Relevant Experience: Previous 5 semesters (of 6 total) in CSU Stanislaus BSN nursing
program, graduation in December 2015.
Purpose: To gain knowledge, skills, and attitudes surrounding quality (and safe) patient
care, hospital policy and protocol as it relates to the nurses role. To prioritize patient
care, identify life-threatening conditions in the Emergency department and to work as a
team player. To transition from student to new-graduate nurse roles.
OBJECTIVES AND ACTIVITIES (EVIDENCE) DESCRIPTION
1. Integrate evidence, clinical judgment, inter-professional perspectives, and
patient preferences in planning, implementing and evaluating outcomes of
care across the lifespan.
a. Complete healthstream training module monthly on topics of interest.
i. Complete the comprehensive test and print the certificate of
completion.
ii. Topics will include, advanced ventilation, antibiotic resistance, and
management of wounds.
b. Utilize research and evidence to support interventions described under the
following objectives (2-8).
2. Apply leadership concepts, skills, and decision making in the provision of
high quality nursing care, healthcare team coordination, and the oversight
and accountability for care delivery in a variety of settings.
a. Function competently within own scope of practice as a member of the
Emergency department team.
b. Engage in leadership by participating in shift report.
c. Write a 1-2 page paper on a clinical scenario on a task that was
inappropriately delegated to you.
3. Function effectively within the inter- and intra-professional teams, fostering
open communication, mutual respect, negotiation, and shared decision
making to achieve quality-nursing care.

a. Engage other members of the healthcare team to ensure safe and reliable
care.
b. Use communication among interdisciplinary teams to provide shared
decision making for patient to provide optimal care.
c. Use SBAR and appropriate pass off report techniques to nurses of
subsequent shifts. Submit a copy in analytical journal monthly.
Recognize the patient, family, and/or population as the source of control and
full partner in providing compassionate and coordinated care based on
respect for human dignity, preferences, cultures, and/or way of life.
a. Provide the patient with sufficient information about their condition and
medical treatment to allow them to be autonomous in their decision
making process.
b. Identify cultural and religious beliefs that may influence the care being
provided and/or possible barriers so that student is able to provide
culturally sensitive patient care.
c. Write 1-2 page paper on Jehovah witness beliefs about medicine and care
while in the hospital.
Describe inter-and intra-professional mechanisms to resolve practice
discrepancies, including ethical, unsafe, and/or illegal care practices.
a. Analyze an ethical dilemma, unsafe and/or illegal care practices
encountered in the clinical setting. The analysis will include a description
of the ethical dilemma, unsafe or illegal care practice concepts involved.
The problem will be described and how it was resolved and what if
anything the student would do differently in the future.
b. Write a 2-3 page paper describing this ethical dilemma.
c. Demonstrate use of ethical decision making in providing nursing care.
Participate in quality and patient safety initiatives, recognizing these are
complex systems issues.
a. Engage in quality improvement/patient safety activities to promote an
understanding of the organizational process, unit application and
evaluation process.
b. Making sure sterile technique is implemented when appropriate (foley
catheter insertion, picc line dressing change).
c. Observe unit for opportunities to improve patient safety and make
suggestions to Preceptor or Charge nurse as appropriate. Address these
changes in analytical journal entry.
Educate patients and families taking into consideration barriers, readiness,
and beliefs as well as mechanisms for evaluation learning in diverse
populations.
a. Informing patient and families about the care, medications, treatments, and
procedures; encourage them to be active participants in their care and to
ask questions and participate with caregivers in the development of their
plan of care.
b. Update whiteboards with patients plan of care so they stay informed
(tests, medications, labs waiting)
