Procedure Guidelines
1. Appraiser will hand over this form to Appraisee for Self Evaluation in Section A (i.e. Performance
Evaluation Section)
2. After Appraisee Completes his self Appraisal, he hands over form to Appraiser, who appraises
employee in Section-A and further fill-up the whole form:

Section B Core competencies Assessment.

Section C Result Areas

Section D Assessment of major Goals and Achievements during the year

3. Appraiser calls Appraisee for a meeting to show how he has appraised, discusses his appraisal with
him, and asks Appraisee to write his comments in Section E. Appraisee then puts his signature at a
place provided as a token of his understanding how he was evaluated.
4. Appraiser then write his overall comments/ recommendation Section F, gives overall rating for
Appraisees Performance during the period of review. He then sends the form to Reviewer.
5. Reviewer goes through all the forms and marks his comments / recommendations in Section G.
(Reviewer may call a meeting with Appraisee / Appraiser, if he feels so.)
6. There is Section I on KRA Planning for the next year. This has to be filled and sent back to HR /
Personnel dept by
7. Once all the sections are completely filled and the Annual Appraisal Report is duly signed, please
forward the same to HR Department for their records / further actions.
8. Subject appraisal review to be done every quarterly, half yearly and yearly.

Notes:
1. Employees joined after will not be covered for Performance Appraisal Report
2. This is performance and competencies evaluation and NOT person evaluation.
