
1.2

Objectives
Whilst the ERP provides details for the appropriate response in most
expected emergency scenarios, the On Scene Commander will maintain
overall command during the event and will determine the necessary actions
most appropriate for the specific circumstances encountered. His decisions
will be based solely upon the following objectives:
Preservation of life
Preservation of environment
Preservation of equipment

1.5 Definitions
Emergency

Unexpected condition resulting in serious injury or fatality,


fire, explosion, spillage of oil or chemical, toxic gas release,
structural damage, total evacuation, collision at sea &
other marine incident, accident/incident at the Support
Vessels, Loss of Radioactive Material, Security breaches,
helicopter crash, etc. which require prompt action to
contain the emergency.
The emergency is categorized as follows:
Major Emergency: The emergency that requires external
assistance after the effort of the "Site Emergency Response
Team to control the situation has not been successful.
Minor Emergency: The emergency that could successfully
be dealt with by the "Site Emergency Team" with the
equipment and facilities available at the work-site, without
any external assistance.
The emergency however, may escalate into
emergency should the ability to contain is lost.

The group of personnel at the site, who are responsible to


the OSC for the containment of the emergency at the site.

The Project Manager shall assume the duties and


responsibilities of the ERDM at the Project Office. He is
assisted by the emergency support team to liaise and
provide the necessary support for the management of
emergency that occurred at the site as required and on call
basis.

1.6 Principles of Emergency Management


The emergency management at the site is classified as follows:
Stage 1:
The ERT is activated to contain the emergency. All non essential personnel
are to muster at the assigned "Muster Stations" for head counting and to
provide assistance as required. The Vessel is to move away from the site
after the personnel are counted for.
Stage 2:
If the emergency is not contained within the accepted period, the OSC may
activate Stage 2 procedure for partial evacuation of non essential personnel
to the supporting Vessels.
The ERT will be remain onboard the Vessel to contain the emergency. The
evacuated non essential personnel however will form the support and
replacement for the ERT if the need arises.
Stage 3:
Stage 3 is the total evacuation and abandonment of the Vessel when the
possibility of containing the emergency is beyond the capability of the Vessel
personnel and equipment In such an event, notification of "May Day" is
transmitted on the assigned marine communication and brief information to
all related parties using satellite phone from main vessel to the other
existing parties near location (e.g. Local fisherman activity, fish net, rumpon,
etc).

3.1 Station Bill


Emergency alarms are given by general alarm bell and will be followed by
Public Address (PA) general announcement explaining the immediate
instructions to be taken. The following alarm signals will be activated in
each type of emergency:

3.2

TYPE OF ALARM AND/OR ANNOUNCEMENT


Seven (7) short blasts and one (1) long blast followed
by general alarm bell with details of the emergency
announcement.
The abandon alarm is a signal for all personnel to
muster at their lifeboat stations. Lifeboat crews will
prepare the lifeboats for launching.
Intermittent / Continuous ringing on fire alarm followed
by general alarm bell with details of the emergency
announcement.
Continuous ringing on the sjps alarm bells followed by
spoken words for the type of emergency on the P.A
system

Verbal instruction from the master "Abandon Ship"


In teh absence of master, then person next in command

Notification
An emergency is an unplanned event which may result in a catastrophic
event. Prompt proper actions are required to be taken to minimize the
consequence of this event. This also requires proper and prompt
notification of the event to correct organization level.
In the event of any case of emergency, Master Vessel will inform Client Rep.
Onboard and Party Chief Onboard. He will decide whether the MEDIVAC should be
activated or not depending on actual situation, all communication between
dedicated person in charge on vessel will be using through radio marine VHF/Ch.
09.
The purposes of the notification are:
1. To report timely and correctly to the authority of the event
2. To mobilize any assistance required by the site to control the event
3. To report to corporate organization on the event so that further
assistance can be mobilize promptly
4. To notify timely to the relevant Government Authorities (SKK MIGAS)
and other related parties.

Medical Injury/Evacuation
The survey vessel is a l s o provided with clinic, medicine and life saving
equipments. Assigned person (Chief Officer / HSE Officer Onboard) will report
patient condition to the OSC.
The decision for a medical evacuation shall be made by the OSC, based
on medical advice and consultation with a shore based referral hospital
doctor. A medical evacuation may be required as a result of serious
accident, illness or death of a person.

