i. INTRODUCTION AND PROJECT SUMMARY


The 13.8kV MVU container of ABB GT A2 has been damaged due to fire
accident happened on said container. As a result of further study and
recommendations, it is necessary to restore back the damaged container by
providing a new state of the art design to accommodate the loads being supplied
from the 13.8kV MVU.
It is the scope of ABB Service Co. Ltd. to perform detailed engineering

and

design, procurement, supply and deliver the materials at PP8 premises, including
installation, testing and commissioning, and put into service for a new 13.8kV MVU
container similar but state of the art design to be in line witrh the GT-A2 (ABB
11D5) gas turbine at SEC-COAs PP8 premises.
To execute this tasks, careful study and safety planning has been conducted since
the tasks is highly prone to any incident may occur during site execution.
As a result, this Safety Plan has been established to served as a guide for the
safety and loss prevention of working personnel and companys equipments.

Responsibilities of the personnel employed during the site execution with regards to
the Safety Plan for Loss Prevention are defined as follows:

1.1

PROJECT MANAGER:
The Project Manager is the overall responsible for the project safety management
assigned on this project. The Project Manager must ensure that:
The health and safety plan drawn up during the proposal till site execution must strictly
followed and that all significant health and safety risks are identified, eliminated or
minimized according to the following safety plan;

Planning and scheduling of safety activities, identifies significant health and safety
risks and eliminates or minimizes them.
Coordinate with SEC-COA Loss Prevention Division through their authorized
representative.
Occupational health, safety performance of personnel, risks assessment, shall be
implemented, monitored and reported to ABB OHS Department as well as SEC-COA
Lossprevention Division (e.g. SOTs, Tool Box Talks, Nearmiss, Injury Reports, etc.).
Insure that only competent Sub-Contractors are appointed and that their employees and
any subcontractors (if necessary) are competent to execute the works.
Insure that competent Sub-contractors and any personnel involved in this project
should undergone ABBs Safety Training prior to any site deployment or start of site
execution.
Insure that Sub-Contractors should conduct their own safety risk assessments and
document their safe working methods before start of work.
Appoint a Site Manager who is competent to effectively perform health and safety
duties set forth in the ABBs Code of Practice for Site Managers Health and Safety
Guide.
SITE MANAGER:
The Site Manager is responsible in implementation of Site Safety Plan for the
successful execution in Upgrading, Overhauling and Repairs of 380kV GIS
Circuit Breakers and is defined under as;

Ensures that Work Permit Procedures are strictly followed.

Is familiar with SEC-COA Work Permit Procedures and did undergone


the prequalification training as Authorized Permit Receiver in compliance to
SEC-COA Safety Procedure.

Recommends ideas for safety improvement to the Project Manager and


Safety Officer and shall act as an example in implementing site safety rules.

Ensures inspection of tools and equipments are done before using.

Attends job progress meetings and safety evaluation in which safety


problems are reviewed.

Ensure that all the employees/subcontractors working at site premises


shall have necessary and valid SEC-COA Authorization/Permission before
commencement of any site work activitygtnb. All workers performing the

specific task in this project such as dismantling, overhauling, erection/reinstallation, wiring works, testing commissioning activities, etc., shall be well
qualified and must have a track record of similar experience in performing the
tasks.
1.3

PROJECT ENGINEER:
The main responsibilities as project engineer in implementation of Site Safety
Plan program for the execution of this project are as under;

Ensures that Work Permit has been issued and safety procedures are
strictly followed.

Is familiar with SEC-COA Work Permit & Safety Procedures.

Recommends ideas for Safety improvement to site manager and safety


officer and shall reinforce the rules and regulations of site safety procedures.

Attends job progress meetings and safety evaluation in which safety


problems are reviewed.

