SWAPPING AGREEMENT
This agreement is entered into and between MONALIZA M. FLORES, T-II of Sta. Cruz
Elementary School, Gonzaga East District, Division of Cagayan, and SHERWIN R. DELA
CRUZ, T-I of Batangan Elementary School, Gonzaga West District, Division of Cagayan.
That we, both agree to swap places in our teaching assignment for the good of the
service.
That I, MONALIZA M. FLORES, married and residing at Batangan, Gonzaga,
Cagayan, a graduate of Bachelor of Elementary Education (BEED) having passed the LET, and
with 10-year experience as elementary grades teacher at Sta. Cruz Elementary School, do
willingly accept/ reassign teaching assignment at Batangan Elementary School, Gonzaga West
District, Division of Cagayan.
That I, SHERWIN R. DELA CRUZ, single and residing at Batangan, Gonzaga,
Cagayan, graduate of Bachelor of Elementary Education (BEED) having passed the LET, with
new regular item as of July 1, 2015, do willingly accept/ reassign teaching assignment at Sta.
Cruz Elementary School, Gonzaga East District, Division of Cagayan.
We, therefore say, that this agreement is entered into and between us through our own
volition without any coercion from anyone else.

The undersigned would like to seek transfer from her current station, Sta. Cruz
Elementary School, Gonzaga East District, Division of Cagayan to Batangan Elementary School,
Gonzaga West District, Gonzaga, Cagayan for the following reasons:
1. She has 3 children who are presently studying at Batangan Elementary School who
needed motherly care and proper attention.
2. She has been teaching more than 10 years in Sta. Cruz Elementary School.
3. Her residence, Brgy Batangan, Gonzaga, Cagayan, is 25 km away from her current
station, Sta. Cruz Elementary School, and Batangan Elementary School is adjacent to
her residence.
4. She wants to serve her own Barangay to the best of her ability.
