Dear Client, Until now, ChildCare Careers has only hired teaching staff that already have fingerprints on file with DCFS. This has limited the number of teachers that we have been able to hire into our pool of on-call substitute teachers. In an effort to increase the size of our pool, we will be creating a new pricing category for teachers who are not in DCFSs fingerprint database. Unfortunately, because we do not have a Provider ID Number we are not able to process these teachers background checks through DCFS ourselves. Rest assured, we will still conduct a criminal background check through First Advantage on all of these teachers prior to hiring them to ensure that they have no criminal record. Attached you will find an updated price list showing the rates for teachers with fingerprints on file with DCFS and our new lower rates for teachers who do not have fingerprints on file with DCFS. These new rates will take effect starting Monday, March 31, 2014. If you request a teacher who does not have prints on file with DCFS, you may be cited with a violation by DCFS unless you follow the steps below. These instructions will be on the front on the employees personnel file, which they will carry with them on every assignment. 1. Make a photocopy of the Authorization for Background Check Form (CFS 718-E) 2. Complete section 4 of the copy and have the employee sign and date the copy. 3. Make another copy of the completed and signed form and mail it to: Dept. of Children and Family Services, Central Office of Licensing 406 E. Monroe St, Station #60, Springfield, IL 62701 4. Send the employee to an Accurate Biometrics fingerprinting location. Go to http://www.accuratebiometrics.com to find the nearest location. 5. Tell the employee to take Form CFS 718-E and a government issued photo ID. The employee will be given a receipt to document that their fingerprints were scanned. The background check results will be sent directly to DCFS, and DCFS will notify your facility of its findings. 6. Keep a copy of the receipt for your records and send a copy to ChildCare Careers Fax: 312-488-3619; Email: chicago.recr@childcarecareers.net 7. The employee may work in your facility on a conditional basis pending the results of the background check, however they may not be left alone with children outside of the visual and auditory supervision of staff until they have cleared the background check.
