1.0 PURPOSE
This procedure outlines the mandatory requirements for confined space entry in order to:
protect all individuals from the hazards of entry into confined spaces.
limit entry to authorize individuals and to define controls necessary to control
hazards when entering confined spaces.
establish minimum requirements for confined space entry at Hyco Site, Kochi, in
accordance with international standards, product owners specifications and other
specifications attached to the contract 0818-000-DOA-MECH2.
2.0 SCOPE
This procedure applies to all onshore personnel working on or within the Hyco Site.
3.0 PREPARATION
Confined space preparation must be completed to prepare the space for entry. This
includes removal of chemical and physical hazards as necessary. Testing for atmospheric
content shall be done. If necessary, ventilate of the space by appropriate method. Initial
entry into confined spaces to test for acceptable entry conditions or to eliminate hazards is
treated as entry into a permit-required confined space. Entry into newly erected vessels
shall be done in accordance with all aspects of this procedure.
Prior to work commencing each day, an effective Task Instruction must take place and be
attended by all those personnel directly involved in the confined space entry, with the
names of all attendees entered on the JSTI form. JSTI meeting is also required when there
is a change in personnel and includes the Job Supervisor and all personnel directly
involved in the confined space entry. If safe entry conditions cannot be maintained, then
immediate evacuation of the confined space must take place.

6.0 PROCEDURE
6.1 Survey of Confined Spaces
A survey shall be conducted thoroughly to identify the hazards related to the entry in to
each confined space and control measures shall be applied. Enter those specific hazards
with control measures on the JSTI form and communicate during the meeting.
For example hazards can be classified as: Atmospheric hazards
Asphyxiating hazards
Toxic atmosphere
Flammable or explosive atmosphere
Engulfment
Mechanical hazards
Electrical shock
Other, such as electric shock, temperature, radiation, noise
6.2 Information Dissemination
All construction and grouting work at confined space is strictly controlled using Safety
Work Permit procedures. These work permit procedures controls the access of people to
the confined space. The methods used for informing exposed persons of the existence and
location and danger posed by the permit spaces are as follows:
The nature of confined space and its hazards shall be made available to all exposed
individuals during JSTI.

A warning signage shall be posted with a caution sign reading "DANGER--PERMIT


REQUIRED CONFINED SPACE--DO NOT ENTER".
6.3 Permit to Work
Before work commences, the Confined Space Entry Permit, together with the Safety Work
Permit must be issued. The Confined Space Entry Permit must be completed by the Entry
Supervisor before entry into a permit required confined space. The Confined Space Entry
Permit is not a safety work permit and alone does not authorize commencement of work.
The permit(s) and all related documents must be in the possession of the Attendant during
confined space entry operations. All personnel must adhere to the job scope outlined in the
Confined Space Entry Permit. Any change in job scope requires the permit and entry to be
suspended and re-evaluated based on the change in job scope. A new or revised permit
must be issued to address the hazards and controls associated with the new job scope.
6.4 Atmospheric Monitoring
Monitors are used in the work area to ensure that safe atmospheric conditions are
maintained. Entry personnel must be evacuated immediately if there is ANY alarm. The
reason for the alarm must be investigated and the Entry Supervisor notified immediately.
No one enters the confined space until the situation is resolved. Atmospheric monitors must
only be operated by personnel that have been trained in the proper operation and
limitations of the instruments. Atmosphere monitoring equipment must have a current
calibration, and be, function-checked before use, after use, and periodically during the work
in accordance with manufacturer's instructions. Technip AGT shall be carried out the Gas
Test and result shall be entered on the log sheet / Permit.
6.5 Gas Monitoring
Testing must be conducted by authorized gas tester to detect potential atmospheric
hazards. Oxygen content bust be between 19.5 21% by volume. Constant O 2 monitoring
is required during permit required confined space entry operations if the initial oxygen
content reading is outside the range 19.521%. No entry is permitted into confined space
containing greater than 23.5% oxygen.
During confined space welding operations, constant atmosphere monitoring must be
conducted for O2 and LEL. Consideration should also be given to constant monitoring for
CO,H2S and other toxics based on the ventilation provided
6.6 Attendant Requirement during Permit Required Confined Space Entry
At all times, a minimum of one hole watcher/attender will be positioned at the entrance to
each confined space to observe the persons working inside and carry their duties as
described in the definitions of "Duties of Authorized Attendants". An Attendant must be

