4. STEP-BY-STEP DETAILS OF THE LIFTING OPERATION

1. Prepare a lifting plan to include all the key element and approved by the supervisor and
PTW approved by OIM

2. Prepare a Job Hazards and Risk assessment for the task


3. Conduct the ToolBox Talk (TBT for all the crew involved). Crane Operator must attend
the toolbox meeting

4. Pre use check for the Crane assembly, parts by the crane operator
5. Perform operational check for the Crane by the Crane Operator (boom angle, length,
radius, load chart capacity, limit switch etc,)

6. Conduct pre use check for 2 pieces of 3 tons wire rope slings

7. Examine and determine the weight of the load (1500 kgs)


8. Attached the wire rope sling to the load (pipe) by using double choker hitch at lifting point
at approximately 25% of total length from both end

9. The chocker hitch shall be secured to avoid damage to the wire rope by using a bow
shackles

10. The load shall be attached with 2 taglines placing either direct to the pipe at both end or
at the chocker hitch point by using bowline or clove hitch knots

11. Banksman to communicate to Crane Operator informing that the load is ready to be
lifted

12. The Crane Operator shall swing to the direction of the load following the direction of the
banksman

13. Banksman to communicate by using hand signal to indicate which type of block to be
used (Used whipline or auxiliary block)

14. Banksman to communicate with the Slingers that they are ready for the lift and ensure
everyone is at the correct standing position.

15. Banksman to direct the crane operator to lower down the block by using hand signal
(used hoist/block down hand signal)

16. Once the block is down at approximately chest level, the banksman shall instruct the
Slinger to attach the wire rope slings to the crane hook

17. The banksman then will direct the crane to hoist up slowly until the sling is tension. He
then must confirm that the sling angle and centre of gravity (CoG)

18. Once the CoG is confirmed, banksman to instruct the Crane Operator to hoist up the
load at the same time 2 Slingers to control the load using the taglines

19. Banksman to direct the crane by hoist up signal until the load is 4 metres high. He then
use boom down and hoist up signal to swing until the load is above point B

20. Lower down the load until it properly landed at point B. Slinger to unhook the sling
from the crane hook. Slinger remove the wire rope sling.

21. Perform post use inspection for all lifting equipment to ensure all are free from defects.
Any faulty equipment found must be reported and recorded
