with the name of surgical operation being contemplated for each
operation and any transfusion required.

performs the closure and he is unable to locate the missing articles, he

will inform the consultant specialist immediately. If the junior specialist
fails to do so and proceeds with the closure, than the responsibility of a
retained sponge / instrument will be on his shoulders and he / she
would be liable to disciplinary action.

7. If the consultant surgeon is called for help by junior surgeons and fails
to turn up, the responsibility would rest with the consultant surgeon.
8. In order to avoid the complication of missed sponges it is advisable to
use sponges with a radio opaque marker only and the MO I/C OT should
demand these and pursue their procurement.
