JUDICIAL AFFIDAVIT OF GERALDINE GUMAGAY

I, GERALDINE GUMAGAY, of legal age, married, Filipino and a resident of # 88
Puguis, La Trinidad, Benguet, state under oath as follows:
PRELIMINARY STATEMENT
The person examining me is Atty. JESSICA BALAWIS with office address at
Room 302 D&L Building KM 5 Pico, La Trinidad, Benguet. The examination is being
held at the same address and is done in English a language known to me. I am
answering her questions fully conscious that I do so under oath and may face criminal
liability for false testimony and perjury.
PURPOSE
This affidavit/testimony is being offered to prove that Transfer of Certificate Title
No. 64071 is recorded in the Office of the Registry of Deeds and that the TCT No.
64071 is in the name of the JULIAN RIZAL FELIPE.
1. Q: Please state your name and other personal circumstances for the
record.
A: I am GERALDINE GUMAGAY, of legal age, married, Filipino and a resident of
#88 Puguis, La Trinidad, Benguet.
1. Q: What is your current profession?
A: I am the Registrar of Deeds of Benguet.
2. Q: How long have you been in that position?
A: I have been the Registrar of Deeds for ten years now.

3. Q: What are the principal functions and duties of your position?

A: As the Head of the Office, I exercise immediate administrative supervision
over all actions and issuances of the personnel of the Registry of Deeds of Benguet.
4. Q: What actions of the personnel do you supervise?

A: I oversee the issuance of decrees of registration and certificates of titles and

registration documents, patents and other land transactions for the benefit of
landowners, Agrarian Reform-beneficiaries and the registering public in general; I
also ensure that the Office provides a secure, stable and trustworthy record of
land ownership and recorded interests therein
5. Q: Is a Certification that a property is recorded in your Office a document that
your Office issues?
A: Yes.
6. Q: If I show you such a document will you be able to identify it?
A: Yes.
7. Q: I am showing to you the document entitled CERTIFICATION, is this
familiar?
A: Yes maam.
8. Q: At the bottom left hand side of this document, whose name is this?
A: Thats my name, maam.
9. Q: There appears to be a signature above the name Atty. Geraldine Gumagay.
Whose signature is this?
A: My signature, maam.
10. Q: What is your purpose why your Office issued this Certification?
A: The CERTIFICATION was issued to attest that the TCT No. 64071 recorded
in this Office is in the name of Mr. Julian Rizal Filipe.
11. Q: I am showing to you a copy of a Transfer Certificate of Title. Is this the
document you are referring to in the Certification?
A: Yes.
