Fire on Premium Tank within Maintenance Activity

I. The Event Scenario

Premium Tank TK 117 typed Cone Roof 12.000 KL is on repairing condition for the leakage shell plate thus it is needed to
be repaired by Technical Maintenance using the Contractors worker. The activity shall comply with the Safe work permit
procedure of Pertamina for the work of Confined Space Entry with the first phase is Tank Cleaning and gas Freeing.
The activity within the tank cleaning phase towards the premium residual by using diesel pump placed on 3 m distance in
front of tank manhole by publishing the Safe Work Permit (Cold Work Permit).
Just a few minutes conduct pumping suddenly fire occurred surrounding the manhole of Tank and causes 1 person of
contractor gets burns due to exposed by heat radiation.
The fire caused by flame from the pump which is not using flame arrester and exposes premium vapor emerging from the
tank man hole.
Maintenance officer reports to fire station in Fuel Filling Depot.
HSE Shift Supervisor at Fire Station reports the event to the On Scene Commander and forwarded to Incident
Commander and Emergency Response Commander as well.
HSE Manager as Incident Commander commanding On Scene Commander to dispatch Fire Brigade and Rescue Team
completed with Fire Crash Tender and rescue team and ambulance as well to the scene.
Incident Commander by the approval from Emergency Response Commander declares the condition as emergency that
shall be activated as stated within emergency procedure by commanding sounds the emergency siren to fire station.

2) Security and Evacuation Operation

Strengthen the control at the Main gate of BBM Depot towards the enter/exit vehicles by
prioritizing the vehicles and officers related with the emergency response and prohibit
the unauthorized enter to the BBM Depot.

Security Officer controls the traffic particularly for the fire truck and ambulance to the
scene (Tank TK 117).

Rescue Team evacuates the workers at the scene to the safe location where U.S Fire
Fighting Practices determine the safe distance is 4 times of Fire height.
Thus, if the fire height is 6 m then the safe baseline can be appointed minimum on 24 m
from the Fired Tank TK 117

3) Rescuing the Accident Victims

After receiving information that there is accident victim, Rescue Team and Medical Team
along with ambulance straight to the scene (Tank TK 117).
The victim lays down at the bund wall of tank when arrives at the scene and Rescue
Team evacuate the victim to the safe area proceeds giving first aid by Medical Team.
The victim immediately taken away to local Pertamina Hospital to get further medical
treatment.

Emergency Situation and Condition

Fire on manhole of tank TK 117 caused by existence of vapor from the premium out
of the manhole and blended with the air outside reaching the explosion range
(explosion range 1.4% - 7.6%) and climbs up exposes the unsafe diesel pump
exhaust therefore emerges sparking and finally fire.

After receiving the fire report of Tank TK 117 then On Scene Commander commands
the Fire Station Officers to dispatch Fire Brigade complete with Fire Crash Tender to
the scene.
On Scene Commander seizes that burning substance is the gas out of the manhole
with the big fire depicts that premium oil in the tank is still copious and gas is within
too rich position (Upper Explosion range-UEL).

Firefighting conducted using 3 units of Water Spray Nozzles sized 2,5 from Fire
Water Crash Tender heading to the manhole.

The fire can be extinguished rapidly followed by cooling the manhole and shell plate
of the Tank to prevent flash back.
