EXTERNAL EXAMINATION:
The decomposing body is that of an adult Caucasian female measuring 65
inches from the crown to the heal, weighing 145 pounds and appearing to be
about 20 to 25 years of age.Rigor mortis is absent. The upper portion of the
body including the head and neck shows a light green-gray discoloration of
the right lower abdomen. The upper part of the chest shows sun tan which
extends over the upper portion of each breast. There is a fine patterned
abrasion on the left forehead close to the hairline with superficial linear
depressions at this point. The eyes are hazel-green and the pupils are round
and equal. There is extensive hemorrhage into the sclera on the left. There is
also periorbital hemorrhage involving the upper and lower lids on the left.
Some pink frothy fluid issues from the nostrils. The ear lobes are pierced for
earrings. The ear canals are not remarkable. The mouth contains natural
teeth in very good condition. There are distinct round 2mm. defects on the
anterior surface of each 1st. molar on the lower jaw. The tongue is grossly
normal. The neck is symmetrical and full and tissue gas is present in
subcutaneous areas. The chest is symmetrical and the breasts are large but
tissue gas is present in subcutaneous areas. There are 3 shallow abraded
zones on the chest in the midline and toward the left inferior mamillary
zone. The abdomen shows distention and there are no surgical scars. The
genitalia are adult female and pubic hair is scantly over the lower labia. No
trauma is noted in the genital region. On the right anterior thigh is a very
slightly elevated pigmented mole measuring 1/2 inch in size. The lower
extremities are not remarkable. The arms, hands and fingers show drying of
the tips and the fingernails are long. On the lower anterior thighs are small
bruises. The back shows lividity with punctate extravasation of blood
primarily on the right and pale areas are present over the left upper back and
the left buttock. On the back of the right wrist is a fine 2-1/2 inch superficial
scar. Two (2) small similar scars are present on the back of the right hand.

INTERNAL EXAMINATION:
The skin and subcutaneous abdominal fat measures 1 inch in thickness at
the level of the umbilicus. There are fractures of the rib cage involving ribs
4 and 5 on the left in the midclavicular line and ribs 3, 4, and 5 on the in the
midclavicular line. The fractured bone ends do not protrude through the
pleura. The pleural spaces each contain a small quantity of bloody fluid
estimated to be less than 200cc. The pericardial sac contains amber fluid.
The leaves of the diaphragm are at their usual levels and there is
hemorrhage into the intercostal tissues on the right lower anterior rib cage.
In the abdominal cavity is an estimated 1000 cc. of fluid blood. This is
primarily in the area of the liver and in the right and left posterior peritoneal
zones. There is also retroperitoneal hemorrhage on the right extending
inferior to the right kidney. The tissues of the abdominal cavity show
advanced postmortem decomposistion.
CARDIOVASCULAR SYSTEM:
The heart weighs 275 grams. The organ is soft and decomposed and the
chambers contain very scant dark fluid blood. The endocardium is
hemolyzed and the valves are of normal structure. No inflammatory or
degenerative valvular changes are present. The coronary artery ostia is
situated normally and the right, left and circumflex branches are all patent
and have thin elastic walls. There is no narrowing or occlusion. The
myocardium is soft and decomposed but shows no injury or other
abnormality. The aorta and its branches are normal and the pulmonary
artery and vein contain fluid blood.
RESPIRATORY SYSTEM:
The epiglottis, larynx, trachea and vocal cords are covered by slightly
hyperemic mucosa. There is no foreign material in the airway. The hyoid
bones are well calcified and there is no injury to these structures or to the
structures of the

larynx or cervical region. The right lung weighs 430 grams and the left
weighs 375 grams. The pleural surfaces are smooth and a few small
petechiae are present. The intralobar fissures are normal. Sections through
both lungs show marked congestion and edema and the cut surfaces release
a small quantity of dark blood stained fluid. There is no pulmonary
inflammation or injury.
THYROID:
The thyroid lobes are symmetrical and normal in size and no nodules or
masses are present.
LIVER:
The liver weighs 1220 grams. The capsule has a dark purple-red color and
the inferior border is sharp. There is a 3 x 1 inch irregular laceration at the
junction of the right and left lobes of the liver extending deeply into the
liver parenchyma. Extensive hemorrhage is noted at this point. The area is
located close to the gallbladder bed. The liver on section shows a dark redbrown decomposed structure in which the lobular pattern is indistinct. The
gallbladder contains scant thin amber bile and there are no calculi.
SPLEEN:
The spleen weighs 100 grams. It is soft and has a mushy consistency and
the follicles are indistinct.
PANCREAS:
The pancreas is normal in size and shape, it is soft and autolyzed.

ADRENALS:
The position of the adrenals is normal and the fat covering is intact. On
section there is softening of cortex and medullae.
GENITOURINARY SYSTEM:
The right kidney weighs 100 grams, the left weighs 80 grams. The capsules
are thin and strips without difficulty and there is hemorrhage into the
perirenal tissues on the right. On section both kidneys have well defined
cortex and medullae and there is normal distinction between these
structures. The blood vessels, pelves and calyces are not remarkable but the
renal parenchyma shows early postmortem decomposistion. The uterus are
patent and of usual size. The urinary bladder is contracted and empty. The
internal genitalia include the uterus, cervix, ovaries and oviducts. The
cervix is small and the os is close. In the uterine cavity is a T-shaped plastic
intrauterine device with a thread extending through the cervical canal. The
myometrium is firm and there are no tumors. The ovaries and oviducts are
not remarkable.
GASTROINTESTINAL SYSTEM:
The esophagus has a pale gray smooth normal mucosal lining. In the
stomach is about 1/2 pint of undigested food material in which portions of
flat carrot wedges, rice and unidentifiable but undigested vegetable matter
can be identified. Some larger fragments suggest meat fragments. No
medication or capsules are identified grossly. The mucosa of the stomach is
normal and the pylorus is distinct. The duodenum and small intestine are
not remarkable and the colon contains a small amount of soft fecal material.
The appendix is present and shows no inflammation.

SKELETON:
Fractures are present on the right 3rd. 4th. and 5th. ribs and the left 4th. and
5th. ribs, in the midclavicular line.
HEAD:
The usual coronal incision is made. There is hemorrhage into the
subcutaneous tissues of the left paraorbital zone. The temporal muscles are
normal. The surface of the calvarium is smooth and the bone is of usual
thickness. The inner surface of the calvarium has a light orange-yellow
color which is mottled. The dura is normal with no epidural, subdural or
subarachnoid hemorrhage. The brain weighs 1225 grams and is
decomposed. The sulci and convolutions are prominent and on section there
is a light green-gray color of the cortex and medulla. There is no injury to
the Central Nervous System and there is no antemortem disease. The base
of the skull and pituitary gland are normal.
DESCRIPTION OF INJURIES:
There are fractures of the right and left rib cage and there is a small amount
of bloody fluid in both pleural spaces. There is a rupture of the liver at the
junction of the right and left lobes with an estimated 1000 cc. of fluid blood
in the peritoneal cavity and retroperitoneal space. There is a hematoma of
the left orbit with scleral hemorrhage.