Man Overboard (MOB)


Upon sighting of a person overboard, the observer must immediately:
1. Throw the nearest life ring, as close as possible to the person in the
water.
2. Hail someone nearby to raise the alarm and notify the bridge (only as
last resorts will he abandons his watch of the man to raise the alarm)
3. Maintain constant watch of the man in the water. Keep pointing at the
location where the person Go into water, to help direct rescue boat.

Any casualty rescued from the water should be sent to town as soon as
his condition is stabilized for thorough medical examination.
His/her condition shall be constantly monitored at all times while
hospitalized on the vessel.
IN THE EVENT OF AN UNSUCCESSFUL RESCUE ATTEMPT:
If the rescue attempt is unsuccessful i.e. inability to retrieve person or
visual contact lost with person - the person in charge (PIC) should then:
1. Inform the local marine authority.
2. Broadcast for assistance on emergency channel.
3. Deploy all vessel personnel around the perimeter of the vessel to scan
for the missing person.
4. Ensure the prevailing weather conditions are known i.e. current /
wind speed and direction.

Collision with Vessel


In the event of a collision with other vessel, the OSC should:
1.

Evaluate the extent of damage caused by collision and act


accordingly to secure the immediate safety of the vessel and crew
e.g. abandon vessel if required.

1. Attempt to contact the vessel by audio and visual signs and warn
the vessel that it is within a prohibited area.

The order to stand down the fire party and to resume normal ship routine will be
taken by the Master and will be indicated verbally or by 3 short blasts on the ships
horn or bells.

Fire or explosion in the engine room would most likely be an electrical or


fuel fire. One of the first immediate actions in containing the fire will be
to stop down ventilation fans, shutdown fuel pumps and the
engines/generators by activating the ESDs (Emergency Shut Down).
For small fires that do not affect the engines or generators (i.e. generators
are still active), the fire should be fought using available portable fire
extinguishers.
For fires that affect the engines and generators, the main engines must be
shut down immediately. In this Case, water hoses can also be used in
conjunction with the portable fire extinguishers.
As a last resort action in which the engine fire cannot be contained by the
Fire Fighting Team:
Evacuate all personnel in the Engine Room
Close all doors securely
Activate the engine room Fix fire suppression system
Full fire fighting suit and breathing apparatus is required when fighting fire
in the engine room, and when entering the engine room after CO2 has
been released. Note that sufficient time must be given after activating CO2
system before re-entering the engine room.
4.8 Fire in the Switch Gear Room
Small fires in the switch room can be extinguished by portable
extinguishers. In the event of an uncontrollable switch room fire, the switch
room must be shut down.
4.9 Fire in the Galley
Fire in the galley can be due to either electrical, oil or common combustible
material sources. In case of fire happened the shutdown of electrical power
and ventilation to the galley should be conducted.
Afterward fire fighting team in the galley should immediately take the action
to contain the fire and should be equipped with a full fire suit and breathing
apparatus.
4.10 Total Loss of Vessel Power
GENERAL
The emergency power will start up as soon as main power is lost.
COURSE OF ACTION
In a Total Power Loss situation or Dead Ship (particularly is total power loss
occurs at night), the priority will be to ensure the safety of personnel on

4.12.1 General
Emergencies resulting from sabotage, riots, criminal acts or situations
of similar nature require assessment of the situation at the time for
planning of actions required. Often this would result in carrying out one or a
few of the procedures addressed in this binder.
4.12.2 Sabotage, Riots
The objective when encountering sabotage or riot situations is to stop the
trouble. The decision to take any action shall be made with the above three
priorities