1.4

SAFETY OFFICER
The safety officer reports directly to Project Manager. He is responsible for the
establishment of an effective safey program which is based upon ABB and
SEC-COA safety policy are properly implemented to meet ABB Service Co.
Ltds contractural responsibilities. His principal responsibility relatd to the
following functions;

Making regular safety inspections of job sites and taking the immediate necessary
corrective action to eliminate unsafe acts and/or conditions. Record observations
using the appropriate forms.
Providing appropriate safety advices on highlighted issues during the safety
meetings.
Assisting in the accident investigation and reporting procedures.
Implementing training programs for the supervisors, employees, subcontractors as
they apply to their specific responsibilities.
Encouraging programs for recognition of individual employee safety efforts and
their contribution toward improved work methods.
Providing advice regarding the necessary safety equipment, use of right tools, that is
required for the job at site.
Complement of general and site based safety instructions
Carry out in internal safety audits and feedback to include subcontractors.

Be familiar with SEC-EOA Loss prevention requirements and company
safety program applicable to work on which subordinate workers are
engaged.

Conducts ten to fifteen minutes tool box meeting with men before daily
work started.

Provides adequate Personal Protective Equipment to all of his team


members.

Interacts with safety officer and ensures implementation of the safety


aspects and instruction of safety officer from time to time.

Incorporates safety instructions in routine orders and see they are


obeyed.

Gives precise instructions on responsibilities for correct work methods.

Corrects unsafe acts, such as horseplay or the taking of unnecessary


risks.

Ensures that newly arriving personnel at site are properly instructed in


precautions to be taken before they are allowed to start work.

Ensures that verbal instructions are fully understood and check back to
see that they are carried out as intended.

Reports unsafe conditions of tools and equipments to site manager and


recommends for replacement if necessary.

Makes sure that all men know how to execute first aid and that proper
care is taken on all injuries, know how to call assistance in case of
emergency and nominate others to act in absence.
1.6

TECHNICIANS & SITE ASSISTANTS

Uses the correct tools and equipment for the job and use the protective
clothing and equipment provided.

Make inspection of their tools and equipment before use and shall
report to his Testing/Commissioning Engineer if found tools is defective or
not suitable to use.

Avoids any situation that may endanger himself or his workplace.

Insure that proper safety procedures are strictly implemented and


practiced (e.g. lock-out tag-out procedure, 7 steps that saves life
procedure, take 5 procedure, etc.)

Keep tools in good condition.

Maintains his work area clean at all times.

Avoid doing horseplay and abuse of safety devices, equipment, and


welfare facilities.

2.0

RULES AND REGULATIONS FOR PERSONNEL AND EQUIPMENT SAFETY


As an agreed principle and guideline, Contractor will follow all the general safety rules
promulgated by SEC-COA for its employees laid down in their ACCIDENT

PREVENTION MANUAL. As well as the ABBs Code of Practice for Site Managers
Health and Safety Guide.

Before start of the work, all involved workers will be made aware to;

Before start of the work, every personnel present at the work site will make sure that:

First aid medication is available and accessible.

Every worker will take care of his personal safety in the first instance. Safety of his coworkers and of the equipment will be given equal importance. Below guidelines and
procedures shall strictly be followed before attempting to execute any work as follows;
Work permit will be obtained from the authorized issuer from SEC-COA by any
of authorized receiver from ABB Service Co. Ltd.
All working staff engaged near live overhead lines or underground cables will
wear safety shoes, leather gloves, helmets and safety goggles. At areas of
high noise levels, workers will use ear plug/ear muffs.
All the existing electrical circuits will be taken for granted as live circuits unless
otherwise confirmed.
All de-energized electrical circuits will be properly grounded and tagged.
Adequate illumination will be made available while working at night.
Adequate signs, warning logos, identification marks will be used at work sites
during the progress of work.
As far as possible, public access to work sites will not be allowed.
Wherever materials are stored, minimum 1.5 m space will be kept to avoid
spread of fire and for inspection purpose.
In no case, smoking will be allowed at the work sites.

