NOTICE OF APPEAL
PLAINTIFF, by counsel and to this Honorable Court
hereby gives notice that he is appealing the adverse decision
of this court dated March 5, 2015, copy of which was served
on plaintiff on March 6, 2015, to the Regional Trial Court, the
same being contrary to the facts and the law. The parties in
this appeal are Plaintiff as appellant and defendant as
appellee.
Plaintiff has paid the appeal fees, per Official Receipt
No. ________, dated __________, copy of which is enclosed.
WHEREFORE, it is respectfully prayed that the records
of the case, together with the original of the Official Receipt
of payment of appeal and other fees, be elevated to the RTC

Money with Damages, Civil Case No. 2014-1, and that such messenger I
served upon the counsel of adverse party and other parties, the motion filed
