(IN RE: CAPTURED IDENTIFICATION OF JTSGM MINERS)
Sir:
Please be informed that the identification of at least nine (9) JTSGM miners were
confiscated on December 4, 2014, by Mr. John Paul Ibasco of the Monitoring
Department with the assistance of Audie R. Ramos and up to the present said nine (9)
JTSGM miners are not allowed to enter and work therein to earn a living.
The reason advanced by Audie R. Ramos in confiscating the said identification of
the nine (9) JTSGM miners with the assistance of the Mr. John Paul Ibasco is that they
allegedly committed illegal actions on December 2, 2014, in forcibly entering mine adit
of TMG and conducted mining activities therein despite the presence and protest of
working miners in said area. Further, the said nine (9) JTSM miners allegedly detained
three (3) working TMG miners for two (2) hours inside the workplace.
The question now is, is it proper for Mr. John Paul Ibasco to immediately
confiscate the identification of the nine (9) miners of JTSG upon the bare request of Mr.
Audie R. Ramos without making any investigation of the truthfulness and veracity of the
request?
In order therefore that the case will not escalate that will inescapably include
Benguet Corporation, it is being requested that the identification cards of the nine (9)
JTSG Miners be released to them the soonest after all the complaint of Audie Ramos is
within the jurisdiction of the Lupon of Barangay Virac.
Moreover, Joseph T. Sibayan as the contractor of the nine (9) miners will take
responsibility of whatever action of his nine (9) miners will commit pending resolution of
the complaint of Mr. Audie Ramos by the proper forum.

AFFIDAVIT OF LOSS
I, IRISH B. PALILENG, of legal age, single, Filipino citizen and resident
of No. IB- 113 Betag, Lat Trinidad, Benguet, Philippines, under oath depose
and state, viz
1.
I finished my degree at Benguet State University, La Trinidad,
Benguet on the 19th day of April 2011 and they issued me the Original
Transcript of Records (OTR) and Diploma, which was placed in one folder
together with other school credentials and cards;
2.
However, the said folder was inadvertently lost sometime on August
2013, while I was looking for employment around Baguio City. The same must
have been left behind somewhere without me noticing it;
3.
Nonetheless, I exerted diligent effort to locate the whereabouts of
my folder but no avail. Hence, I now consider the same to be lost and beyond
recovery for all intents and purposes;
5.
I am executing this affidavit to attest to the truth of all the
foregoing and to request the good Office of the University Registrar of Benguet
State University, La Trinidad, Benguet to issue me another OTR and Diploma
in lieu of the one lost, as well as for other purposes it may serve; and
6.
I hereby hold the Office of the University Registrar of Benguet State
University free from any liability that may arise as a consequence of the
issuance of new OTR and Diploma in my favor.
