TITLE: HAZID PROCEDURE - COMPLETIONS

1.0 PURPOSE
This document describes the procedure for conducting a HAZID study on the Gas
Sweetening Facilities Project (GSF) at MESAIEED and DUKHAN during Pre-Commissioning
and Commissioning of the Project.
2.0 SCOPE
This Procedure is intended to be used by QP, Petrofac and their Sub-Contractors involved
in the management of HAZID study covering all activities during the Pre-Commissioning
and Commissioning of the Sulphur Recovery Unit (SRU) at MESAIEED. The scope of this
procedure covers the following steps:
1. Step 1 - Project Scope & Activity
2. Step 2 Hazard Identification Review
3. Step 3 Probability/Credibility and Quantification
4. Step 4 Hazard Classification & Corrective Action
5. Step 5 Record on Worksheet
6. Step 6 Develop JHA, Method Statements and Safe Work Instructions
It explains the methodology followed to identify hazards, assess their consequences and
risks and devise mitigation/corrective actions.
The procedure is not intended as a stand-alone document, but links to other project
HSSE documents. Therefore this procedure should be read in conjunction with the
Project HSSE Plan(s) and associated attachments.

4.0 DEFINITIONS

Hazard

The potential for human injury or loss of life, damage to


the environment or to material assets or a combination of
these.

Risk

A combination of the likelihood of a hazardous event and


the severity of the possible consequences of that hazardous
event.

Incident

An event or chain of events, which has caused or could have


caused personal injury, damage to property or environment.

Hazard Analysis

It is a systematic and quantitative assessment of potential


hazards and its probable consequences.

Likelihood

Indicate the possibility of something happening.

Frequency

Is a rate, e.g. number of incidents per year.

Probability

Is a ratio of an incident to occur (an event) over a possible


number of outcomes.

Risk Assessment

The overall process of risk analysis and risk evaluation.

Risk Evaluation

The process to support management decisions as to


acceptability or risk reduction requirements by comparing
the estimated risk against relevant criteria.

Acceptable Risk

Risk that has been reduced to a level that can be tolerated


by the organization having regard to its legal obligations and
its own.

As Low As
Reasonably
Practicable
(ALARP)

To reduce a risk to a level which is as low as reasonably


practicable involves balancing reduction in risk against the
time, trouble, difficulty and cost of achieving it. This level
represents the point, objectively assessed, at which the
time, trouble, difficulty and cost of further reduction
measures become unreasonably disproportionate to the
additional risk reduction obtained.

Coarse HAZID (Draft) document SHJ-BMS-EN-GDE-S-2453


6.0 HAZID -METHODOLIGY
The HAZID study was conducted as a desk top exercise in accordance with the
Methodology defined in the Coarse HAZID (Draft) document SHJ-BMS-EN-GDE-S-2453.
The following steps outline the Coarse HAZID approach, and are also illustrated in Para
7.0.
This document shall be used and reviewed following site tour to confirm hazards, hazard
classification and corrective actions.
6.1 Step 1 - Project Scope & Activity:
The Project scope and activities in completions shall be confirmed against the Contract
to ensure coverage of the major work elements and complexity.
6.2 Step 2 Hazard Identification Review
Under the hazard groupings of:
Hazard associated with difference in height
Electrical Hazards
Exposure to toxic gases
Exposure to toxic liquids
Corrosive substances
Hydrocarbons presence
Other flammable materials
Noise & Vibration
Exposure to high pressure
Exposure to high temperature
Open flame
Interface potential hazards
Environmental potential hazards
Confined Spaces
Establish a list of potential hazards associated with the scope / activity of work under
review.
6.3 Step 3 Probability/Credibility and Quantification
Each potential hazard shall be reviewed to determine whether it is credible and based on