4.12.3 Bomb Threat


A bomb threat or terrorist call is unlikely to be received directly by the
vessel. The threat will probably be directed to the shore-base.
All
communications regarding a threat must be kept strictly confdential
without any reference to a bomb threat made over the radio. Any publicity
on a bomb threat should be avoided because it can involve further threats
and even prank calls which unnecessarily increase the perceived danger.
1. If the threat is directly received by the vessel, ensure that all
particulars are noted in the Particulars on Bomb Call.
2. Inform the CTR Project Manager to notify police and/or armed forces
bomb disposal units.
3. The OSC will discuss situation with Liaison Officer Personnel.
Consider evacuation of non- essential personnel.
4. Stop all work and secure survey operations.
5. Sound the general alarm, requesting all personnel to proceed to mess
rooms.
6. Follow shore-base or Police instructions.
7. Search priority should be given to the accommodation areas. This will
ensure safe evacuation if required.
8. Note that radio transmitters can trigger detonating mechanisms
4.13 Serious Criminal Acts
1. Any action such as murder, serious assault, unauthorized drug use,
arson, and serious theft, shall warrant immediate notification of the
police.
2. The OSC and other personnel will proceed to the scene of the crime
to gather as much information as possible to make certain a crime has
been committed.
3. Obtain names of suspect and witnesses. Restrain any suspect using the
minimum amount of force necessary. Organize suitable detention of
suspect.
4. Notify CTR project manager and liaison officer for advice and assistance.
5. Photograph of the scene of crime and seal off area.
6. Make arrangements to transport suspect onshore
4.14 Bad Weather
Typhoons are usually detected in advance before they pose serious
danger to the vessel and the personnel onboard.

Warning of an incoming typhoon is typically issued by the weather


forecasting bureau through the daily weather report or as an emergency
alert that is updated every hour.
The four hourly sea condition and vessel response report maintained by
the Barge Department would also be able to detect some changing trends
in the weather pattern that might suggest an incoming typhoon.
The specific course of action and when to carry them out is best left to the
OSC, in consultation with the client representative to decide, depending on
the exact situation, consider taking shelter

MEDICAL CRISIS
The content of the following procedure shall always be adhered to if
circumstances so permit. The First Aider Onboard (HSE Officer) in charge of a
vessel, at his role discretion liaise with Chief Officer, request the mobilization and
aactivated MEDEVAC by Master Vessel and deliver all information's to Company
Site Representative and Party Chief in the event of such need at site.
The MEDEVAC procedure is to be used for the evacuation of personnel injured or
taken seriously ill at an offshore site and vessel to onshore for further medical
treatment.

personnel onboard include one (1) Liaison Officer and one (1) Security Officer

5.7 First Phase Evacuation Summary


In this phase, the patient will be moved by MV Mahakarya Geo Survey vessel
to in transit care facility / patient handover point that has been pre-decided
and mutually pre-agreed PRIOR to the mobilization of the evacuation
response plan. The movement will occur via transport provider identified by
MGS, again prior to the mobilization of the evacuation response plan.
In the event of Medical Evacuation cases occurred, the MV. Mahakarya Geo
Survey will stop the project and intentionally to be used for Medical
Evacuation Transport to nearest hospital at Balikpapan base on Client ERP
suggestion. Travel time is approximately 4 hours from site to the hospital,
and travel time approximately 30 minutes from hospital to Sepinggan airport
Balikpapan. Alternatively patient could be transfer to another hospital which
have more compete facility to receive further treatme

The in-transit care facility may not be- and in remote area usually will not
be - the medical facility best suited for the patient's definitive treatment.

Medical facilities recommended only for initial and stabilization prior to


onward transfer for more serious case; but in any case represent the best locally
available option.

5.8 Second Phase Evacuation Details

In this phase the patient will be moved by Hospital medical care team to a
definitive care facility hospital recommended as company suggestion in
Surabaya, depending on the medical condition/indication of the patient on
logistical criteria as well as resources available.

Noteworthy Points to Consider for Evacuations and Repatriations


All emergency responses are on a best effort basis; unavoidable delays may
occur for several reasons, for e.g. if aircraft is not available, if the weather does
not allow safe flying or a safe voyage, if an airport is closed due to runaway
maintenance, etc.
If the patient is sick with an infectious disease, health authorities may not allow
the transport of the patient either locally, regional or internationally.
Second Phase evacuation to a place where the patient's language is spoken or
where he is culturally more comfortable can be arranged upon request, if this
considered medically safe. Repatriation following an initial evacuation to a
center of medical excellence depends on the patient's fitness to fly as well as
the medical clearance of the patient by the commercial airline's medical
department

