CONSTRUCTION RISK ASSESSMENT EXECUTIVE SUMMARY

PROVISION OF TRANSPORTATION AND INSTALLATION OF OFFSHORE FACILITIES FOR SKO


PIPELINE REPLACEMENT
INTRODUCTION
1) Construction Risk Assessment (CRA) for Baram Field was conducted on the 22nd June 2011
2) The tentative works schedule are as follows:

SCOPE OF ASSESSMENT
1) The CRA covered the following specific activities only:
1) Barge Setup
2) Barge Positioning
3) Pipeline pick-up
4) Barge side walk
5) Diver metrology
6) Install new riser clamp
7) Install new riser
APPROACH
1) The construction risk assessment was performed based on the project scope of work and general
execution plan for the work.
2) The construction risk assessment process was conducted firstly by listing of major work activities
followed by listing of all risk issues associated to each major work activity.
3) For each risk issue, the consequences and causes of the risk were identified. Each risk was then
evaluated using the risk matrix.
4) Mitigation measures for each cause were then identified for implementation to further reduce the
5) A re-evaluation of the risk level was done to determine the risk reduction resulting from the
implementation of the mitigation measures.
6) A conscious decision was made to exclude normal construction safety risks and other routine
activities that are generally covered by Job Safety Analysis (JSA) during assessment.

For each risk issue in every activity, the highest rating was taken as the risk rating to describe the
worst case scenario.
A total of 8 risk issues were identified.

(R6)- (4) Drop object on diver
(R7)- Rigging failure during lifting and lowering of Riser

On overall, 0 of the risk issues fell under "Very High Risk", 1 rated in "High Risk), 5 risk issues were
rated as "Medium Risk" and 5 "Low Risk".After applying appropriate mitigation measures the ratings
were reduced as depicted below.
