Rule 1069: Yards

Observation:
The covers are not properly placed and fitted for the ditch and might cause injuries to the
people passing by the area.
Violation:
(3) Ditches, pits, and other hazardous openings shall be provided with adequate covers,
enclosed or surrounded by sufficient guards.
Recommendation:
The yard should be repaired for the safety of all the people that goes to the
workplace or passes by the place.

Rule 1943: Exits

Observation:
The exit in the back part is blocked by various things and is elevated to a height where it
might be inconvenient to leave in emergency cases.
Violation:
(1)At least two exits shall be provided in every floor and basement of every workplace
capable of clearing the work area in five (5) minutes.
(9) No steps or stairs shall be used in horizontal exits. When there is a difference

in

level between cemented floor areas, ramps or inclines of not more than one to ten (1 to
10) slopes shall be installed.
Recommendation:
Another entry/exit should be provided in the workplace so that, in case of
emergency, every person inside the workplace can clear the area in less than 5mins.

1061: Construction and Maintenance:

Observation:
The roofing has holes and whenever it would rain, the machines under it would get wet
and electrical wires are also affected, thus machine operations is put into a halt.
Violation:
(2) Roof shall be of sufficient strength to withstand normal load, typhoons and strong
winds in addition to normal weather conditions and where required to carry suspended
loads.
Recommendation:
The roofs should be repaired so that no water can distract the workers from the
roof holes/leakage caused by rains.

1062: Space Requirement

Observation:
The space in the lathe machine is insufficient for two people to use.
Violation:
(1) Adequate spaces shall be provided between machinery or equipment to allow normal
operation, maintenance or repair and free flow of materials under process or in finished
form Passageways between machinery or equipment shall not be less than 60 cm. (24 in.)
Recommendation:
The machines could be adjusted and the distance between them could be widened
for the convenience and free flow the people using.

1060.01: General Provisions:

Observation:
Only a part of the tool room is arranged, and the rest is not in order and it makes the
finding of any equipment to take too much time, the gas tank is left without markings or
stored in a proper place, the waste bins are in a mess.
Violation:
(4) Good housekeeping shall be maintained at all times through cleanliness of building,
yards, machines, equipment, regular waste disposal, and orderly arrangement of
processes, operations, storage and filing of materials.
Recommendation:
A good housekeeping is good for machine shops so that every equipments, tools
used is in place and every workers could know where all the tools and equipments are
being stored, it can also help make the work faster by having the knowledge to know
where the tools and equipments are. So, arrangements for all machines, tool, equipments,
charts, etc should be orderly arranged.

1075.03: Artificial Lighting:

Observation:
There is sunlight in the welding area, however, it does not suffice the light required to
operate and is too dark. The artificial lighting does not work either.
Violation:
(1) Artificial lighting shall be provided when daylight fails or for area where the daylight
illumination is insufficient.
Recommendation:
Artificial lights should be provided in all rooms, places to where there are
insufficient lights so injury could be avoided.

1082: Eye and Face Protection:

Observation:
There are broken face protection equipments that are in use and not repaired, replaced or
discarded.
Violation:
1082.02: Eye and face protective equipment shall conform to the following minimum
requirements:
(1) Provide adequate protection against the particular hazard for which they are designed
or intended;
(2) Be reasonably comfortable to use;
(3) fit snugly and shall not unduly interfere with the movements of the user;
(4) Be durable, easily cleaned and capable of being disinfected;
(5) Be kept clean and in good condition, and
(6) Be of the approved type.
Recommendation:
Damaged goggles, face masks, and welding masks should be disposed off
immediately and replaced or repaired.
