fromanyBSESofficeandfillit.Youcanalsodownloadtheformfromhere.ForallotherBSESrelated
form,clickhere.
5. Photos:2Passportsizephotographs.
Theinspectorwillvisityourplaceandaskfororiginalsofallattacheddocuments.Hewillverifyapplication
andputastamponalldocuments.Hewillprovideyouwithanacknowledgementslip.
