Mauston High School Football Field August 7th 6:00 pm

(Bad weather make-up date August 8th)

Help kick off the 2015 Football Season and help raise money for the Golden Eagles Football Team. A
night of fun for all ages. There will be activities and contests available for boys and girls K- 8th grade
starting at 6:00 p.m. Watch the High School Football Companies battle it out in the team
competitions and stick around to watch former legends play in the 2nd Annual Alumni Flag Football
Game. Save your appetite and plan on eating at the field. Food will be sold in the concession stand.
Activities during the evening include a Silent auction, raffles, 50/50 drawings and a chance to
purchase football discount cards and Mauston Football sportswear. Come and meet YOUR 2015
Mauston Golden Eagle Football players.

Assumption of Risk and Release Waiver
By signing the Assumption of Risk and Release, the individual named below wishes to participate in
the Mauston Alumni Flag Football Game and recognizes that there are risks of damage or injury
arising from this event or from other activities (including travel) that may be associated with
participation in the game.
By his/her signature below, the participating individual voluntarily agrees to assume and/or incurs all
risks of loss, impairment, damage or injury of whatever kind, including death, that may be sustained
or suffered by participation in this game whether or not the result in whole or in part of acts or
omissions, negligence or other unintentional fault of the Mauston Football Team or Mauston School
District. In addition, the participant (including his/her heirs, assigns and personal representatives)
agrees to release, hold harmless, and indemnify the Mauston Football Team and Mauston School
District from and against any claims, demands, actions, causes of action, lawsuits, expenses, or losses
(including attorney's fees) on account of property damage or personal injury (including death) arising
out of or attributable to the individual's travel to or participating in the game.
This Assumption of Risk and Release Waiver applies to the Mauston School District and all of its
trustees, officers, directors, managers, administrators, agents, faculty, staff, coaches, students,
volunteers, employees, advisors, and/or representatives.
The undersigned acknowledge that he/she has read and understands this document and agrees to
abide by the rules of the game.