3.1

ACCIDENT REPORTING
In case of accident a report will be sent to SEC-COA Division Manager of
proponent division and Contractors Project Manager.
Streamline system of accident reporting to be maintained in case of fatal
injuries/Non fatal serious injuries, oral reports to be promptly followed by written
report as per the accident report within 24 hours to SEC-COA Loss Prevention
Division along with photographs if necessary.
In case of damage to plant equipment or machinery, a written report will
be submitted immediately to SEC-COA using the attached standard
forms. The Vendor/manufacturer will be furnished a copy of the report for
any damage to new equipment.
All the important telephone numbers for the nearest fire brigade, police
and hospital services will be posted near each fire extinguisher and near
all exits.

3.3

ACCIDENT INVESTIGATIONS
The Safety Office/Engineer and Site Engineer will immediately investigate all
accidents or near miss incidents within his area of responsibility. He should
complete a report and submit to the SEC COA project manager within 24
hours.
Project Manager will review all accident investigation reports to ensure the
corrective action is taken.
Accident investigator shall classify the accidents based on its type, like fall from
high level, struck by or against, caught by/in between under, contact with
electrical current, vehicular, etc.

4.0

SAFETY ORIENTATIONS AND MEETINGS


To ensure that workers fit into the companys plans for accident prevention,
safety orientations and meeting will be held as follows:
a)

Every Saturday from the First week of every month, Safety officer will
conduct a meeting, the meeting will be held to discuss and review any
unsafe act resulting in an accident. The Site Manager, Site Engineers, and
Field Commissioning Engineer will attend.

b) Safety orientation and instruction for newly arriving personnel in the job site
will be conducted by Safety Engineer or Safety Officer before he will
assigned for any work.
c)

5.0

Commissioning Engineer & supervisors shall hold a 5 to 10 minutes tools box


meeting on daily basis prior to commencement of work.

SITE PLANNING AND HOUSE KEEPING


5.1

SITE PLANNING
The ABB Existing site office or store yard shall be utilized so that working
areas, roadways, access ways, stairways and scaffold platforms are kept
free from unnecessary obstructions. Electrical cables and temporary
utility cables, conduits and pipes shall be properly arranged so that, they
do not become tripping hazards.

Good house keeping plays an important role in doing the work more
efficiently and reducing man hours lost due to accidents. Stored
materials shall be identified as to name, size, type and quantity. The
materials shall be neatly stacked as per the identification codes
mentioned above. Files/records shall be kept neatly in racks and indexed
for easy access.
Adequate number of workers will be deputed for the housekeeping as
and when required.
Smoking will be controlled within plan premises and confirmed to safe
designated places only.
Providing trash bin all around store and cleaning daily will help us to
maintain good house keeping.
To maintain effective house keeping schedule in order to maintain safe
working area, all unused and scrap materials shall be collected and
disposed of in a timely manner to prevent formation of tripping hazards
and fire hazards.
The safety signs, posters and banners are to be displayed at different
work location for awareness of the workers.

Safety Manuals.

6.0

7.0

HAND TOOLS AND POWER TOOLS


Handling and use of Hand tools and Power tools in proper manner is vital for reducing
the risks involved.
7.1

HAND TOOLS

The tools shall be of high quality.

Appropriate tools shall be used for specified works.

Defective tools shall not be used.

When work is being carried out at the elevated position, the tools That
are not in use shall be secured or placed in holders.

Throwing Tools or materials from one location to another, from one


worker to another, dropping them to lower levels shall be prohibited.

The Tools and tackles shall be protected against corrosion and


maintained to be free of defects.

All tools shall never be altered and shall be kept in proper work
conditions.

7.2

POWER TOOLS
The Tools shall be inspected regularly.
Only trained persons shall be allowed to handle the tools.
Necessary Guards shall be provided.
Use of cracked or damaged grinding wheels shall be prohibited.
The electric tools shall be provided with equipment grounding
connections.
The Tools shall be checked before use and used with extreme care &
precaution.
Check correct operating voltages and connect the power tools.
Always draw power supply from a switch board, which is easily
accessible.

