The Event Scenario

Electrical Pump of Production is in running condition to pumping the condensate oil, but suddenly the pump seal is getting
leak and emerging the oil bursts accordingly.
At the same time there is activity to replace the pipe by welding (Hot Work Permit) which is not far from the pump of
condensate oil.
The welding activity is stopped but the oil bursts exposes the heating pipe because of the welding, therefore the fire
cannot avoided and open fire is rapidly wide spread.
Welder gets serious burns and can be helped by his colleague getting far away from the fire. The accident reported
immediately to the fire station by Maintenance Supervisor using Handy Talky to get help for extinguishing the fire.
HSE Shift Supervisor at Fire station reports the emergency to the on scene commander and forwarded to the incident
commander as well as Emergency Response Commander
Incident Commander declares the situation as emergency after getting the approval from Emergency Response
Commander and commanding to sound the emergency siren therefore the team involves in Emergency Response
Organization conducting the response action at the scene.
Emergency Response Accomplished and Incident Commander by the approval from Emergency Response Commander
declares that condition at the scene is safe.

Fire of condensate oil pump caused by the leakage of pump seal thus the oil flow out
and exposed the heating pipe.
After receiving the report from fire station, On Scene Commander reports the
emergency to Incident Commander and forwarded to Emergency Response
Commander.

Emergency Response Commander and commanding to sound the emergency siren

HSE Supervisor sounds the emergency siren as the notification that emergency
occurs and all officers involve in Emergency Response Organization conducting the
task and responsibility as meet procedure.

Then the on scene commander coordinates the emergency response with Emergency
Team, Fire Brigade, Rescue Team and Medical Team at the scene .

range about 20 m if the height of fire 5m (4 times of the height of fire-U.S. Fire
Fighting Practices).

Rescuer Team and Medical Team arrives at the scene and proceeds rescuing the
victims who get serious burns with stadium 3 (welder) but still in conscious condition
and placed in safe radius.
The victim proceeds taken away to the local Pertamina Hospital to get further medical
treatment.

Incident commander orders the On Scene Commander to dispatch Foam Crash

Tender and Water Crash Tender along with Fire Brigade to the scene followed by
Medical Team and ambulance.

On Scene Commander coordinates with Production supervisor to ensure that

pumping activity has been stopped in order to stop the gusher.

Based on concerning the size of open fire with 5 m fire height therefore to extinguish
effectively, fire brigade conducting the extinguishment using Foam Crash Tender by
utilizing Foam Monitor/ Branch Pipe 500l/minute thus the fire can be extinguished
rapidly.

To prevent from back fire, the HSE Officer conducting:

- Testing and monitoring the Hydrocarbon gas condition regularly.
- Controlling the ignition source at the vicinity area.

containment and vacum the oil spill to prevent from environment pollution.

III.

Emergency Situation and Condition

Providing the logistics for the emergency response team at the scene.
Documenting the emergency response process since the dispatching Crash Tender,
arranging the traffic at the scene, rescuing the victims until the fire extinguishing
