1.0 PURPOSE
This document describes the procedure for conducting a HAZID study on the Gas
Sweetening Facilities Project (GSF) at MESAIEED and DUKHAN during Pre-Commissioning
and Commissioning of the Project.
2.0 SCOPE
This Procedure is intended to be used by QP, Petrofac and their Sub-Contractors involved
in the management of HAZID study covering all activities during the Pre-Commissioning
and Commissioning of the Sulphur Recovery Unit (SRU) at MESAIEED. The scope of this
procedure covers the following steps:
1. Step 1 - Project Scope & Activity
2. Step 2 Hazard Identification Review
3. Step 3 Probability/Credibility and Quantification
4. Step 4 Hazard Classification & Corrective Action
5. Step 5 Record on Worksheet
6. Step 6 Develop JHA, Method Statements and Safe Work Instructions
It explains the methodology followed to identify hazards, assess their consequences and
risks and devise mitigation/corrective actions.
The procedure is not intended as a stand-alone document, but links to other project
HSSE documents. Therefore this procedure should be read in conjunction with the
Project HSSE Plan(s) and associated attachments.

The significance of the hazard may be different depending on whether safety,
environmental impact, or asset loss are considered.
6.4 Step 4 Hazard Classification & Corrective Action
Each hazard carried forward from Step 2 shall be evaluated to determine:
The potential causes;
The consequence arising from each cause, and
Potential escalation
The corrective actions identify the measures in place (design/procedure) to prevent,
mitigate and/or control the hazard.
The hazards identified are classified as High, Medium or Low risk according to their
degree of potential human impact considering severity and probability as per Qualitative
Risk Assessment Matrix attached in Appendix A.
Hazard areas are identified and colour coded.
High risk
A condition or practice likely to cause permanent disability, loss of
life or body part and/or extensive loss of structure, equipment or
material.
Medium risk
A condition or practice likely to cause serious injury or illness
(resulting in temporary disability) or property damage that is
disruptive, but less severe than Class A.
Low risk
A condition or practice likely to cause minor (non-disabling) injury
or illness or non-disruptive property damage.

Identify the level of administration required to control the risk. The control measures
shall be identified and recorded in the form of task based JHA, Method statements; SWIs
depends on the significance of the risk.
6.6 Step 6 Develop JHA, Method Statements and Safe Work Instructions
Task Based JHA, Method statements and safe work instructions shall be developed from
initial hazards identified in this HAZID exercise.
JHA, Method statements, SWIs shall be communicated to the work groups while
performing the activity.

8.1 People
1. Slight Injury/Health effect: Slight/Negligible injury or health implications-no
absence from work. Negligible loss of function (First aide case and Medical
treatment case (MTC) etc. e.g. First aid treatment for minor cuts, headache,
nausea, dizziness, mild rashes etc.
2. Minor Injury/ Health effects: Minor injury or health effects- Affecting work
performance e.g. (restricted work case-WC) or need to take time off work to
recover (Last time incident- LTC). Minor health effects e.g. skin irritation, food
poisoning etc.
3. Major injury/ Health effects: Event leading to a lost time incident (including
Permanent Disability). Affecting work performance in the longer term.
Irreversible health damage without loss of life. E.g. noise induced hearing loss,
chronic back injuries.
