Equipment and Technology orders must be placed and signed during the

value), subscription fee after no-charge 6 months is $500/month. This does
not include conversion and training, Ascend setup fee of ($995) is waived.
Dentrix Practice Management Software ($6,995) includes up to 10 licenses
and support, training, and conversion are not included. Customer cannot
combine offers.
TERMS AND CONDITIONS: **One EZPay transaction per customer. Sale must
be on one invoice. Subject to approval by Henry Schein Credit Department.
Form of payment must be provided with signed order. Customer must sign
payment authorization for installments to qualify for this promotion. For new
customers & non-established accounts a Social Security number must be
provided in advance. Sales must be made after September 8th, 2015 and
invoiced no later than December 26th, 2015. EZPay is available when your

unused Henry Schein credit line exceeds the amount you are financing. If you

This promotion offers a bundled discount. The cost of any item received at
no additional charge is a discount applied to your other purchases on the
same invoice or purchase order. The reportable value (unit price) of the no
additional charge item will be identified on the invoice or purchase order. To
calculate the applicable discount on the other items on the same invoice or
purchase order, divide the total value of any no additional charge items by
the total value of all other items purchased on the same invoice or purchase
order, and apply the resulting percentage equally to each item to determine
net prices.

Management Software or the 6 month subscription to Ascend rebate
provided under this program is a discount. You are required to comply with all
applicable federal and state laws and regulations, specifically including, but

and any similar applicable state laws. You may be obligated to report and
provide information concerning any discounts, rebates, or other price
reductions provided under this program. You acknowledge your legal
obligations to fully and accurately report the discounts, rebates, and/or other

any other documentation of discounts, rebates, or other price reductions and
make such information available to federal or state health care programs or
other payers upon request.
PRIVILEGES is intended to be a promotional discount program. Award Points
earned on applicable purchases of Henry Schein goods and services
represent discounts on those purchases and, accordingly, are not reportable
under federal Sunshine Act requirements. In addition, earned Award Points
that members later redeem as credits toward additional Henry Schein
purchases are not reportable under the Sunshine Act requirements.