8.0

WORKING PLACES AND ACCESS LADDER


8.1 WORKING PLACES
Work place shall be safe to perform the job. Key personnel shall monitor
working conditions on a daily basis.
Construction sites shall be clean, safe and enclosed (if necessary) with
temporary fencing. There shall be adequate lighting, ventilation and
sanitary systems.
Warning, safety and traffic directional signs shall be placed at appropriate
places.

Portable fire extinguishers shall be provided during welding work,


transformer installation work etc.
Project site offices shall have adequate parking facility. The access
roads shall be free from obstruction and shall have adequate lighting.
8.2

LADDERS
Ladders shall be of appropriate length. Ladders with metal reinforced side rails
shall not be used near electrical equipment with exposed conductors.
Ladders shall be inspected periodically for any damage, deterioration, wear
and tear, corrosion and structural failure.

8.2.1

POSITIONING OF LADDERS
Ladders shall not be positioned against insecure structures. They shall
be placed firmly on solid ground and the area at the base of the ladders
shall be kept clear. Atype ladders or scaffolding platforms shall be
used on slippery grounds.
8.2.2 USAGE OF LADDERS
The user shall face the ladder when ascending and descending. The
user shall not carry tools and material in his hands while using ladders.
The user shall maintain a secure hand hold or use a full body harness
when both hands are engaged in work.
If the ladder is erected closed to the doorway, the door shall be either
locked shut or be secured in the open position with a man on guard.
When an extension ladder is used fully extended, the minimum overlap
of sections shall be four rungs.
Where a ladder is carried by one man, the front end shall be kept high
enough to clear mens heads and special care shall be taken at corners
and blind spots.
Special care shall be taken while carrying ladders inside the live
substation areas.

9.0

MATERIAL HANDLING AND STORAGE

Proper attention should be paid while handling material. Ascertain whether it


can be done manually or it needs mechanical equipment. The following
conditions and procedures as below:

Ascertain the weight of the object.


Remove any sharp objects present.
Wear appropriate protective equipment.
Decide where to place the object.
Survey the area around the object and proposed route of travel.
Remove any obstruction present.
Get sufficient help to lift and transport safely.
Lift within your physical capacity. Keep feet close to object. Crouch close
to the load with knees bent. Position or keep backbone straight. Lift the
object using legs and arm muscles.
While two or more persons are transporting one extra man should give the
signals while others march with the load.

The workmen shall be trained for lifting loads manually with demonstration
during their training sessions.
9.2

STORAGE AREAS
Available site storage area as defined by the client will be used by ABB to store
materials at site.
Adequate area shall be provided for safe handling and storage of materials.
Store materials shall be identified by Name, Size, Type, Quantity.
Flammable materials such as paints, fuels, lubricants, chemicals and thinner
shall be separated from combustible materials.
Applicable/appropriate warning signs shall be displayed inside warehouse.
Fire extinguishers shall be provided in storage area.
Non compatible material shall be segregated in storage.

10.0

LIFTING AND HOISTING EQUIPMENT


The site has an available lifting equipment provided for this overhauling and
maintenance purposes. Lifting operations shall be performed by SEC-COA site
representative only or can be operated by a trained personnel capable to
operated the overhead crane after proving his track record of experience in
performing the lifting works and following the below lifting requirements;

Hoist operators must inspect equipment daily before use.

Do not operate a crane or hoist if limit switches, steel ropes, chains or other
components are worn or in disrepair.

Replace nylon or web slings immediately if excessively worn.

Associates in the direction of travel should be warned to move and remain


clear of a lifted load at all times.

Loads should not be suspended over personnel below.

Under no circumstances may anyone ride the hook or load.

Directional movement should be made smoothly and deliberately. Avoid


rapid movements in any direction.

Do not exceed the rated loadcapacity of the crane, hoist,


chain, cable, slings, or other component.

Slings, load chains and other lifting devices must be fully and securely
seated in the hook before moving a load. Remove slack from the sling,
chain, or cable before lifting a load.