best judgement.
The significance of the hazard may be different depending on whether safety,
environmental impact, or asset loss are considered.
6.4 Step 4 Hazard Classification & Corrective Action
Each hazard carried forward from Step 2 shall be evaluated to determine:
The potential causes;
The consequence arising from each cause, and
Potential escalation
The corrective actions identify the measures in place (design/procedure) to prevent,
mitigate and/or control the hazard.
The hazards identified are classified as High, Medium or Low risk according to their
degree of potential human impact considering severity and probability as per Qualitative
Risk Assessment Matrix attached in Appendix A.
Hazard areas are identified and colour coded.
High risk
A condition or practice likely to cause permanent disability, loss of
life or body part and/or extensive loss of structure, equipment or
material.

Medium risk
A condition or practice likely to cause serious injury or illness
(resulting in temporary disability) or property damage that is
disruptive, but less severe than Class A.

Low risk
A condition or practice likely to cause minor (non-disabling) injury
or illness or non-disruptive property damage.

6.5 Step 5 Record on Worksheet


Hazard identification findings and output shall be recorded in HAZID worksheet.

Identify the level of administration required to control the risk. The control measures
shall be identified and recorded in the form of task based JHA, Method statements; SWIs
depends on the significance of the risk.
6.6 Step 6 Develop JHA, Method Statements and Safe Work Instructions
Task Based JHA, Method statements and safe work instructions shall be developed from
initial hazards identified in this HAZID exercise.
JHA, Method statements, SWIs shall be communicated to the work groups while
performing the activity.

8.0 DETAILED BREAKDOWN OF CONSEQUENCES:

LOW RISK
( 1- 6)

May be acceptable, however task should be


reviewed to establish if risk can be reduced
further.

MEDIUM RISK
(8 12)

Acceptable but must demonstrate ALARP.


Task should only proceed with appropriate
management authorization after consultation
with specialist personnel and assessment team.
Where possible, the task should be redefined to
take account of the hazards involved or the risk
should be reduced further prior to task
commencement.

HIGH RISK
(15-25)

Task must not proceed. The hazard should be


eliminated or further control measures put in
place to reduce the risk to tolerable level. The
controls should be re-assessed for adequacy
prior to task commencement.

8.1 People
1. Slight Injury/Health effect: Slight/Negligible injury or health implications-no
absence from work. Negligible loss of function (First aide case and Medical
treatment case (MTC) etc. e.g. First aid treatment for minor cuts, headache,
nausea, dizziness, mild rashes etc.
2. Minor Injury/ Health effects: Minor injury or health effects- Affecting work
performance e.g. (restricted work case-WC) or need to take time off work to
recover (Last time incident- LTC). Minor health effects e.g. skin irritation, food
poisoning etc.
3. Major injury/ Health effects: Event leading to a lost time incident (including
Permanent Disability). Affecting work performance in the longer term.
Irreversible health damage without loss of life. E.g. noise induced hearing loss,
chronic back injuries.

4. Single Fatality/ Permanent Total Disability: Single fatality or permanent total


disability from an accident or occupational illness e.g. poisoning, dangerous
infection etc.
5. Multiple Fatalities : Multiple Fatalities from an accident or occupational illness.
8.2 Assets
1. Slight Damage : No disruption to the operation (costs less than US $10,000)
2. Minor Damage : Minor disruption (Costs less than US $ 10,000)
3. Localized Damage : Partial shutdown of operations (Costs up to US$ 10,000 to
US $ 100,000)
4. Major Damage : Major damage (Costs up to US $100K to 500 K)
5. Extensive Damage : Substantial or total loss of operation (Costs more than US $
500K).
8.3 Environment
1. Slight effect: Undetectable or limited local degradation of the environment,
rapidly returning to original state by natural action.
2. Minor effect: Detectable degradation of the environment.

national media. Regional / National policies with potentially restrictive
measures and impact on grant of licenses etc.
5. International Impact: Irreversible degradation in the quality/availability of
habitats or socio economic conditions at local, regional or national level beyond
site boundary.
