Dear Dr. Williams,
Enclosed you will find for return the speech prosthesis that Dr. Wong made for
me. It does not function. I informed you by letter dated March 20, 2008 that the
appliance does not function. You did not to respond to my letter. You did not address
any of my concerns. You failed to correct the defective speech prosthesis.
Patent advocate Jerri Wainer located and provided my medical records. (After
your assistant Sandra Dean refused my request). This is what I found:

In 1996-1997 I consulted Dr. Truner. He was unwilling or unable to
maintain the speech obturator I obtained from Dr. Blakeleys team at Oregon Health
Science University. Dr. Jeffrey E. Rubenstein, Director of the Maxillofacial Prosthetic
Clinic at the University of Washington Medical Center, had maintained my appliance up
to August 22, 1996. But once I returned to Florida the distance was too great to commute
to Seattle for ongoing regular maintenance.

On or about February 4, 1997, I consulted Dr. M. Brent Seagle, MD,
University of Florida Shands Clinic at Park Avenue, 1015 NW 56th Terrace, Gainesville,
for velopharyngeal incompetence and surgical options including a palatal extension or
pharyngeal flap surgery. Dr. Seagle examined me but did not follow up with either a
prognosis or recommendation. I am still waiting to hear from him. Dr. Seagle did not
return my phone calls.

In March, 2006, I contacted you for velopharyngeal incompetence. I
spent about 18 months with Dr. Turner and Dr. Wong, neither of whom can construct an
obturator. Dr. Turner is easily frustrated and quick to anger, which is a significant
obstacle. Dr. Wong is very pleasant but has admitted to me that she lacks the skills
needed make an obturator. So at present there is no prosthodontist capable of making an
obturator for me at the UF Craniofacial Center, College of Dentistry.
Dr. Williams, I have given the UF Craniofacial Center three opportunities to treat
me and it has failed. Your assistant, Sandra Dean, refused to provide my medical records
and made discriminatory comments. As such, I will proceed on that basis. Please keep
the enclosed speech obturator as evidence for use in possible future proceedings.
