The Insurance Commissioner of the state of Washington, pursuant to the authority set forth
in RCW 48.17.530 and RCW 48.17.560, and having reviewed the official records and files of the
Office of the Insurance Commissioner (OIC), makes the following:
FINDINGS OF FACT
1.
On January 11, 2011, Kori Lynne Koch (Ms. Koch) obtained an insurance producer
license by submitting an online application and paying a license fee of $105.25. The license fee
was rejected by Ms. Koch'sfinancialinstitution.
2.
Requests to replace the rejected payment were sent to Ms. Koch via email (January
19, 2011); hai^-copy letter (February 23, 2011); and certified letter (April 6, 2011). Ms. Koch did
not respond to the requests and did not replace the rejected payment.
3.
Ms. Koch*s Washington resident producer license, number 778644, was revoked by
order dated May 6, 2011. The revocation became effective May 26, 2011, Order No. 11-0096.

Ms. Koch has now requested to reinstate her insurance producer license.

CONCLUSIONS OF LAW
1.
By paying a license fee that did not clear the bank, Ms. Koch has violated RCW
48.17.090 (2)(d).
2.
By failing to respond to inquiries of the Insurance Commissioner, Ms. Koch has
violated RCW 48.17.475.
CONSENT TO ORDER
Ms. Koch, acknowledging her duty to comply fully with the applicable laws of the State of
Washington, consents to the following in consideration of her desire to resolve this matter without
further administrative or judicial proceedings. The Insurance Commissioner consents to settle the
matter in consideration of Ms. Koch's replacement of the 2011 rejected payment, and payment of a
fine and such terms and conditions as are set forth below.
1.
Ms. Koch consents to the entry of this Order, waives any and all hearingrights,and
further administrative or judicial challenges to this Consent Order.
2.
By agreement of the parties, the Insurance Commissioner will rescind Order
Revoking License, No. 11 -0096 and impose afineof $500, on the condition that Ms. Koch will
pay thefinewithin thirty days of the entry of this Order.
3.

Ms. Koch agrees to replace the 2011 rejected license fee payment of $105.25.

4.
Ms. Koch's failure to timely pay this license fee andfineshall result in the recovery
of the payment through a civil action brought on behalf of the Insurance Commissioner by the
Attorney General of the State of Washington.
5.
Upon receipt of the requested payments, and the submission of a new license
application and appropriate fees, the OIC will issue Ms, Koch a Washington resident insurance
producer license.
6.
Ms. Koch understands and agrees that any future failure to comply with the statutes
and regulations that are the subject of this Order constitutes grounds for further penalties.

ORDER
Pursuant to the foregoing Findings of Fact, Conclusions of Law, and Consent to Order, the
Insurance Commissions hereby orders as follows:
1.
Kori Lynne Koch shall replace the rejected payment of $105.25 and pay afinein
the amount of $500.00 within thirty days of the entry of this Order.
2.
Kori Lynne Koch's failure to make the payments within the time limit s^ forth
above and abide by the conditions of this Order shall result in the denial of her resident insurance
producer license application and in the recovery of the payments through a civil action brought on
behalf of the Insurance Commissioner by the Attomey General of the State of Washington.
3.
Upon receipt of the payments, and submission of the appropriate plication and
fees, the OIC will issue Kori Lynne Koch a Washington resident producer license.
4.
When the aforementioned conditions are met. Order Revoking License No. 11-0096
will be rescinded.