In the event that it was necessary to execute a partial or total evacuation of the
vessel or site, the main method of personnel evacuation is carried out via life
rafts, which are located at strategic locations at various parts of the vessel or site,
is only to be considered as the final alternative; should the access or availability
of other means of escaping the catastrophe could not be safely obtained.
The OSC or person in charge of the vessel or site shall at his sole discretion decide
on the method to be used for partial or total evacuation and inform the CSR of his
decision.
His decision will of course be influenced by the nature of the emergency,
prevailing weather conditions, availability of the evacuation car, etc. The design
of vessel could also influence any such decision. Personnel should be evacuated
immediately by the most appropriate method under the prevailing circumstances,
without undue delay which could put personnel at additional risk.
6.2 Evacuation Procedures
In the event of major incident on the vessel that could result in the total loss of
the vessel or render it temporarily uninhabitable, it will be necessary to partially
or totally evacuate the vessel.
The OSC or person in charge of the vessel shall be solely responsible for deciding
if evacuation should take place, although guidance may be sough from the
onshore (office). Seeking such advice however, should never create a delay in
deciding to evacuate the vessel if there is any possibility of jeopardizing the
safety of personnel on board.
The OSC or person in charge of the vessel shall decide which level of evacuation
is required i.e. partial or total and by which method the evacuation will take place;
tug, supply; etc.
As mentioned the evacuation using life rafts is only considered as the last option
to escape the emergency.
The following priorities shall be used as guidelines in the event of such
evacuation. They are:

First Priority: Any visitor(s) and non essential contractor's personnel.

Second Priority: Other non essential personnel.

Third Priority: Essential personnel i.e. ERT

The above are guidelines and can be varied as deemed appropriate by the On
Scene Commander or person in charge of the vessel when he was fully appraised
the situation.
A message regarding the necessity to evacuate the vessel may be transmitted by
the vessel itself by telephone, fax or radio or alternatively from the other vessels
in the vicinity.
A distress message i.e. "MAYDAY" call may be relayed by any of these facilities
near the vicinity if the situation is deemed to be catastrophic.

The following procedures are to be followed by emergency response personnel


during any incident, which involves or may involve the partial or total evacuation
of the hook-up and commissioning.

6.3

Offshore Procedures
The OSC or person in charge of the vessel shall ensure that the following points
are addressed in the event of the partial or total.
Inform the RSES and the ERDM of the decision to partially or totally evacuate
the vessel.
If situation is catastrophic and immediate total evacuation of the vessel is
required, initiate "Prepare to Abandon Ship" alarm and issue "MAYDAY" call
either directly from own or other facilities in the vicinity.
Notify nearby other vessels (if any) of the situation and make PA announcement
regarding method of evacuation.
Ensure that all personnel are accounted for. If time and circumstances permit,
initiate the appropriate searches for missing personnel.
For partial evacuation, i.e. the evacuation of non essential personnel, the
"Prepare to Abandon Ship" alarm shall not be initiated.
Note:
Clear instruction regarding method of evacuation shall be announced on the PA system.

Shut down operations as deemed appropriate.


Endeavour to contain and overcome the emergency situation.
Control the evacuation of personnel, communicate the status of personnel
transferred, clearly stating the names and number of personnel onboard rescue
vessel.
The names of evacuated personnel must be rechecked on arrival at their
assigned destination prior leaving the vessel shall be implemented.
Note:
The OSC or the person in charge shall ensure that they take from the vessel any essential
documents to facilitate later investigation e.g. log book and the updated POB lists.

6.4 Onshore Procedures Damage Control Team


Upon notification of the emergency, the Project Management shall:

2. Receive update the situation, particularly in respect of person evacuated, the


subcontractors involved, the Organization or Authorities to be contacted.
3. Ensure continuous monitoring and correlation of personnel movements.
4. As core team members assemble, apprise them of the situation and jointly
commence activities related to the emergency.
5. Project Management upon take over of duties shall maintain close liaison with
the vessel, CPY and other effected companies and shall coordinate onshore
these assistance or support as required He shall provide directions and
instructions as to achieve these means which may include medical services,
helicopter arrangement, support vessels, etc.
