By our signature/s herein, I/we acknowledge that I/we have read, understood, and fully agree with the UNIONBANK DEPOSIT TERMS AND CONDITIONS with which I/we was/were duly provided, receipt of which I /we confirm by
our signature/s below.
FOR CHECKING ACCOUNTS:
I/We agree and undertake not to use cheques printed or secured from printers not accredited by your Bank and that I/we shall be held responsible and liable for any and all losses/damages arising from violation of this undertaking.
FOR JOINT ACCOUNTS:
1. JOINT "AND" ACCOUNT - the depositors agree and declare that the account is their joint account belonging to and payable to both / all the depositors. UNIONBANK of the Philippines ( the BANK) requires all the signatures
of the depositors in any withdrawal transaction / payment of funds. Upon the death of one of the co-depositors, the legal representative of the deceased co-depositor shall co-sign the withdrawal form with the surviving codepositor(s), in accordance with the rules of settlement of estate of a deceased person, and other related laws, rules and regulations when applicable.
2. JOINT AND /OR ACCOUNT - the depositors agree and declare that the account is their joint account belonging to both / all the depositors and payable to either / any of them as specified in the Joint Account Information
Record. In case one of the co-depositors pre-deceases the other(s), the entire balance of the account shall be payable to the surviving co-depositor(s). The depositors authorize the Bank to recognize either / any of the
signatures of the depositors in withdrawal transactions or fund transfers. Payments to or withdrawals by made by either / any of the depositors shall completely discharge the Bank from any liability.
3. All depositors agree that whomsoever is nominated as Primary Accountholder shall be the primary recipient of all correspondences with regards to the account, and these shall be sent to his mailing address, as specified in
the Customer Information Record. The depositors also agree that this joint account is governed by the provisions and requirements of the Philippine Deposit Insurance Company (PDIC) Bulletin no. 2004-04, which pertains
to the treatment of Joint Deposit Accounts for Purposes of Deposit Insurance and the determination of insured deposits in cases where depositors have obligations with the Closed Bank.