Use the following practices to avoid swinging loads:


Locate the hoist directly above the lifting point of the load before lifting.
Lower loads directly below the hoist.
Keep hoisting ropes vertical. Do not pull or push the load.
Maintain two full wraps of cable on the hoisting drum.

When lifting loads at or near capacity, test the hoist brakes by returning the
master switch or push button to the OFF position after raising the load a
few inches off the floor. If the brakes do not hold, lower the load to the floor
slowly and do not operate the crane. Report the situation immediately and
DO NOT USE until repaired

Ensure that all loads are lifted high enough to clear obstructions before
moving the bridge or trolley. Whenever possible, maintain a minimum
clearance of one foot above loads and to the sides. Raise the load only to
the height necessary to clear lower objects.

Never pull a hoist by the pendant cable.

Never leave the controls unattended while a load is suspended. If it


becomes necessary to leave the controls, lower the load to the floor.

Familiarize yourself with the location of the disconnect switch.


If loss of electrical power occurs, place controls in the OFF position to
prevent unexpected startup upon restoration of power.

11.0

Disconnect power to a hoist that is unsafe or in need of repair. Arrange to


have the disconnect switch locked and the control panel tagged with and
Out of Order or Do Not Operate tag. Never operate a hoist that has
been tagged with an Out of Order or Do Not Operate tag, or is your
opinion, UNSAFE TO OPERATE.

Work permit is an authorization to do or perform certain work on an energized


equipment or part of the energized system in a restricted area. It is issued by an
authorized employee of the concerned proponent organization. The receiver will
also be an authorized person, who will be delegated the authority of receiving
the work permit issued by SEC-COA Authorized issuer. Before issuance of the
work permit both the issuer and receiver will ensure the safe working conditions
of personnel and equipment.
Before issuing and receipting of work permit, the equipment or the circuit
involved will be switched off by the on-duty switch man of the proponent
organization. Appropriate lock and tags will be placed on the equipmentunder
work permit as long as the permit is valid.
After completion of the work, the work permit will be closed-out by the authorized
issuer and receiver after observing all closing-out requirements.
The tag will be then removed by the authorized person.
Every care will be taken while restoring the electric supply. No job is more
urgent or no service is more important than the safety of the workers. Therefore,
safety of personnel must be ensured in a safe location before energizing the
involved electrical equipment or system.
In case the receiver of the work permit needs to leave the work site (for genuine
reason), he will inform the issuer and designate other authorized receiver of work
permit to look after the work in his behalf.
The work permit will be displayed at the job site.
Receiver of the work permit will stay in the work place for the complete duration
of work permit.
Issuer/receiver of the work permit shall stop the work if the facility is found
unsafe.
Work permit shall be taken before execution of the work.
The Work Permits shall be handled carefully while on the job. They shall be in
the safe custody of the personnel for whom the work permit has been issued.
For details, client guidelines G.I. # 65.111 shall be referred.

12.0

FIRE PREVENTION AND HAZARDS


12.1 FIRE PREVENTION
To avoid the risk of fire and to minimize the losses due to fire a well organized
fire prevention program will be established at site on each stages to achieve
the objectives that;

1. Fire may not occur.


2. If occurs, may not be spread.
3. If spread, the losses shall be minimum.
The fire prevention program will be contained as under;
a. No exposed wiring and lose connection will be allowed; only conduit wiring
will be permitted.
b. Distribution boards and other supply will be inspected regularly.
c. No chemicals and liquids will be permitted in the warehouse.
d. Sites will be kept clean and free of high inflammable/combustable liquids or
materials, stray wooden logs, waste paper, catrons, rubber, trash etc. for
fire prevention.
e. Smoking is strickly prohibited.
f.

