    Job Description

    Psychiatric Consultant

    JOBSUMMARY
    Theconsultingpsychiatristisresponsibleforsupportingbehavioralhealthcareprovidedinprimarycaresettings
    byateamcomprisedofprimarycareandbehavioralhealthproviders.

    DUTIESANDRESPONSIBILITIES
    1. Provideregularlyscheduled(usuallyweekly)caseloadconsultationtobehavioralhealthcaremanagers
    (CMs).Theseconsultationsaretypicallyconductedbytelephoneandfocusprimarilyonpatientswhoare
    newtotreatmentorwhoarenotimprovingasexpected.
    2. Provideoccasionaltelephonicconsultationtoprimarycareproviders(PCPs)asneeded,focusingonpatients
    intheCMscaseload.
    3. WorkwithassignedCMstotrackandoverseetheirpatientpanelsandclinicaloutcomesusinganelectronic
    registryorothertypeofsystemcapableoftrackingclinicalprocessesandpatientoutcomes.
    4. Suggesttreatmentplanchanges,includingmedicationrecommendationsforpatientswhoarenotimproving
    asexpected.
    5. Discusspatientswhoneedreferralforadditionalspecialtymentalhealthcare(e.g.,toacommunitymental
    healthcenter)andadviseontreatmentplansuntilpatientsareengagedinsuchcare.
    6. Useapopulationfocusedregistrytodocumentrecommendationsfortreatmentand/orreferralswithin24
    hoursofconsultingwithaCMsothattheycanbeeasilysharedwithPCPsandothertreatingproviders.
    7. ClearlycommunicatetoCMsandPCPsthelimitationsoftheconsultationandtreatmentrecommendations
    ifyoudidnotevaluatetheclientinperson.Includethefollowingdisclaimerstatementacknowledgingthese
    limitationsinallconsultnotes:
    o Theabovetreatmentconsiderationsandsuggestionsarebasedonconsultationwiththepatient'scare
    managerandareviewofinformationavailableinregistry.Ihavenotpersonallyexaminedthepatient.
    Allrecommendationsshouldbeimplementedwithconsiderationofthepatient'srelevantpriorhistory
    andcurrentclinicalstatus.Pleasefeelfreetocallmewithanyquestionsaboutthecareofthispatient.
    8. MaintainprofessionalcellphoneandEmailaccountsforcontactduringusualbusinesshours.
    9. RespondtotelephonecallsfromprimarycareprovidersandCMswithinonebusinessday.Respondto
    urgenttelephonecallswithinonehour,ifavailable.
    10. CheckprofessionalEmailaccountdaily.RespondtoEmailquestions/consultationswithintwobusinessdays,
    soonerifurgent.
    11. Coordinatewithotherconsultingpsychiatristsforvacationcoverage.
    12. Whenpossible,visiteachparticipatingclinicatleastoncewheninitiatinganewconsultingrelationshipand
    thenatleastonceperyeartomeetclinicprovidersanddiscussongoingcollaboration.

    TYPICALWORKLOAD
    Atypicalworkloadforaconsultingpsychiatristincludesthefollowing:

    1. Caseloadfocusedconsultation(3hours/designatedCM*/week).
    Thisincludesregular(weekly)consultationwiththeassignedCM,preparationforconsultationand
    documentationofrecommendationsintheregistry.Thismayalsoincludeconsultationaboutpatientswith
    PCPs.Theseconsultationsmaybeperformedinpersonorbytelephone/video.

    *AfulltimeCMmayhaveacaseloadof50100patients.3hourswillbedesignatedtoeachCMwhois
    workingat0.5orgreaterFTE.TheconsultationtimeisadjustedandallocationsareproratedforCMswhowork
    smallerFTEs(e.g.,0.4FTE).


    OPTIONALACTIVITIES

    1. Directevaluationofpatients
    Directevaluationofpatients,focusingonclientswithdiagnosticortherapeuticchallengeswhoareidentified
    indiscussionwiththepatientsCMand/orPCP.Suchconsultationmaybeprovidedinpersonorvia
    telemedicine(videoortelephone).

    2. Training
    Thismayinvolvedevelopmentanddeliveryofinservicetrainingforprimarycarebasedprovidersandstaff
    regardingcurrentunderstandingofbestpracticesfortherecognitionandtreatmentofbehavioralhealth
    conditionsinprimarycare.

    REQUIREMENTS

    Licensedpsychiatristorpsychiatricnursepractitioner.
    Demonstratedabilitytocollaborateeffectivelyinateamsetting.
    Willingnesstomaketreatmentrecommendationstoprimarycareteamforpatientsthatconsultantdoesnot
    seedirectly.
    Excellentcommunicationskills.
    Positive,flexible,andsolutionfocusedattitude.
    Abilitytoquicklysynthesizemedicalandpsychiatricdataandformulateeffectiveandevidencebasedclinical
    recommendations.

    DESIRED

    Boardeligibleinpsychiatry.
    Experiencewithprimarycarepsychiatryorconsultationliaisonpsychiatry.
