Sobre la base de este texto se les pide a los alumnos que encuentren
explicaciones para la causa del dolor, sus procesos fisiolgicos y los mecanismos
por los cuales se extiende desde un rea determinada hacia otras reas del cuerpo.
Lo esencial del mtodo ABP es que los conocimientos anteriores de los
alumnos son insuficientes para que ellos puedan comprender el problema
con profundidad. Durante la discusin inicial se presentarn preguntas y
alternativas que pueden transformarse en metas de aprendizaje para el
aprendizaje individual y autodirigido. Adems, los estudiantes intentan
resolver el problema sugiriendo formas de diagnosticar y resolver el caso.
Mientras discute el tema, el grupo recibe la gua de un tutor, generalmente
un profesor del claustro. Un curso basado en problemas generalmente est
organizado temticamente, ms que por materias, y debe ser preparado por un
grupo pequeo de profesores con backgrounds en distintas disciplinas. Los
materiales esenciales para el aprendizaje son, por supuesto, las descripciones de
los problemas y una bien provista biblioteca. No obstante, referencias, ayudas
audiovisuales, y algunas clases magistrales ocasionales se incluyen tambin
como fuentes de aprendizaje pertinentes a la comprensin de los problemas. (4)
Segn Barrows, (5,6) uno de los objetivos principales del ABP es
fomentar el razonamiento clnico o las habilidades de resolucin de problemas
en los estudiantes. El supone que a travs de la exposicin continuada a
problemas de la vida real con soluciones estratgicas modeladas por sus
profesores, los estudiantes adquirirn la capacidad de evaluar los problemas de
los pacientes, descubrir qu funciona mal, y tomar decisiones sobre las acciones
adecuadas para tratar o manejar el problema.
El segundo objetivo es fomentar la adquisicin, retencin y el uso del
conocimiento. Se sugiere que el aprendizaje de nuevos conocimientos en el
contexto de problemas puede favorecer su recuperacin y uso cuando sea
necesario para la solucin de problemas similares. Schmidt y otros (4) suponen
que la activacin de un conocimiento anterior, que tiene lugar cuando un
problema se discute por primera vez, podra tener una funcin de escenario de
nuevos conocimientos que faciliten el procesamiento por parte del estudiante.
El aprendizaje fundado en problemas tambin podra llegar a reducir la
brecha entre las ciencias bsicas y la clnica, que se ensean separadamente, tanto
en tiempo como en espacio, en el currculo tradicional. Los estudiantes de un
currculo ABP podran tener mayor capacidad para organizar e integrar sus
conocimientos de ciencias bsicas con los problemas clnicos, puesto que han
aprendido muchos de los conceptos de las ciencias bsicas mientras resolvan
problemas clnicos simulados.
Bruner, (7), sugiere que cuando los estudiantes trabajan en problemas que
tienen algn significado para ellos, demuestran inters en temas relacionados con
esos problemas, inters que va ms all del estudio suficiente para pasar un
examen. Hunt (8) explica este fenmeno diciendo que la gente, al ser confrontada
con una situacin que no es fcilmente comprensible, busca informacin para
llenar las lagunas. La confrontacin con problemas significativos que no se
comprenden bien, debera motivar el aprendizaje. Hunt considera que esta
necesidad intrnseca del organismo para reducir la incongruencia es una
caracterstica innata del proceso cognitivo.
En las prximas secciones discutiremos la evidencia que est disponible
en la actualidad con respecto a la medida en la que ABP promueve estos
objetivos, en comparacin con enfoques ms convencionales.

El nfasis que el currculo de ABP pone sobre las habilidades para lograr
el aprendizaje autodirigido, podra conducir a formar profesionales mdicos ms
capacitados para mantenerse actualizados con la literatura de su profesin.