provided to monitor each permit-required space during the period Entrants are in the
permit-required space. One Attendant may not monitor multiple spaces.
The Attendant will be equipped with means of communicating with the Entrants, reserve
lighting, and a means of sounding an alarm if an emergency arises.
6.7 Unauthorized Personnel
There must be a means of ensuring that unauthorized personnel do not enter any
unattended, confined space, i.e., barricade tape and sign postings. The supervisor has to
ensure only authorized personnel are working inside the cordoned area.
6.8 Change of Permit Receiver
For shift change or duty change between Permit Receivers, the Safety Work Permit and the
Confined Space Permit shall be re-authorized by the Permit Issuer at this time and shall be
accepted by the 'oncoming' Permit Receiver.
6.9 Permit Closure
When the work is completed, the Safety Work Permit and the Confined Space Permit shall
be signed off by the Permit Receiver and the status of the work shall be entered on the
Safety Work Permit. Prior to signing off the Safety Work Permit and the Confined Space
Permit, the Permit Issuer or their designee shall visit the work area to verify the status of
the work and that the equipment/work area has been brought to safe condition.
6.10 Training
All personnel involved shall receive appropriate training and retraining in the Confined
Space Entry and shall be assessed to verify their competence against the procedure. Refer
Technip procedure 02.05.33 ("Confined Space Entry").A sticker of such training shall be
stickered on helmet to ensure that all personnel working under the Confined Space Entry
program have been trained and have understood the training. Prior to each entry operation,
the Entry Supervisor shall verify that all participants in the entry have completed Confined
Space Entry training.
6.10.1 Duties of Authorized Entrants:

Know the hazards that may be faced during entry, including information on the
mode, signs or symptom, and consequences of exposure;
Properly use the following equipment:
Testing and monitoring equipment
Ventilating equipment
Communications equipment
Lighting equipment.
Personal protective equipment

Barriers and shields


Equipment such as ladders needed for safe ingress and egress
Log in and out of the confined space.
Communicate with the Attendant as necessary to enable the Attendant to monitor
Entrant status and to enable the Attendant to alert Entrants of the need to evacuate
the space.
Alert the Attendant whenever
a) The Entrant recognizes any warning sign or symptom of exposure to a dangerous
situation;
b) The Entrant detects a prohibited condition
.
Exit from the permit space as quickly as possible whenever;
a) An order to evacuate is given by the Attendant or the Entry Supervisor;
b) The Entrant recognizes any warning sign or symptom of exposure to a dangerous
situation;
c) The Entrant detects a prohibited condition; or
d) An evacuation alarm is activated
6.10.2 Duties of Authorized Attendants
Know the hazards that may be faced during entry, including information on the mode,
signs or symptoms, and consequences of the exposure;
Be aware of possible behavioral effects of hazard exposure in authorized Entrants;
Continuously maintain an accurate count of authorized Entrants in the permit space and
accurately identify who is in the permit space;
Remain outside the permit space during entry operation until relieved by another
attendant or all entrants have exited the space and the entrance has been secured.
Communicate with authorized Entrants as necessary to monitor Entrant status and to alert
entrants of the need to evacuate the space.
6.10.3 Duties of CSE Supervisor
Know the hazards that may be faced during entry, including information on the mode,
signs or symptoms, and consequences of the exposure;
Verify, by checking that the appropriate entries have been made on the Safety Work
Permit and Confined Space Entry Work Permit, that all tests specified by the permits and
entry plan have been conducted and that all procedures and equipment specified by the
permits and entry plan are in place before endorsing the permit and allowing entry to
begin;
Terminate the entry and cancel the permit when;
a) The entry operations covered by the entry permit have been completed; or

b) A condition that is not allowed under the entry permit arises in or near the permit
space.
6.11 Lighting Arrangement
Lighting equipment shall be provided to enable employees to see well enough to work
safely and to exit the space quickly in an emergency. Only 24 V low voltage light system
shall be used inside the confined space. This shall be ensured during pre-entry meeting /
JSTI.
7.0 Confined Space Rescue
All confined space rescue must be carried out in accordance with AP / TP 01.05.34,
Confined Space Rescue Procedure. Site management is responsible for the assurance of
the rescue readiness, and it must be reviewed during the JSTI.
The list with contact number of Emergency Response Team, Hyco Site, Kochi shall be
made available with CSE Supervisor as ready reference, and shall seek assistance in case
of any emergency. Site Emergency Number (99954 22322) shall be known to each entrants
and shall be verified prior entry to Confined Space.
The duty medic shall be alerted prior the entry to the confined space.
If Mechanical lifting devices are required to retrieve entrants from vertical confined spaces,
Respac Structure shall be used.
