1. List the two components of the Resident Assessment Instrument and briefly describe
what they are.
The components of Resident Assessment Instrument are Minimum Data Set (MDS) and
Care Area Assessment Process. First of all MDS incorporates around 450 items that address
functional and clinical aspects of patients for instance, diseases, clinical conditions, mobility,
elimination, skin, nutrition, oral health, sensory, cognition and physical functioning. The
underlying purpose of MDS is to highlight or indicate whether a patient had an issue/ problem or
not and its not used to identify the causes and factors behind the healthcare issue/ problem. On
the other hand Care Area Assessment help nurses to assess pertinent factors relevant to the
identified healthcare issue/problem such as dehydration.
2. What are possible consequences of not completing documentation required by CMS in
an accurate and timely manner?
The possible consequence of not completing the documentations required by CMS in
accurate and timely manner is that proper intervention and diagnosis care plan cannot be
implemented.
3. How soon after admission must an assessment be completed?
The patient must complete assessment within 14 calendar days of admission.
4. How often should a resident be re-assessed with the RAI?
The prospect of re-assessment depends on whether there has been any significant change
in patient condition or not. If there has been a significant change in patients condition than reassessment can take place on the basis of 7 days observation look back and 14 day observation
looks back. However if there have been no significant change in patient condition, then reassessment is completed within 366 days of the most recent comprehensive assessment.

5. How /why is culture change important in meeting CMS guidelines?


Culture change is important in meeting CMS guidelines because culture change attempts
to alter the inflexible and traditional environment of long term care facilities into a welcoming
and home-like environment and it also focuses on changing the working environment prevailing
the health care facility so that nurses are motivated and empowered to provide quality care
services to patient.
6. List two examples of a significant change that indicates a need to re-do the Resident
Assessment Instrument.
One example of significant change is that Mr. John is not responding to verbal requests of
controlling his screaming behavior and his behavior has not changed even after intervention and
treatment. He is becoming more stubborn and violent. Previously his stubborn screaming sued to
be once a week but now its on daily basis. In such case Mr. John needs reassessment as there is a
significant change. Second example is that that Ms. Roma was admitted in health care facility as
she was recovering from her injury but over the last month she had a 5% unplanned weight loss.
7. What are the four main steps in care planning?
Understanding patients care needs
Communicating with health care providers and patients family
Document care plan
Review care plan periodically
