INSTRUCTION TO FILLUP APPLICATION FORM

1. Kindly affix photos (paste it in the space provided). Office will issue photo ID Card to the members
i.e. one photo ID Card to Proprietor in case of Proprietorship, two photo ID Cards to any two
partners in case of Partnership firm, any two Directors in case of Limited / Private Limited
Companies and elected President and General Secretary in case of Association / Federation.
Please send the two extra photos for issue of Photo ID Cards.
Only one person will be allowed to attedn the AGM, SGM & ECM with photo ID card.

3. Membership Fee :
(a) Ordinary Membership : Admission fee of Rs.2,500/- plus yearly subscription of Rs.3,000/total amount is Rs.5,500/- + applicable Service Tax in force to be remitted along with
application form.
(b) Life Membership : Admission fee of Rs.2,500/- plus one time payment of Rs.30,000/- total
amount is Rs.32,500/- + applicable Service Tax in force to be remitted along with application
form.
(c) Patron Membership : Admission fee of Rs.2,500/- plus one time payment of Rs.10 Lakhs
total amount is Rs.1,02,500/- + applicable Service Tax in force to be remitted along with
application form.
(d) Membership fee (by demand draft/ cheque in favour of Federation of Indian Granite &
Stone Industry payable at Bangalore).
4. The Financial Year is 1st April to 31st March of every year.
5. Furnish all necessary information while submitting this application form and strike out whichever is not
applicable.
6. Enclose required documents as per Para No. 3 of the application form.
7. Executive Committee will have the right to accept or reject the application without assigning any
reason.
8. Attach Separate Sheets wherever required.
9. If you / your company is member of any other organization related to Natural Stone Industry, kindly
mention the same.
10.In case of change of address and telephone numbers, kindly intimate the same at the earliest.