Office equipments electic apparatus including A/C units, fixtures, and fans
shall be switched off before leaving the work sites.

g. During unloading of fuel, trucks engines must be stopped and trucks and
other unloading equipments should be well earthed to prevent static electric
charge.
h. Observe material segregation and spacing according to class and type in
storage area/warehouse.
12.2

KNOWING THE LOCATION OF FIRE EXTINGUISHERS


The substation is fully equipped with fire fighting equipment and it is the
rights and duty of every personnel involved in site work execution to know
the locations of existing fire extinguishers in the area. Every worker must
investigate the integrity of fire extinguishers by checking the pressure
gauges to make sure that the fire extinguishers are in proper working
capability in eliminating the fire.
All working personnel including Project Manager, Site Manager, Testing
Commissioning Engineer shall include in the tool box discussion the
locations and integrity of each extinguishers and condition their minds where
to approach to the nearest extinguisher whenever fire may happen.

a)

A first aid kit equipped with all the necessary medicines, dressing, bandages etc.
will be placed at work site. Every worker will be made familiar with the location
and use of this kit.

b)

If any worker gets injured or develops some ailment during working hours, first
aid treatment will be administrated on him immediately. If necessary, he will be
taken to the nearest hospital or medical clinic.

c)

Chemical burns will be washed with sufficient water before applying first aid
treatment.

d)

In case of electric shock, the source of electric supply shall be disconnected


immediately. The victim will be detached from the live circuit using some dry log,
rope or safety gloves. The rescure should ensure his own safety at every cost.
Artificial respiration should immeidately be applied to the victim.

e)

In case of sun stroke, the victim will be shifted to a cool area and taken to
hospital. His head will be wrapped with a cool and moist cloth.

f)

In case a person becomes unconscious due to any serious accident or shock


then he should be checked whether he is breathing or not. If he is not breathing
then artificial respiration should be immediately applied to him.

g)

Important telephone/contact numbers of hospitals for emergency and treatments


are available in attachmen F.

MEDICAL HEALTH AND WELFARE

Botlled drinking water supply shall be provided at site.

Site toilets can be utilized for workers personal necessity

Dining can be done at nearby restaurant facility since no available dining hall in
substation site.

Transporation of employees is provided by Company for conveyance of to and from


the Site. These vehicles are fully maintained properly.

Working hours is restricted to the regular eight hours per day with one hour lunch
Break. This may vary due to workload schedules. Friday, which is normal rest day,
shall be utilized if need arises.

15.0

ELECTRICAL HANDLING
15.1 General Rules Electrical Safety

Electrical equipment is subject to Electrical Safety Rules when it is


capable of being energized from a power system through operation of
switchgear, or by the replacement of fuses or links.
Electrical Safety Rules must, at all times, be applied in such a
manner as to prevent danger to persons switching, working, on, or in the
vicinity of live apparatus.
Each person within the Work Party must have the Competence to
undertake the scope of work planned for them.
Normally the customer shall undertake all actions to safely
disconnect and earth Electrical Equipment to be worked on.
Normally ABB Service personnel shall not work on live
equipment.
Work packages which require work on or very near live equipment
must follow relevant additional rules in Section 15.3.
Personnel on Site must at all times wear appropriate personal
protection equipment (PPE). Flame resistant clothing must be worn. NFPA
standard 70E will be used in the absence of local standards.
Other PPE may be used according to local regulations and risk
assessment controls.
There must be safe and proper means of escape in the event of
failure of any live apparatus.
The Nominated Person must establish prior to starting work relevant
telephone numbers for emergency services.
Service technicians may not walk on top of live indoor switchgear
panels.
Appropriate safety precautions shall be taken when working on high
equipment to avoid injury through falling.
No un-authorized person may enter the work area.

