Medical History
Synonyms for medical history: case history, anamnesis (often abbreviated hx or Hx).
The medical history of a patient is a record of medical facts pertaining to past and present, relevant to the patient's
current state of health. The facts are gathered by a physician by asking specific questions, either of the patient or of other
people who know the person and can give suitable information (in this case, it is sometimes called heteroanamnesis),
with the purpose of formulating a diagnosis and providing medical care to the patient.

How to perform an examination


To begin the examination:
