1. The Communication Officer is the official PSME-UPSU representative and contact person prior to the
event. If you have questions, clarifications, concerns and other needs, call, text message, email or
contact the person below.

2. Please inform us of a representative of your school, who shall be our point person in the event.
3. We encourage the participating schools to ask questions regarding the procedures

3. After sending the email, notify our above-mentioned Communications Officer via SMS.
4. Please wait for the confirmation from our Communications Officer notifying that your
submissions are complete. Our Communications Officer will also notify you if there are any concerns
with regards to your registration.

PAYMENT PROCEDURE
1. All payments, files and documents shall be submitted ONLINE, unless otherwise stated, only via:

2. All payments shall be made via bank deposit. The following are the registration fee for each
event:
ME Students Convergence : Php 500 per head
Brainmesh Inter-university Quiz show : Php 350 per head
PSME Cup

: Php 120 per head at pre-registration,


Php 80 per sport

4. Fill out the provided excel file, with the name Payment_SCHOOLNAME.xls, be mindful of the
following:

1. Indicate the specific purpose of the transaction (e.g. payment of Cup


registration, shirt pre-order)

2. Please write the information as seen on the receipt


3. Include a scan or picture of the receipt
5. Take note of the time and the branch of the bank where you paid in case of problems in confirming
your transaction.

6. Scan the Deposit slip or a copy of your transaction and send it together with the
requirements of registration.

7. After doing so, inform any of our Communications Officers via SMS.
8. The PSME Registration and Finance Officers will verify your payment. After passing
the verification process, youll be receiving a confirmation e-mail together with your scanned
receipt (soft copy).

9. Make sure to pay a minimum of 75% of the total fees on or before August 24, 2015.
Announcements on extension of payment shall be made, if the organization deem
necessary. Please contact any of our Communications Officers regarding your transactions.
The rest of the payment may be settled on the event. There will be absolutely NO REFUND.

10. The PSME Month 2015 Finance Committee will release the receipts on the day of the event.

REGISTRATION PROCEDURE
1. All pre-registration transactions will be done ONLINE.
2. Final registration will be done before the event.
3. Submit all the requirements together with the corresponding payment (please refer to the
COMMUNICATIONS AND PAYMENT sections for more details) until their given
deadlines.

4. The participating school must send the following registration files and

forms: a. Two (to a maximum of five) photos of your organization/school, your


organizations logo, and school seal

2. A fully accomplished Form 01 to be submitted per participant (PSME-UPSU copy


only)

3. A fully accomplished Form 02 to be submitted per participating


organization/school

4. Roster of the participating students in the respective excel files:

1. Fill-in the initial school identifier row, the school representatives complete
name and contact number, and the facultys complete name and contact
number (where applicable).

2. Reminders regarding the information to be put in the columns:


1. The full name of the participant must be in the following format:
2. Surname, First name, Middle initial
3. The list of participants must be in alphabetical order
4. For PSME Cup, the information of the team captain will be put in the
first row

5. The contact numbers must be in the following format: 09XX - XXX - XXXX
6. Indicate in the Certificate of Enrolment column whether or not the
participant has submitted their scanned copy of the certificate

7. For multiple shirt orders in the shirt pre-order form, separate sizes and
quantity by a comma (ex. S,M,L | 3,1,2 will be read as S-3, M-1, L-2)

8. Highlight the cell of the School Representative with YELLOW and that of
the Faculty Adviser with BLUE (do indicate if the accompanying
faculty/participant is an Engineer or instructor etc.).

1. The information (particularly the name of the participants) must be true and
correct to the tiniest detail in spelling.
ii.Participants must submit READABLE scanned copy of their school registration
form or ID. Failure to submit the said requirement will not affect the approval of
pre-registration. However, the PSME Month 2015 Registration committee is not
liable for any incorrect details (i.e. misspelled name) that may reflect on
participants event certificate since the committee relies only on the participants
event pre-registration form. The committee may print another copy of certificate to
correct such mistake, however Php 50 will be charged to cover the shipping fee.

WALK-INS AND LATE REGISTRATION


A. Walk-in participants will be allowed ONLY if there are still remaining slots after the PreRegistration of the events. The acceptance of walk-in participants will be on a first come,
first served basis. Walk-ins will not be given the discount applied to pre-registered
participants.

2. Given there are no problems in communication and in the processing of the invitation letter and
mechanics; any registration after the given deadline will be considered as Late Registration. Late
registrants will not be given the discount applied to pre-registered participants.
