NOMINATION PAPER INSTRUCTIONS

1. Use only BLUE or BLACK ink. Do NOT use pencil.


2. Fill out “County of Signers” in Section A BEFORE anyone signs. All signers must be from this
county. You must be a registered voter in PA & can circulate nomination papers in all counties.
3. Signers MUST ONLY BE REGISTERED VOTERS in Pennsylvania's 15th Congressional
district – 100% of Northampton County, 95% of Lehigh County, parts of Berks and
Montgomery counties (map opposite). Signers can also be from ANY party, and CAN SIGN
EVEN IF they have signed Republican or Democratic petitions for the same office.
4. Signing this nomination paper is NOT promising to vote for Jake or endorsing him, it ONLY
gets Jake on the November general election ballot.
5. EACH SIGNER MUST PERSONALLY fill out Section D themselves.
◦ Signature, printed name, address, and signature should match their voter registration card.
◦ BE CAREFUL WITH THE “City, Boro, or Twp.” line – the postal city may not be
correct, ex. Lower Nazareth Township instead of Nazareth.
◦ The date of signing must include day, month, year. March 10, 2010 or 3/10/10 are OK.
◦ State or ZIP Code is not needed. Inform signers to WRITE THE DATE IN THE LAST
COLUMN as this is a common mistake. Make sure the date is correct after signing.
◦ DO NOT USE DITTO MARKS anywhere.
6. If an error is made, just re-enter the information on the following line below.
7. DO NOT FILL OUT the Page _______ Side X line at the bottom of the page.
8. Each paper has 40 lines for signatures, but 40 signatures is NOT NEEDED for the paper to be
valid. Even 1 signature is enough. You CAN sign your own nomination paper.
9. After a signer finishes, doublecheck the signature to ensure it is LEGIBLE. If not, ask them to
sign again. Always THANK SIGNERS. Always BE POLITE and not argue.
10. When you have finished collecting signatures, you must sign Section E “Affidavit of Qualified
Elector” IN FRONT OF ANY PENNSYLVANIAN NOTARY PUBLIC. Check the Yellow
Pages to find one near you. Contact the campaign at 610-392-8156 or
towneforcongress@gmail.com to arrange pickup or mail the paper to Towne for Congress, PO
Box 22542, Lehigh Valley, PA 18002. The notary will charge a fee less than $5, you can
contact the campaign to reimburse, but must have the receipt.

The last day to circulate (and file) nomination papers is August 2nd, but please obtain signatures as early
as possible to help the campaign secure ballot access. Return all papers to the campaign before 7/23/10.