15.2 Rules for Working on Dead Electrical Equipment


safe working area must be created and maintained in accordance with the
following principles. All 7 principles must be applied on every occasion.
Step 1 : Clearly Identify the work location
a) Sufficient labels, schematics and plans must be available to clearly identify
the location and the Electrical Equipment to be worked on.
b) Where Electrical Equipment is not easily identifiable, such as cables,
suitable identification arrangements must be made by the Nominated
Person.
c) Nearby live Electrical Equipment must be identified as dangerous by the
application of highly visible temporary warning labels.
d) There must be clear and continuous identification of the safe working area. It
must be possible to easily distinguish between safe and dangerous areas
throughout the course of planned work.
Step 2 : Disconnect Completely, Secure against re-connection
a) Verify that the Electrical Equipment has been safely disconnected and
earthed.

b) A suitable electrical gap (see Section 15.7.2 & 15.7.3 appendices 2 & 3)
must be made across all points at which the apparatus may be made live
including potential in-feeds from low voltage apparatus.
c) The gap must be physically secured from inadvertent or willful reconnection, by the application of barriers or locks; or removal of fuses to a
safe place; or removal of apparatus from its normal service position; or
disconnecting/blocking stored mechanical energy devices.
d) It shall not be possible for remote protection or control to reconnect the
circuit under work through electrically closing a switching device.
e) A prominent warning notice must be fixed to each point of disconnection.
f) All keys must be kept in a secure place.

Step 3 : Provide protection against live parts


a) All live apparatus in the vicinity of the work place must be locked off, clearly
identified and made inaccessible by barriers or other means.
b) A risk assessment must be carried out to ensure that dangerous voltages
cannot be directly or indirectly created on any of the electrical circuits
connected to the point of work from nearby live circuits (see appendix 2)
including the re-arrangement of earth connections.
c) Special attention must be paid when the scope of work requires phased
outages of different circuits. The sequence of circuits to be made dead
must be clearly understood by all members of the work party. With each
change of circuit from live to dead, or vice versa:
Access control and warning labels must be changed to suit
Each member of the work party must fully understand which new
circuits are now live
d) Work on Electrical Equipment connected to overhead lines must cease in
the event of a lightning storm.
e) On wood pole lines with one or more circuits live, precautions must be
taken against steelwork being or becoming live.
f) On certain high voltage cable systems it may be necessary to take
precautions against large induced voltages, for example by fully insulated
working.
g) Precautions must be taken to prevent danger from low voltage conductors
at the point of work.
Step 4 : Take special precautions when close to bare conductors
a) Special precautions must be agreed with the Nominated Person when the
minimum clearances (Appendix 1) cannot be made to unearthed
conductors during preparation of the safe working area, which typically
occurs when entering switchgear cells, applying screens, testing and
applying portable earths.
b) The minimum safety precautions in the above circumstances are:
Wear suitable insulated gloves
Ensure access is suitable to avoid inadvertent slips.
Be accompanied by a second person who can render assistance.
Step 5 : Verify the installation is dead

a) Where the design of the Electrical Equipment allows, it must be confirmed


dead by a suitable tester at all points of work and points of application of
portable earths.
b) The tester shall be of a proper design, and proved before and after each
test.
c) Where the design of the Electrical Equipment precludes this, other suitable
arrangements shall be agreed with the Nominated Person. Typically: firing
a spike through underground cables, using proximity testers on insulated
conductors, operation of switches to earth or tracing cables or conductors
back to a visible earth point.
Step 6 : Verify earthing and short circuiting
a) The Electrical Equipment to be worked on must be connected to earth by
connections and conductors capable of carrying the full short circuit current
at that point.
b) Where possible the Electrical Equipment shall be earthed by a fully rated
switch.
c) Earths must be applied between the point of work and all possible sources
of HV supply, and they shall remain in position for the full duration of work.
d) Portable earths shall be applied to all phases and in such a manner as to
prevent danger from residual charge or induced voltages.
e) Separate earths at the point of work shall be connected to create an equipotential zone for all persons at the point of work. They may be moved
during the course of the work.
f) Precautions shall be taken to prevent danger from voltages developed
across earth conductors connected to earth at different points.
Step 7 : Issue a permit to work
a) Note: The permit to work is about control of the workplace and people. It is
a summary of the previous protective measures and hazards. It is also a
clear statement of responsibility from the Nominated Person for these
protective measures.
b) The Electrical Equipment to be worked on must be released for work
through the issue of an electrical Permit to Work at the point of work. The
Nominated Person issuing this is responsible for creating and maintaining
the safe working area in accordance with 7 steps principles.
c) The permit to work shall contain clear, legible details of:
The location of the work, and the precautions that have been taken to
establish the safe working area.
The scope of the work
Hazards in the immediate area
Completion of site risk assessment
Completion of Safety Check sheet
Signature of the Nominated Person in charge of the work party, times and
dates of issue and cancellation.
Signatures of work party members
d) The Nominated Person in charge of the work party shall be responsible for
the safety of his work area and all other persons in his work party
regardless of seniority.

