SCHOOL CITY OF HAMMOND SUMMER DAY CAMP PROGRAM PROPOSAL

Program Sponsored by the Special Education Department


For Students With Severe Disabilities and Non-Disabled Peer Mentors
All programs are pending funding and enrollment requirements.

Program

Summer Day Camp For students with Severe Disabilities and General Ed Peer Mentors.
Team must include Gen Ed and Sp Ed Teachers Three aides per teacher Preplanned Community Sites
Base school: to be determined Hours: Usually 9 to 1 daily Dates: To be determined
12 days total.

RULES
You and your team must commit to physical care of students including toileting, changing diapers
and lifting, assisting the health staff with physical care procedures, getting in the water and
assisting students in pool/water activities etc.
2. Your proposal will be evaluated by a team of professional staff per the Hammond Teachers Union
Agreement.
3. Proposal MUST be word processed/typed.
4. Proposal must contain all the components of the protocol.
5. You may not use the same name or proposal you used last year.
6. Your team must include General Ed and Special Ed Teachers.
7. You can have 3 aides per teacher.
8. Staff is expected to fulfill the entire contract.
9. You must find your own aides who will commit to and fully participate in the implementation of
your proposal.
10. The Personnel Applications for teachers and aides must be included in your proposal package.
11. The Day Camp Staff Agreement will be completed at the mandatory unpaid Faculty Meeting
on the 1st Day of Camp.

PROTOCOL FOR WRITING PROPOSALS


1. Course Name: Please state the name of your proposal. Make it SHORT and
catchy. This will be used in publications.
2. Course Description: Please write a course description in 4 or 5 sentences. This
will be used in publications.
3. Grade Level : Please state the level for which this proposal is written:
ELEMENTARY- MIDDLE SCHOOL- HIGH SCHOOL

4. Program Goals: Within the theme of the course description stated above, please describe
the goals/activities for your program. State which Alternative Assessment Language Arts and
Math indicators relate to each Program Goal.
6. Enclose a ten item pre/post test for Math
7. Enclose a ten item pre/post test for Language Arts
8. The Math and Language Arts tests should be on separate pages.
9. Enclose the Summer Applications of all staff.
10. The Staff Agreement and Summer Applications shall be enclosed separately with
the proposal and not stapled to the proposal. These will be removed from the proposal
itself so that the proposal can be evaluated without regard for the authors.
