Subject: SUICIDE NOTE Indian Oil WhistleBlower

1. I was the dealer of INDIAN OIL CORPORATION LIMITED at Uttiramerur, Kanchipuram

District, The A site Retail Outlet as per lease 5440 sq ft, NOC for 6498 sq ft and was built
in violation of the norms in 6756 sq ft and most important 2 out of 3 tanks were placed
on sand instead of placing tanks in Mat raft Concrete Bed as per the norms of the No
Objection Certificate Issue by District Revenue Officer and Explosives License issued
by Explosives department resulted in 3 Pipe line leaks, 3 fire accidents, 4+ holes in the
pipe line, tank tilts (placed against Law of Gravitation) and loss of over 8000 lts of fuel
endangering the public, the recording of loss (2977 lts of fuel) in 10.7.2008 inspection
report is missing from Indian oil (having replied Yes in RTI, but confirmed in RTI visit on
17.9.2014 to IOC) only to deny the infrastructure failure, IOC failed to correct the default.

2. Indian oil citing the loss of fuel due to tank tilts recorded the reasons agreed upon
Resitement of my dealership vide Minutes of Meeting (MoM) dated 30.12.2009 and again
National Commission for Scheduled Caste (NCSC) on 11.10.2010 advised Resitement of
the dealership both cited TANK TILTS, was rejected.
3. As a WHISTLE BLOWER I Bring it to Your Goodself Notice, improper installation of
tanks, a violation of NOC Norms, Violation of Petroleum Rules 130, PROFORMA OF
CERTIFICATE OF SAFETY which can cause devastation to the PUBLIC Safety, Indian
oil didnt rectify it and forced a SC dealer to operate in violation for loss, to establish
that the pump is fit for operations sales Officer ASR.Sekhar and V.Ramaswamy
destroyed the inspection report record for 3,13,000 of supplied fuel and 2977 Lts of
loss of fuel, THE SALES OFFICER IS DENYING HIS VISIT ITSELF AND INSPECTION,
thereby placed in danger, forced to violate law and operate even without closing the
pipelines which had 4+ holes in it, Indian oil directors paid no attention to correct
the violation.

4. Indian oil misled the NCSC and MoPNG Intervention in 2011 and now President, Prime
Minister, MoPNG, NCSC, TN Governor have intervened on my Complaint against Chairman
B.Ashok and Indian Oil, and in last 8 Years I have Proved
(a) Having leased only 5440 sq ft, got NOC for 6498 sq ft and Constructed RO in 6756 sq ft.
(b) The Initial Explosives License where the Matraft Foundation design was explicitly drawn
and was approved, NOC and Explosives License is for Mat Raft Tank Foundation
(c) The Work Order No: 17494070 dt 20.11.2006, Issued by Indian Oil for Strip Raft Tank
Foundation for 2* 20Kl tanks and Placed 15 Kl Tank in Mat Raft Tank Foundation
(d) Instead of relaying the 2 diesel tanks for 4 Lakhs, Indian Oil is paying 2 Securities Rs
35000 per month and have paid Rs 13, 30,000 till date.
(e) RTI Records of Indian Oil Proves that there is no records (Inspection dt 10.7.2008) for 3
months, 3130000 of fuel supplied which is due to Mismanagement of Indian Oil officials.
(f) I was Terminated for not selling committed volume of fuel, without relaying the tanks and
(g) No norms to issue NOC, the State Highway norms need a minimum frontage of 100 feets
since the Road was upgraded from Major District Road to SH-118.
5. The Officials of Indian Oil Debangshu Ray, a Director Designate was suspended in 2014
due to Irregularity (Leaking of sensitive information on Oil Pricing) and Mohammed
Sirajuddin Arrested for IS Links in 2015, Indian Oil encouraged and cultivates the wrong
doers like ASR.Sekhar (Sales Officer who is denying his visit, in spite of His Tour Program
Approved and TA Paid to Him) and V.Ramaswamy (who helped destroy my Inspection
Report) to deny the Visit to a Retail Outlet only to Hide Indian Oil Chairman and other
Officials lapses and loss of nearly 2977 lts of fuel which will be hazardous to the Public.