e) Warning signs and demarcation equipment is not to be removed until after


cancellation or the issue of further safety documents.
f) The Nominated Person in charge of the work party shall be responsible for
ensuring all persons and tools are withdrawn on completion of work, and
hand over of the Electrical Equipment in proper condition according to the
work undertaken.
g) Where the boundary of the safe working area changes during the course of
the work, further permits to work must be issued.
15.3

Rules for working on or very near live Electrical Equipment


a) All work packages that require work on or very near live Electrical
Equipment must have prior written approval from the local Service Unit
manager, who must be satisfied that personnel have the appropriate skills
to undertake the required task.
b) The rules in Sections 11.1 11.2 shall be followed where relevant.
c) The risk assessment shall specifically evaluate whether the work can be
done dead or in some other way, and what additional special safety
measures must be undertaken.

15.4

Switching

a) Note that switching includes insertion or withdrawal of a switching device.


b) ABB service personnel may only undertake Switching operations with
specific written approval from the local Service Unit manager.
c) Any person carrying out switching must have sufficient information and
competence to understand the effect of that switching on the system, and
potential danger to himself or others.
d) Switching must only be carried out by a person properly trained and
authorised in writing, and who is familiar with the function and operation of
the switchgear concerned.
e) Switching must be carried out in conjunction with the person in control of
the system.
f) Appropriate personal protection equipment must be worn by personnel
undertaking switching operation. At minimum flame retardant overalls,
gloves and a safety visor must be worn.
g) Where possible remote switching shall be undertaken.
15.5 Work on, or very near, live conductors
a) Any work which cannot be done beyond the safe working clearances in
Appendix 1 must be the subject of a special written procedure, and carried
out by specially trained and authorised persons.
b) Work is considered Live work when it comes within the Live zone
distances in appendix 1, or if the work requires the removal of covers,
doors or barriers to expose live conductors.
c) Work must only be carried out by a person properly trained and authorised
in writing, and who is familiar with the function and operation of the
Electrical Equipment concerned.

d) Appropriate personal protection equipment must be worn by personnel


undertaking the work. At minimum flame retardant overalls, gloves and a

15.6

Testing and Commissioning


a) Where it is necessary to carry out electrical tests on Electrical Equipment,
the safe working area must be established by following steps 1 6 as
described in Section 15.2, before being released for tests by a safety
document, and earths removed.
b) The test area must be under the control of a suitably competent person and
precautions taken to avoid electric shock as in step 4 in Section 15.2.
c) Additional warning signs and barriers must be used to protect other
persons nearby from test voltages.
d) Work on Electrical Equipment shall only be done, or supervised, by a
person competent to recognize danger and apply controls from:
Live low voltage wires ac and dc
Voltages arising from current transformers, protection, pilot cables and
earths
Primary power conductors at high or low voltage
Interference with fully operational power systems
Test equipment

Minimum working clearance
The minimum working clearance is the minimum distance, in air, between a live
exposed conductor, and any part of a persons body, tool, conductor or apparatus
that a person may be touching. The clearance must take into account the nature
of the work and relative positions of the live conductor and the working area.
Where work infringes this clearance then addition control measures should be
implemented to maintain safety.
Where work infringes the live working zone then it shall be considered to be live
work.