6. The No Objection Certificate to the retail Outlet was CANCELLED due to violation of
Highways norms and land size difference by the District Revenue officer, Kanchipuram
vide order dated 19.9.2011. and again after High Court direction, The Government of
Tamilnadu have passed order dated 5th Dec 2014 DENYING NOC and also recorded

Violation of Proforma of Certificate of Safety, Violation of Petroleum Rules
7. Its my private battle for the safety of the Customers, THE ABOVE MENTIONED DRO
ORDER CONFIRM IT ALL, Hence for the SAFETY OF THE PUBLIC, to reveal the TRUTH,
Inspection Report is an official, Public record and EVIDENCE of the defaults, remembering
Manjunath Shanmugam who was brutally murdered during a Inspection, and The
Individual ASR.Sekhar decision to deny his Visit(its learnt that July 14th 2015 ED, TNSO
had a meeting with ASR.Sekhar and have questioned why he is denying his visit) need to
be proved as his actions have put the entire machinery to deny loss of fuel, thus failed to
relay the tanks, a corrective action, MoPNG Just need few Hours and the will to Request
Indian Oil to present.

a) ASR.Sekhar is denying his Visit to the pump, which can be confirmed by the Tour
Program Approval and Travel Allowance claim and the Payments will prove his
Inspection on 10.7.2008.

b) Analyzing the Hand writing expert to confirm the handwritten work sheet of SO
ASR.Sekhar used on 10.7.2008,nearly 3000 lts of loss.

c)

Mr,KB.Thiruvelan, Manager RS who did subsequent inspection on 16.7.2008 using

the same closing reading have confirmed the availability (Enclosed the Audio file).

d) The Former CDRSM V.Ramaswamy, CUSTODIAN OF THE CHENNAI OFFICE for

records of supply and sales, Monitoring and Implementation of the Petroleum Rules
and Explosives Rules, lawfully violated the Rule 130 will reveal why he didnt deny
the inspection on 10.7.2008, in his reply to 1st Dec report and 22nd Mar 2010 reports,
and the 18th Dec 2008 Officers meeting (Audio Recording will confirm SO Visit and
confirm he destroyed the record).

8. Iam sure iam writing this letter with great hope to your Goodself, I have completed the
Arbitration after 1128 days Before B S Giridhar, Internal Arbitrator and ED(H,S&E), hence
I request MoPNG to stop encouraging Indian Oil by not acting on my complaints and just
passing on my complaints to Indian oil again, Its time the Ministry need to accede to my
LAST request, My Retail Outlet is said to be the FIRST RETAIL OUTLET AMONG 24,000
RETAIL OUTLETS OF INDIAN OIL TO BE DENIED NOC FOR SAFETY VIOLATION, Indian
Oil Terminated me for not selling committed volume of Fuel thereby willfully forced me to
Place Public in Danger, does this ministry ignore from its duties, SAFETY is a RIGHT and
NOT PRIVILEGE, I Remind Ministry cant perform a Postman job by forwarding my request
to Indian Oil, Its the Responsibility of Ministry to ensure Constitutional obligation as well
apart from
Supply distribution, Marketing and pricing of petroleum products and
Administration of Petroleum Act, 1934 and Petroleum Rules.
9. Its the Duty of the Ministry, the Authority for corrective actions in the Interest of the
Safety, have few minutes for a SC dealer, was denied of fundamental rights, forced to
violate all relevant laws, all these years, Fighting Indian Oil for the safety of the Public, You
cant just watch as a mute spectator with no respect for any intervention, Unlike Rahul
Vemula, SC Scholar I dont want to Die with a CLEAN NOTE accusing anyone, This is my
Suicide Note, indict ASR.Sekhar, (the Other Person name will be revealed by April 15th
2016) by now bringing your notice, I request your goodself to Render Justice or my Death
can bring in the desired Justice to me, I may be allowed to meet Your Goodself in Person to
appease my case with 2 Audio Recordings and others evidences to Confirm Sales Officer
Visit.
